Histological, cytological and immunological analyses are complementary for the detection of neuroblastoma cells in bone marrow.

On 80 occasions 4 iliac biopsy trephines and 4 iliac aspirations were performed in 37 children with neuroblastoma at various stages of the disease. In 38 of these procedures, tumour cells were detected. In 24% of cases, both trephines and aspirates were positive, whereas in 63% neuroblastoma cells were detected only on the trephines and in 13% only on the aspirates. In addition, in 37% of the stagings, only one out of the 8 investigations was abnormal. Only in one of 33 pathological cases, was BM involvement diagnosed on trephine imprint. No involvement was ever observed on tibial and sternal aspirates without iliac involvement. Immunological studies with two monoclonal antibodies HSAN 1-2 and UJ13A were performed on 56 occasions. Cytohistological and immunological studies were concordant in 39. In 3 studies, the antigens recognized by the two monoclonal antibodies were not expressed by the initial tumour and in 3 additional studies immunological results were falsely negative; but in 11 cases monoclonal antibodies identified residual malignancy despite normal cytohistology. From this study, biopsies appear more helpful to detect malignant cells than aspirates. Immunological staining clearly leads to a better definition of tumour cells in aspirates.

Human epidermis reconstructed by culture: is it "normal"?

Human keratinocytes were grown on a dermal equivalent (or lattice) at the liquid-air interface in an attempt to reconstitute a functional epidermis in vitro. Although the multilayered epithelium thus obtained is well differentiated, as shown by the presence of keratohyaline granules and horny layer, several differences from its in vivo counterpart were also observed: In the reconstructed epidermis, basal keratinocytes do not have the cuboidal shape found in vivo; they synthesize bullous pemphigoid antigen and laminin, but the distribution of these antigens is not linear as in vivo; they contain the plasma-membrane antigens restricted to the basal layer in vivo (VM1, BC1), but these antigens are not polarized; lack of polarization is also evidenced by the distribution of actin. Differentiation markers appear but with a topography slightly different from that of epidermis in vivo; the 67-kD keratin does not appear in the first suprabasal layer as in vivo but above; involucrin, which appears in the granular layers in vivo appears as soon as the cells leave the basal layer. psi 3 antigen and fibronectin found in vivo only in hyperproliferative epidermis (wound healing, psoriasis) are detected. Hyperproliferation would also explain the unexpected straining of basal cells by KL1 monoclonal antibody. Because of the potential clinical or pharmacologic use of artificial epidermis, the question of whether the epidermis obtained in vitro can be considered as "normal" is discussed.

[Intraepidermal melanocytic proliferation. A practical guide to the classification of ambiguous melanotic skin lesions and of possible precursors of malignant melanomas].

A group of senior Pathologists engaged in review work on international randomized trials for the W.H.O and/or the E.O.R.T.C., propose a new simplified classification of melanocytic lesions with an intra-epidermal component, applicable in routine. This classification attempts to introduce standards to permit morphological identification of a large group of intra-epidermal melanocytic proliferations with three classes of atypia (slight, mild, severe) and group of malignant melanomas especially those without dermal invasion. The new definitions and objective criteria (at cytological and architectural level) of diagnosis are given with examples of equivalence between some established entities and this new universal terminology.

[Carriage of HBs antigen and infection by delta agent in Cameroon].

Serum samples taken from 370 healthy individuals (aged 15-44) constituting a randomized sample representative of the adult population of the town of Nkongsamba, Cameroon, were tested for Hepatitis B virus surface antigen (HBs); positive samples were then tested for Hepatitis B virus [e] antigen (HBe) and anti-Delta antibody. The mean carrier rate of HBs was 8.4% +/- 2.8%; within HBs carriers, 29% exhibited HBe and 6.5% anti-Delta antibody.

Prognostic value of the topographic grid method in women with T2 N- breast cancer. Statistical results from a series of 203 patients.

In 1977, the authors described the grid method, a topographic technique for mapping the microscopic spread of breast cancer. The current report describes the relapse-free survival of a series of 203 patients with T2 N- mammary cancer (T2 N0 N1 ab N-) who were treated by mastectomy and regional lymph node dissection and have been followed since 1974. A good prognosis did not prevent relapse in 15% to 20% of patients within the first 5 years. To determine whether the grid method can identify patients at high risk within this group, disease-free survival and survival were compared in two subsets, a "limited" group (L) and a "diffuse" group (D). The difference between the two groups was statistically significant for both parameters (88%) versus 77% and 93% versus 85% after 5 years) (P less than 0.01 and P less than 0.02). Other prognostic measures were uniformative. Thus, the grid method has a good prognostic score when compared with the factors usually measured--menopausal status, estrogen receptors, histologic type--and could be used to select appropriate adjuvant therapy in breast cancer.

