Electron spin resonance studies on normal human uterus and cervix and on benign and malignant uterine tumors.

Electron spin resonance (ESR) studies at -130 degrees have been made on frozen samples of normal human cervix and uterus and on frozen samples of various pathological conditions of the cervix and uterus including fibroleiomyoma and carcinoma. Fifty-five samples of normal cervix and endometrium, 40 samples of nonmalignant disturbances, 15 benign tumor samples, and 20 malignant samples were studied. Very strong ESR signals were seen in frozen powders and frozen intact samples of normal cervix and endometrium and in nonmalignant gynecological conditions. In many cases, the ESR signal was greatly decreased or even undetectable in cancer samples. The substance(s) responsible for the ESR signal in frozen intact tissue (g = 2.11 to 2.15) is decreased in concentration when the sample is ground to powder under liquid nitrogen, and an anisotropic signal (g = 2.002 to 2.035) then becomes much more evident. The ESR signals in intact and in powder samples are sensitive to temperature variations; the signals disappear around 0 degrees, and only the intact samples show significant recovery of signal on recooling. The anisotropic g values and temperature sensitivity in the powders may result from an organic peroxy radical that is more strongly associated with a metal ion in intact samples.

Thyroid hormones in tissues from human embryos and fetuses.

This study was intended to quantify T3 and T4 in various human tissues at different stages of gestation as a contribute in the evaluation of the role of thyroid hormones in fetal development, particularly before the maturation of fetal thyroid function. Moreover, for a better comprehension of the influence of thyroid hormone status in tissues, the study was extended to adults. Embryonic specimens were obtained from voluntary abortions between 6 and 12 weeks of gestation, fetal and neonatal specimens from fetuses and neonates between 15 and 36 weeks of gestation after spontaneous abortion or stillbirth, and adult specimens from men (age range: 45-65 years) after death for cardiovascular diseases. Thyroid hormones were measured by the method of Gordon and coworkers. In embryos T3 and T4 were measured in limbs, carcasses, brain and liver: considering all values measured in the period 9-12 weeks, a mean concentration of 0.11 ng/g for T3 and 1.28 ng/g for T4 was obtained. In pooled limbs of 6-8 weeks T3 was barely measurable (0.01 ng/g). In the carcasses there was an increase in T3 and T4 concentrations of 40 and 20 times respectively from the 9th to the 12th week, when thyroid follicles organization takes place. In fetuses and adults T3 and T4 were measured in brain, heart, kidney, liver, lung, skeletal muscle and skin (mean concentrations: 0.86 ng/g for T3 and 7.44 ng/g for T4 in fetuses and neonates; 1.36 ng/g for T3 and 12.75 ng/g for T4 in adults). Hormones concentration increased with gestational age; the T3/T4 ratio increased until 22-24 weeks, when the prevalent increment in T4 occurs. T3 concentration up to 30 weeks was generally higher in tissues than in cord serum of the corresponding age. During the last month of gestation T3 increment was faster in serum. T4 level was always predominant in serum. In conclusion, T3 and T4 have been detected in the limbs of embryos before the onset of thyroid hormone secretion. Concentrations were 1/150 and 1/70, of the normal maternal blood values respectively. It is conceivable that these hormones are of maternal origin, and the question of whether such small quantities may play a role in fetal development is open.

Cortisol in human tissues at different stages of life.

Aim of the work was to measure the cortisol level in human tissues at different stages of life, by means of radioimmunoassay and by chromatography. Viable samples of 13 different tissues were obtained during surgical intervention from 30 to 70 years old patients of either sex. Mean tissue cortisol concentration was 78 +/- 35 ng/g, ranging from 20 +/- 10 ng/g in the thyroid to 124 +/- 76 ng/g in the kidney. Similar values were measured in the corresponding tissues from not decayed corpses, so that paired values could be mediated. However the pancreas, and corrupted autopsy tissues, gave nil or exceedingly high cortisol concentration values; in some cases, opposite extreme values were measured in different organs of the same body. Cortisol concentration was also measured in 11 sound different tissues of spontaneously aborted or stillbirth fetuses, between 16 and 36 weeks of gestation. Mean value was 63 +/- 27 ng/g, ranging from 30 +/- 25 ng/g in the liver to 104 +/- 52 ng/g in the lungs. Also in fetuses nil or exceedingly high cortisol values occurred in altered tissues. One hundred and fourteen samples of limbs and carcasses of 7 to 12 gestational weeks embryos, obtained from voluntary abortions, were also examined: 20% gave nil result, in the remaining mean cortisol concentration was 32 ng/g. In 33 samples of embryos' mixed viscera, RIA and chromatography gave unreliable exceedingly high values. The nil and the exceedingly high values measured in the altered autoptic tissue specimens were inconsistent with the cortisol blood level measured in the patients, as were those measured in embryonic tissues with the acknowledged blood and adrenals cortisol levels at that stage of life. Thus cortisol may be measured by RIA and by chromatography in sound tissues, while the values obtained in the pancreas, in corrupted tissues, and in embryonal viscera do not represent the hormonal milieu, but are likely artifacts due to impeachment of the diagnostic system.

