[Measurement of plasma 1,25 dihydrovitamin D. Normal and pathological values in adults (author's transl)].

Plasma levels of 1,25 dihydrovitamin D (1,25 (OH)2D) were measured in normal subjects and in patients with various diseases, using a radiocompetition method. Mean values of 89 +/- 58 pmol/l (1 s.d.) were found in normal adults, irrespective of sex. Plasma 1,25 (OH)2D values were high in 12/31 patients with lithiasis and hypercalciuria, in 1/7 patients with lithiasis and normal calciuria and in 2/4 patients with idiopathic parathyroid adenoma; they were normal in 2 patients with essential hypoparathyroidism. 1,25(OH)2D was undetectable in 5 patients with chronic renal failure, low in 7 and normal in 4. Following successful kidney transplantation (serum creatinine less than 120 mumol/l) one-half of the patients had normal values and the other half high values.

[Evidence for a glucose-dependent renal phosphorus leak in normo- and hypercalciuric stone-formers (author's transl)].

Changes in phosphatemia and in maximum renal threshold phosphate concentration (TmP/GFR) have been measured before and 45 and 75 min after oral ingestion of 75 g of glucose in 10 hypercalciuric lithiasic patients (HCI), 8 normocalciuric lithiasic patients (NCI) and 7 control subjects. After glucose ingestion, whereas plasma phosphorus decreased in all 3 groups, TmP/GFR decreased significantly in HCI and NCI but not in controls. A slight increase in calcium excretion was noted in the subjects with HCI and NCI after glucose load. Plasma concentration of 1,25(OH)2D was measured in 8 HCI and 7 controls before and after glucose: there was a significant increase in HCI 45 min after glucose and no modification in control subjects. These results show that a glucose load causes a decrease in TmP/GFR in lithiasic subjects. The results demonstrate the existence in lithiasic patients of perturbations of renal handling of phosphates which could, at least in part, be related to glucose metabolism. This trouble could contribute to the elevation of the serum 1,25(OH)2D observed in these patients.

Ineffectiveness of topical adenine arabinoside 5'-monophosphate in the treatment of recurrent herpes simplex labialis.

The ability of topical 10 per cent adenine arabinoside 5'-monophosphate to decrease the severity and frequency of recurrent herpes simplex labialis was evaluated in a double-blind, randomized study of 233 patients at three collaborating institutions. Nine clinical and four virologic measurements were used to evaluate drug efficacy during a single episode. No statistically significant improvement in any measurement was seen in the drug-treated patients. Analyses according to institution and age, stage and size of lesion before therapy also indicated no benefit attributable to the drug. There was no effect of the drug on the rate of recurrence of herpes simplex labialis. We conclude that, despite activity against herpes simplex virus infection in tissue culture and in some laboratory animal models, topical use of the drug is ineffective against recurrent herpes simplex labialis. This may be due to failure of the drug to penetrate the skin.

The natural history of recurrent facial-oral infection with herpes simplex virus.

Fifty-seven episodes of facial-oral infections with herpes simplex virus (HSV) (cold sores) were studied in 41 ambulatory patients. Patients were examined within 24 hr of the onset of symptoms and for five consecutive days. Clinical parameters were assessed, lesion size was measured, and daily cultures for virus were performed. HSV was isolated in 61% of the episodes and was HSV type 1 in all cases. Serum neutralizing antibody to HSV was measured initially and 21 days after the onset of symptoms. All patients had antibody initially, but a fourfold or greater rise in titer was seen in only four patients. Lesion size and stage of healing were compared in patients with virus-positive episodes and those with virus-negative episodes. These two groups were found to be clinically distinct. Virus-positive lesions were larger, and the rate of healing was slower. This finding provides the first clinical correlation associated with the presence of HSV in cold sores.

Noninvasive experimental determination of the individual kidney filtration fraction by means of a dual-tracer technique.

A noninvasive method for measurement of the individual kidney filtration fraction (FF) is presented, based on an analysis of the early rise of the kidneys' time-activity curves obtained after simultaneous injection of tubular [131I] ortho-iodohippurate and glomerular (Tc-99m DTPA) tracers. The analysis is based on the assumption that an insignificant amount of tracer leaves the kidney during the first few moments following injection. Therefore the kidney activity during this period is directly proportional to the integral of the blood (heart) activity. The dual-tracer technique allows the direct calculation of the ratio of glomerular to tubular clearances, i.e., the FF. In vivo studies were performed on 12 dogs, including normals as well as others with acute ureteral ligation or Benemid-induced tubular blockade. The calculated FF correlated well with the FF obtained from single-shot clearances performed simultaneously. We conclude that the FF can be calculated directly for each kidney, noninvasively, from the early part of the tubular and glomerular time-activity curves by noninvasive external detection.

[Demonstration of a microtubule protein (tubulin) in swine parathyroid cytosol].

A cytosolic colchicine-binding protein has been studied in Porcine parathyroid glands. The molecular weight of this protein is 110,000 dalton with a 6 S sedimentation constant. Its colchicine affinity is 1.3 X 10(6) l/mole at 37 degrees C. Polymerization-depolymerization kinetics and vinblastin sulfate-induced precipitation are the same as that of a tubulin.

Role of intracellular calcium and sodium in light adaptation in the retina of the honey bee drone (Apis mellifera, L).

In the honey bee drone, the decrease in sensitivity to light of a retinula cell exposed to background illumination was found to be accurately reflected by the difference in amplitude between the initial transient depolarization and the lowest steady depolarization evoked by the background light. It is shown that both the decrease in sensitivity to light and the accompanying drop in potential from the transient to the plateau can be prevented by injecting EGTA intracellularly. A decrease in duration and amplitude of responses to short test flashes such as observed immediately after illumination was found to occur too when Ca or Na, but not K, Li, or Mg injected into dark-adapted retinula cells. Injection of EGTA into a retinula cell maintained a steady state of light adaptation, was found to cause an increase in amplitude and duration of the response to a short test flash, thus producing the effects of dark adaptation. It is suggested that, in the retina of the honey bee drone, an increase in intracellular calcium concentration plays a central role in light adaptation and that an increase in intracellular sodium concentration, resulting from the influx of sodium ions during the responses to light, could lead to this increase in intracellular free calcium.

[Value of screening for gonorrhea at a family planning consultation (author's transl)].

We report the result of an investigation carried out in the Gynecology Policlinic of Geneva. Systematic screening of 217 patients led to the detection of 2 cases of gonorrhea. Although of interest, it is not justifiable economically to screen for gonorrhea a group of patients in whom the chances of infection are small. However, the study of a larger population might lead to a revision of this conclusion.
