Measures to decrease the risk of acquired immunodeficiency syndrome transmission by blood transfusion. Evidence of volunteer blood donor cooperation.
We studied whether volunteers giving blood to the Greater New York Blood Program (GNYBP) cooperated with procedures implementing public health recommendations intended to decrease the risk of acquired immunodeficiency syndrome (AIDS) transmission by blood transfusion. Predonation medical screening was expanded to exclude donors who might be ill with AIDS. To exclude possible asymptomatic carriers of the disease, members of groups at increased risk of AIDS were asked either not to give blood or to give it for laboratory studies. A confidential questionnaire, administered to all donors after medical screening, provided the vehicle for donors to advise the GNYBP whether their donation was for laboratory studies or for patient transfusion. We found that the number of male donors decreased; AIDS-related questions in medical history led to a 2 percent increase in donor rejections; 97 percent of donors said their blood could be used for transfusions; 1.4 percent said their blood could be used for laboratory studies only; and 1.6 percent did not respond. Only units designated for transfusion were released to hospitals. People who indicated that their donation was for laboratory studies had a higher prevalence of markers for hepatitis B virus and of antibodies to cytomegalovirus. White cell counts and helper/suppressor T lymphocyte ratios were not significantly different in the two groups. We conclude that volunteer donors have cooperated with the established procedures. None of the laboratory assays identified blood units donated by individuals who, based on information about AIDS high-risk groups, designated their donation for laboratory studies.
In vitro susceptibility of fungi to acyclic inhibitors of 2,3-oxidosqualene cyclases.
In the present study we determine the antifungal properties of two acyclic inhibitors of 2,3-oxidosqualene cyclases: 22,23-epoxy-2-aza-2,3-dihydrosqualene (EAS) and azasqualene alcohol (ASA). Fungistatic and fungicidal activity towards dermatophytes and other fungi involved in cutaneous and systemic infections was tested (48 isolates from 10 species). The tests were carried out by inoculating 10 microliters of mycelial homogenate in 1 ml of Sabouraud glucose liquid medium containing serial dilutions of 100 to 0.25 micrograms ml-1 of the substance. For each isolate, the minimum inhibitory concentration (MIC) and the minimum fungicidal concentration (MFC) of both compounds were determined. EAS was more active (MIC range 1.5-25 micrograms ml-1) than ASA (MIC range 3-50 micrograms ml-1). At the highest concentration tested, EAS also showed fungicidal action towards some isolates of Trichophyton mentagrophytes, T. terrestre, Epidermophyton floccosum, Microsporum canis and Scopulariopsis brumptii. The most sensitive species was T. mentagrophytes, the most resistant T. rubrum.
A truncated form of teratocarcinoma-derived growth factor-1 (cripto-1) mRNA expressed in human colon carcinoma cell lines and tumors.
The human teratocarcinoma-derived growth factor (TDGF)-1 gene encodes a 188-amino acid protein, cripto-1. The TDGF-1 gene is overexpressed in the majority of human primary colorectal carcinomas and hepatic metastases, in breast carcinomas and in testicular nonseminoma germ cell embryonal carcinomas. In the human embryonal carcinoma cell line NTERA-2 clone D1, a 2-kb TDGF-1 mRNA transcript is expressed. The present study shows that a 1.7-kb mRNA transcript lacking the first two exons of the TDGF-1 gene is expressed in the human colon carcinoma cell line GEO. This shorter mRNA is the only TDGF-1 transcript that is present in the majority of primary human colorectal carcinomas and hepatic metastases and in adult human tissues such as the pancreas, heart, stomach, mammary gland, skeletal muscle, liver and placenta. In contrast, in the kidney, brain, testis, ovary and spleen, the longer 2-kb TDGF-1 mRNA transcript is expressed. The putative shorter protein starts at a CUG codon 129 nucleotides downstream of the starting AUG codon of the longer protein. These data indicate the potential for differential transcriptional regulation of the TDGF-1 gene in different normal and tumor tissues.
The first breast cancer screening program in southern Italy: preliminary results from three municipalities of the Naples Province.
AIMS AND BACKGROUND: It has been demonstrated that breast cancer screening induces a 30% reduction of specific mortality. In May 1990, we started a pilot screening program to assess the feasibility of carrying out such a program in Campania (southern Italy). Herein we report the results of the first round of the program from three municipalities (Giugliano, Mugnano and Qualiano) that lie within the local health district no. 23, close to the city of Naples. METHODS: Women between the ages of 50 and 69 years were sent a personalized letter inviting them to attend the screening test; those not responding were sent a second invitation. The screening test consisted of clinical examination followed by two-view mammography. Second-level diagnostic tools were sonography, fine needle aspiration (manual, echo-guided and stereotaxic) and surgical biopsy. RESULTS: Out of 5,732 women invited for the first round, 1,813 (31.6%) attended the screening. Attendance rate was higher among younger women. Ninety-one women were positive at the screening test and underwent further examination (recall rate, 5.0%). Among them, 19 had surgical biopsy (biopsy rate, 1.0%) that led to breast cancer diagnosis in 11 cases. The benign/malignant biopsy rate was 0.73. Detection rate was 6.07 x 1,000 screened women and varied among age categories, increasing within the 60-69 subgroup; detection rate/expected incidence ratio in the overall group was 4.5 and also increased within the older age category. Seven out of 11 cancers were at UICC stage O-I. Among 327 self-referring women, 38 were positive (recall rate, 11.6%), and 14 underwent biopsy (biopsy rate, 4.3%), which showed cancer in 7 cases (benign/malignant biopsy rate, 1.0). In addition, 2 inflammatory cancers were diagnosed without surgical biopsy. Thus 9 cancer cases were detected in this group. Self-referring women differed from responding women in that they had a higher frequency of symptoms or familiar history of cancer, and a higher educational level and awareness of preventive medicine. Clinical examination added no diagnostic advantage in the responding group but did not significantly worsen the recall rate. In the self-referring group, one case of inflammatory cancer was missed by mammography and diagnosed by clinical examination. CONCLUSION: The early results (recall rate = 5%, detection rate/expected incidence ratio = 4.5, benign/malignant biopsy rate = 0.73, advanced cancers = 36.4%) are encouraging and indicate the validity of the program. Strategies to improve attendance rate are planned.