[The long-term results of mechanical anal sphincteroclasis].

BACKGROUND: The long-term results of 36 patients operated of manual dilatation of the anus are presented. Chronic anal fissure or benign anal stricture have been treated by this procedure in the years '80s. METHODS: Patients have been in part clinically examined and the other called up by phone interview. Average follow-up is 78 months. RESULTS: Nobody has been re-operated for the same cause, but 2 patients underwent sphincter sparing operation for rectal cancer. Twenty-five patients (72%) completely recovered, 5 (12.5%) complain sometimes from sudden anal pain and 3 (8%) report minor incontinence problems. CONCLUSIONS: These long-term results do not confirm disastrous conclusions reported by other authors in the literature, even if the incidence of continence disorders is higher than in anal sphincterotomies.

[Dexketoprofen trometamol in the treatment of acute migraine attack].

BACKGROUND: Migraine is a common, very disabling pain condition: the occurrence of an acute attack often requires bedrest and prevents from participating in normal activities. Nonsteroidal anti-inflammatory drugs are widely used in the symptomatic treatment of migraine. Aim of this study was to evaluate the efficacy and safety of a potent new formulation, dexketoprofen trometamol (DT), in the treatment of migraine pain. METHODS: The study enrollment was 42 women (mean age 37.2 +/- 12.2, range 21-65) suffering from migraine, with (n = 6) and without (n = 36) aura, diagnosed according to the International Headache Society criteria. Patients were asked to treat a single attack with a 25 mg dose of DT. The primary efficacy criterion was the change in migraine pain, recorded on a Visual Analogue Scale (VAS) immediately before and at 30, 60, 120, 240 minutes after assumption. The presence of accompanying symptoms and the onset of side effects were also considered. RESULTS: Pain intensity significantly decreased after drug intake, showing a tendency toward a reduction along the time. The drop in mean VAS values resulted already significant at 30 minutes after DT assumption. Also the reported incidence of accompanying symptoms (nausea, vomiting, photophobia and phonophobia) significantly decreased after treatment. No serious adverse event was reported during the study. All the adverse events recorded were of mild severity and did not require any specific medical treatment. CONCLUSIONS: DT proved to be an effective and safe oral therapy for the treatment of migraine attack.

[Psychosomatic assessment in patients with gravidic hyperemesis].

Thirty female subjects examined in the first three months of pregnancy took part in the study. Ten of these suffered from severe gravidic hyperemesis, while twenty had a physiological pregnancy. All subjects with hyperemesis and six controls were subjected to measurement of cortisolaemia at the following times: 8, 12, 16, 20, 24 and 4.00 so as to obtain the circadian curve of the hormone, measurement of T3 and T4 and of TSH. The Cattell test in form C was carried out in all patients. The results obtained evidenced a global increase in the production of cortisol in the group of hyperemetic women, with maintenance of the circadian rhythm. The Cattell test identified characteristic personality traits in the subjects suffering from hyperemesis compared to the controls.

Elevated plasma levels of platelet-activating factor (PAF) in breast cancer patients with hypercalcemia.

Peripheral plasma concentrations of PAF were measured in patients with benign and malignant tumors of the breast, and in healthy female controls with a very sensitive PAF assay. The mean values +/- SEM were significantly higher in the malignant group than in healthy controls (1070 +/- 111 fmol/ml vs. 142 +/- 12 fmol/ml, P less than 0.01) and patients with benign tumors of the breast (1070 +/- 111 fmol/ml vs. 208 +/- 16 fmol/ml). Patients with malignant tumors of the breast and hypercalcemia contributed predominantly to the higher values of PAF (1500 +/- 167 fmol/l). The high levels of PAF were not found to be correlated to the clinical and histopathological data. The surgical removal of the tumor had little effect on the plasma concentration of PAF. In contrast, hypercalcemic patients showed an increase, although not significant, in plasma PAF levels after the surgical operation.