Stimulation by glucagon and PTH of Ca and Mg reabsorption in the superficial distal tubule of the rat kidney.

The effects of glucagon and PTH on electrolyte reabsorption in the distal tubule were investigated in rats deprived of vasopressin, calcitonin, PTH, and glucagon. Micropunctures of distal tubule, at a late and an early site of a same nephron, have been performed in 23 rats, nine control, seven infused with glucagon (5 ng X min-1 X 100 g-1 b.w.) and seven with PTH (5 mU X min-1 X 100 g-1 b.w.). The Ca and Mg reabsorptive capacity of the distal segment was increased by glucagon and by PTH. Moreover, fractional Na and Cl reabsorption was significantly higher than in control during PTH administration. A K secretion appeared during the administration of both hormones. No phosphate net transport was observed in any group. Finally, the data presented here, together with those previously reported, indicate that the increase of Ca and Mg renal reabsorption observed with glucagon and PTH results from an effect located in both Henle's loop, where the bulk of Ca and Mg is reabsorbed, and the distal tubule.

Epidermal morphogenesis and induction of the 67 kD keratin polypeptide by culture of human keratinocytes at the liquid-air interface.

Culture in the presence of delipidized serum (i.e., in the absence of vitamin A) has been shown to allow terminal differentiation of human keratinocytes, both in terms of morphological appearance and in terms of 67 kD keratin polypeptide synthesis (Fuchs, E & Green, H, Cell 25 (1981) 617) [2]. Culture at the liquid-air interface is known to induce morphological differentiation in a variety of culture systems designed for keratinocytes (Pruniéras, M et al., J invest dermatol 81 (1983) 28s) [3]. We report here that human keratinocytes grown on a dermal equivalent (or lattice) in the presence of total serum are able to express the 67 kD keratin polypeptide, provided that the culture is raised at the liquid-air interface. Loss of contact with air results in switching off this synthesis.

PTH-like glucagon stimulation of Ca and Mg reabsorption in Henle's loop of the rat.

The effects of glucagon and PTH on renal tubular electrolyte handling were studied in anesthetized, thyroparathyroidectomized Brattleboro rats infused with somatostatin. Fractional excretion of Ca and Mg was significantly lower during infusion of both hormones. Micropunctures of same nephrons localized the bulk of the increase in reabsorption in Henle's loop, where both hormones significantly enhanced the reabsorptive capacities for Ca, Mg, and K. Beyond the early distal tubule, Ca and Mg reabsorption was significantly greater during glucagon infusion and Ca reabsorption was significantly greater during PTH infusion. Cyclic adenosine monophosphate at a plasma concentration of 10(-6) M did not reproduce the effects of either glucagon or PTH. These results are similar to the findings reported for the effects of ADH and calcitonin on Ca, Mg, and K tubular handling, but different as far as Na and Cl are concerned, since their loop reabsorption was not significantly altered by glucagon and PTH. The data obtained here for glucagon and PTH, together with those for ADH and calcitonin, support the hypothesis that these four hormones exert similar effects in the thick ascending limb.

A new approach to the management of epidermoid carcinoma of the anal canal.

Until recently most squamous cell carcinomas of the anal canal were treated by radical surgery. Radiation therapy was only considered for palliation in case of inoperable tumors. Important progress has been made in the knowledge of the natural history of the disease and in the field of radiotherapy. Anal canal squamous cell carcinoma should not be treated any longer by the same procedure as adenocarcinoma of the lower rectum, because both these diseases differ markedly. Multimodality therapy with radiotherapy as first approach has been considered. This series of 121 cases treated since 1971 and followed more than three years suggests that three protocols based on irradiation followed or not by surgery should be used according to the extent of the disease. Of the 72 patients with resectable tumor, the five-year survival rate was 65%. Three-quarters of the patients cured had normal anal function. The rate of death from cancer was 18%. The method requires an accurate assessment of the extent of the tumor and of its pelvic lymphatic spread. Great care must be taken in planning treatment in a close cooperation between radiotherapist and surgeon.

[Primary malignant melanomas of the urethra. Apropos of 4 cases].