Hepatitis testing.
There has been remarkable progress in hepatitis testing in recent years. This article reviews the transmission of Hepatitis A by blood products indicating that it is a rare event. However, it has been recently observed with certain preparations of Factor VIII concentrate. Hepatitis B transmission has been effectively reduced by the application of HBsAg screening tests, and screening for antibodies to HBcAb. Hepatitis C transmission has been substantially reduced by the used of the second generation tests for antibodies to HCV. A new generation of screening tests for HCV is expected to be licensed for use in the United States in the near future, and should further reduce the risk of transmission of HCV. At the present time, the incidence of post-transfusion hepatitis in hospitalized patients is indistinguishable from the incidence of hepatitis among patients who did not receive blood transfusion.
[Nephroblastoma and polycystic dysplastic kidney].
The Authors pass on the case of a nephroblastoma, associated to Wilms Tumorlet and combined nephroblastomatosis, arose on a multicystic dysplastic kidney. They examine the relationships between the nephroblastoma and the kidney malformations, the possibility of malignant degeneration of a multicystic dysplastic kidney and the necessity of the nephrectomy as a prevention of the degeneration. At the moment the statistic data don't justify the nephrectomy a the birth to prevent the arising of a nephroblastoma.
[Femoral hernia in childhood: a rare entity].
The Authors present 4 cases of femoral hernia treated in their pediatric surgery department during the last 11 years. Childhood femoral hernia is rare and often missed clinically because of difficulty in eliciting their clinical signs. During an operation for inguinal hernia it is essential to think of femoral hernia when a non obliterated processus vaginalis is not found.
[Primary bladder diverticula in children: report of 15 cases].
The Authors report their experience in primitive bladder diverticula treatment during the last six years. They identify criteria for a general classification of this clinical entity and limited to primitive forms they divide in 4 groups. This classification is related to clinical and topographycal criteria, aiming to delineate a new diagnostic and therapeutic pathway.
Antibiotic prophylaxis for biliary tract surgery: selection of patient and agent.
One hundred and three patients undergoing biliary surgery have been entered in a randomized prospective trial for antibiotic prophylaxis. Fifty six received piperacillin (2 gr) and 47 tobramycin (100 mg). Cultures were taken from bile at operation and a clinical and microbiological assessment for infection was carried out postoperatively. Overall infection rate was significantly lower in the piperacillin group (P less than 0.05). The positive bile culture and wound infection rates were also lower in the piperacillin group, though this did not reach statistical significance. Anaerobic bacteria (three patients) and Streptococcus faecalis (one patient) were isolated in the tobramycin group. Overall postoperative infection rate was similar in patients with risk factors for infection and in those without. In conclusion, piperacillin did not significantly improve bile and wound infection rates compared to tobramycin. However piperacillin showed a more appropriate spectrum of activity. Antibiotic prophylaxis is suggested for all patients undergoing biliary surgery.
[Transcranial magnetic stimulation of the facial nerve: evaluation of a new method in neurophysiological study of Bell's palsy].
Transcranial magnetic stimulation is a new technique used to stimulate brain areas as well as peripheral nerves in healthy, waking persons. To date this technique has appeared safe. The aim of the present study was to assess the clinical application of this method in patients with Bell's palsy. Electromyographic responses were elicited by electrical and magnetic transcranial stimulation of the facial nerve in 26 patients affected by Bell's palsy. Electrical stimulation: stimuli of 0.1 ms duration, and up to 15 V, were delivered through surface electrodes set 2.5 cm apart over the facial nerve at the stylomastoid foramen. Magnetic stimulation: the coil was placed tangent to the parieto-occipital surface of the scalp. The stimulus intensity was then increased stepwise until a supramaximal response was obtained. Recording: the focal recording electrode was placed ipsilateral to the side stimulated over the superior orbicularis oculi; the reference electrode was placed over the nasal bone. The patients were tested with two neurophysiological determinations: the first 15-30 days from the onset of the palsy; the second 30-60 days after the first. A clinical follow-up was performed six months after the second determination. The results indicate that, contrary to traditional electroneurography, transcranial magnetic stimulation is not able to supply useful neurophysiological indications in patients with Bell's palsy. Although the absence of compound muscle action potential upon stimulation of the side with the lesion did demonstrate some impediment to conduction, it did not have any prognostic value since it was also present in patients who were clinically well at the time of the second check-up.
