The urban setting of juvenile pedestrian injuries: a study of behavioural ecology and social disadvantage.

Population-adjusted rates of fatal and serious pedestrian injuries (in 435 Calgary residents age 0 to 17) and some other indicators of behavioural problems (including juvenile delinquency) and social indicators (including poverty and proportions with low birth weight) were compared across 120 neighbourhoods. Two areas with high rates of pedestrian injury, delinquency, and various indicators of disadvantage were identified, one close to the city core and the other in modern, low-income housing on the city periphery. The patterning of injury rates by income levels across neighbourhoods supports earlier Canadian work. Once again, a multivariate, causal model of stress and vulnerability in coping with physically hazardous traffic environments is implied. Social intervention and community development models based on social work research are advocated.

Psychology of computer use: XX. Sexual abuse recalled: evaluation of a computerized questionnaire in a population of young adult males.

Development of a computerized questionnaire for investigating prevalence of child sexual abuse as "unwanted sexual contact" prior to Age 17 and associated long-term mental health sequelae is described. The computerized technique has technical advantages, and in a study of 200 university men elicited significantly more recall of prior abuse (in 14% of subjects) than a paper questionnaire.

Poverty and suicide among native Canadians: a replication.

In replication of USA research, poverty and suicide were shown to have strong positive correlations in an ecological study of males, aged 15 to 34 years, in 26 Native reservations in Alberta, Canada. The importance of cross-cultural research in replicating and refining psychological research is stressed.

Validity of a short measure of child sexual abuse for use in adult mental health surveys.

An 18-yr. follow-up of 49 children for whom presence or apparent absence of sexual abuse was independently verified by social service reports in childhood, indicated partial validity for a recently developed measure of sexual abuse. Of 19 subjects known to have been sexually abused in childhood, 74% recalled details of such abuse when young adults.

Is the prevalence of child sexual abuse decreasing? Evidence from a random sample of 750 young adult women.

An adult recall study in Calgary, Alberta in 1988-89 of child sexual abuse used stratified, random sampling to identify 750 women ages 18 to 27 yr., divided into 10 age cohorts of approximately 75 each. Those aged 18 and 19 yr. recalled significantly less contact abuse up to age 16 than those aged 20 to 27 yr. Experiencing print or visual media on the topic of sexual abuse was associated with a decreased prevalence. It is argued that in a climate of publicity and greater understanding of help sources and harmful effects, the actual prevalence of child sexual abuse may be decreasing.

The taxonomy and course of childhood autism.

The hypothesis that prognosis in autism would vary across subgroups was explored through cluster analysis of 52 cases. Four clusters related mainly to presence or absence of mutism, speech pathology, and lack of social responsiveness. Children who were mute and socially unresponsive on admission had particularly poor outcomes at follow-up 4.5 yr. later. For a third of the 35 subject followed, the Vineland Social Quotients (mean 46.0 for all subjects) had declined.

Juvenile delinquency and child pedestrian accidents: an ecological analysis.

Data on child pedestrian and cycle accidents and on juvenile delinquency were expressed as population-adjusted rates and covaried with social and demographic data for 120 districts in Calgary, Western Canada. Earlier results were confirmed, in that high rate areas on the indicators employed tended to coincide, identifying particular urban areas with high rates of many types of social problems.

Tamoxifen and fluoxymesterone versus tamoxifen and danazol in metastatic breast cancer--a randomized study.

A prospective randomized trial of tamoxifen and fluoxymesterone versus tamoxifen and danazol in metastatic breast cancer was conducted from December 1980 to September 1985. Patients were eligible regardless of site of disease, estrogen receptor status, or age. Sixty-two of sixty-three randomized patients were evaluable for response. Overall response for tamoxifen and fluoxymesterone was 11% with 61% stabilization of disease, versus 12% response rate for tamoxifen and danazol with 59% stabilization. Toxicities with tamoxifen and fluoxymesterone were greater with an increase in masculinization. We conclude that the response rates to the combinations of tamoxifen and fluoxymesterone or tamoxifen and danazol reported are equivalent in this study but that the increased toxicity with tamoxifen and fluoxymesterone would make tamoxifen and danazol the treatment of choice if a combination were to be used.