The authors report four cases of primary malignant melanoma of the urethra. The rarity of these lesions can lead to their mis-diagnosis. Only histological examination of a biopsy specimen stained with Fontana stain can confirm the diagnosis. These tumours have a very poor prognosis because of their tendency to locally invade the corpus cavernosus in men and the vulva and vagina in women. These lesions carry a high risk of lymph node and metastatic dissemination. For very limited tumours, the authors recommend a relatively extensive urethrectomy (2/3 of the urethra in women and amputation of the penis in men with a perineal urethrostomy) completed by interstitial irradiation with irridium 192 (6,000 rads) and inguinal lymphadenectomy. In larger tumours, with lymph node invasion, the very brief survival time makes extensive excision inappropriate. Limited palliative surgery is to be preferred. Radiotherapy and chemotherapy have no proven effectiveness.

Effect of glucagon on magnesium renal reabsorption in the rat.

The recent localization, in the rat, of a glucagon-sensitive adenylate cyclase in these segments where the bulk of calcium and magnesium is reabsorbed suggests an effect of this hormone on calcium and magnesium tubular transport. Renal tubular handling of calcium and magnesium as well as of sodium and phosphate was therefore studied by clearance methods in anesthetized rats, either intact or thyroparathyroidectomized (TPTX), infused with glucagon at a rate of 25 ng.min-1/100 g bw just after a priming dose of 2.5 micrograms. The hormone administration resulted in a significant decrease of absolute and fractional magnesium excretion (from 16.3 +/- 0.7% to 9.7 +/- 1.7% for intact rats and from 20.9 +/- 1.8% to 6.9 +/- 1.0% for TPTX rats), associated with the well-known increase in sodium and phosphate fractional excretion. Moreover, a small and transient decrease of calcium fractional excretion was observed concomitantly with a decrease of plasma calcium concentration. The significant increase in magnesium absolute reabsorption, observed whatever the filtered load and independently of PTH and calcitonin, may be an evidence for a direct tubular effect of glucagon.

[Auto-immunity and ovarian failure (author's transl)].

The authors report the case of a 17 year-old girl presenting with primary amenorrhea. Investigations showed that it was due to auto-immune hypogonadism. Such an etiology is rarely found in ovarian insufficiencies. In the reported case, there was no other associated auto-immune disorder. Two types of anti-ovarian antibodies are known: antibodies to steroid-producing cells and antibodies to germ-cells. The first ones are found in patients presenting with auto-immune Addison's disease, and are directed against antigens common to ovary and adrenal glands and their presence does not imply ovarian dysfunction. The second ones are more specific and found chiefly in patients with sterility and amenorrhea; they are divided into 2 subgroups: antibodies against the cytoplasm of the ovacyte antibodies and antibodies against zona pellucida. In patients with primary ovarian insufficiency, the presence of such antibodies should always be investigated.

[Operable carcinoma of the breast. Prognostic value of estradiol receptors. Preliminary report (author's transl)].

151 women were treated for potentially curable breast cancer. Their tumors were routinely analysed for estrogen receptors by the dextran-coated charcoal technic. Data were analyzed in relation to several parameters: age, age of menstruation, menopausal status, tumor size, TNN staging, axillary node status, nature of treatment. We have examined the pronostic implication of an ER negative content (ER-) in the primary tumor. We have found that the absence of detectable estrogen receptor in primary breast tumor appears to be correlated with a short disease free interval. In positive axillary node patients the value of the disease free interval is significantly shorter in ER- than in ER+ group: 11.5 months versus 23.5 (p less than 0.01).

The distal nephron of rat kidney: a target site for glucagon.

Glucagon-sensitive adenylate cyclase [ATP pyrophosphate-lyase (cyclizing), EC 4.6.1.1] activity was measured in nine different portions of the rat nephron. Each sample contained a single piece of tubule isolated by microdissection from collagenase-treated kidney tissue. As compared to basal activity, 1 microM porcine glucagon stimulated adenylate cyclase 60-fold in the medullary portion and 40-fold in the cortical portion of the thick ascending limb, 23-fold in the early distal convoluted tubule, 11-rold in the cortical collecting tubule, and 8-fold in the medullary collecting tubule. No stimulation was observed in proximaly tubules and thin segments of the loop of Henle. Half-maximal stimulations were obtained with about 10 nM glucagon in the responsive nephron portions.

[Dystrophy and necrosis following radiotherapy for maxillary cancer (author's transl)].

About 10% of patients developed severe sequelae following radiotherapy, which had been associated with surgical treatment, even though they were, or appeared to be cured. Complications affected the skin and soft tissues (causing skin retraction, loss of substance, and sometimes trismus) and bone (necrosis required surgical treatment, and when there was associated cutaneous dystrophy, resulted in exposure of the bone and severe functional and esthetic problems). A critical study of the radiotherapeutic techniques used suggests that the dose given should be reduced, the fields modified, and Cobalt used in preference to electrons in most cases.