Preliminary results of a phase II trial for the treatment of metastatic breast cancer with 5-fluorouracil and leucovorin.

The active metabolite of FUra, 5-fluorodeoxyuridine monophosphate (5-FdUMP), requires the presence of reduced folates to form a covalent ternary complex with the target enzyme thymidylate synthase (TS). In vitro and in vivo studies have demonstrated a potentiation of the cytotoxic effects of FUra when combined with the reduced folate, leucovorin. We have applied this concept to the treatment of metastatic breast cancer in a phase II trial, as recent clinical studies on patients with colorectal carcinoma have suggested an enhanced efficacy for the combination of FUra plus leucovorin. Patients entered on the present study are undergoing treatment with a 5-day daily regimen of leucovorin (500 mg/m2, iv) followed by FUra (375 mg/m2, iv). Toxicity and response data are currently being collected on patients who have failed "standard" combination regimens that included FUra. In patients with accessible tumor, serial biopsies are being obtained during treatment with the combination of FUra and leucovorin and during therapy with FUra alone to assess the degree of 5-FdUMP binding to the target enzyme, TS, in the presence and absence of exogenously administered leucovorin. Preliminary results from the biochemical studies suggest an enhanced saturation of TS by the fluorinated pyrimidine when administered with leucovorin.

The prevalence of suicidal behaviors, attitudes and associated social experiences in an urban population.

The development of an epidemiological study of mental health, social background factors, suicidal behaviors (suicidal ideas, deliberate self-harm, and attempted suicide), values concerning suicide, and experience of suicide in others is described. 679 adult respondents in a stratified random sample of residents of a large Canadian city were interviewed. Results indicate a more accepting attitude to suicide than reported in previous studies; considerable experience of suicide in others; and a much higher rate of suicidal ideas and action than has been reported in previous work. Thirteen percent of the sample had made plans for suicide in their lifetime, 6% had deliberately harmed themselves, and a further 4% had attempted suicide in their lifetime. In the past year, 4% had made plans for suicide, and 2% had deliberately harmed themselves or had attempted suicide.

Intensive chemoradiotherapy with autologous marrow transplantation for small cell carcinoma of the lung.

Ten patients with small cell carcinoma of the lung (seven with extensive and three with limited disease) underwent several courses of conventional therapy. The patients were then referred for autologous marrow transplantation, three during a complete response, six during a partial response, and one following no response. The pretransplantation regimen consisted of 120 mg/kg of cyclophosphamide followed by 800-1000 rad of total-body irradiation. In addition, six of the seven patients with extensive disease received high-dose nitrosourea. Following the infusion of cryopreserved autologous marrow, all patients achieved engraftment. Of the three patients without detectable tumor at the time of transplant, two died with tumor present and one survives without recurrence 27 months after transplantation. Of the other seven patients, two had a complete tumor response; both died of interstitial pneumonitis, one without detectable tumor and one with microscopic tumor at autopsy. One of the other five patients had a partial response, but all died of tumor progression. The median survival from initial therapy in patients with extensive disease was 9 months and with limited disease was 18.5 months.

Preliminary results of combined surgery and adjuvant Bacillus Calmette-Guérin plus levamisole treatment of resectable lung cancer.

The administration of intrapleural bacillus Calmette-Guérin (BCG) alone and intrapleural BCG plus levamisole was compared to placebo in patients with resectable, non-small cell cancer of the lung. This report is based on an interim analysis of 100 eligible patients with a median followup of 245 days. No significant treatment effects are apparent at the present time, although current trends are consistent with an approximate 30% reduction in recurrence rate in patients receiving intrapleural BCG. There is little evidence to suggest that the addition of levamisole will contribute to this effect. A relationship between purified protein derivative conversion and tumor recurrence is apparent. Failure to manifest purified protein derivative conversion following administration of intrapleural BCG is associated with a significantly greater risk of tumor recurrence.
