Characterization of myeloid leukemias with monoclonal antibodies 3C5 and MY9.

The expression of two membrane antigens identified by the monoclonal antibodies (McAb) My9 and 3C5 has been investigated in cells from 80 acute leukemias. My9 was positive in the blasts of 33 out of the 38 (87 per cent) cases of acute myeloid leukemia (AML) tested, regardless of FAB subtype, and in 13 of 18 (72 per cent) cases of chronic granulocytic leukemia (CGL) in myeloid blast crisis. The reactivity of 3C5 was confined to myeloblastic (M1) AML, 85 per cent of cases, and to lymphoblastic leukemia (ALL) of B-lineage, 70 per cent of cases, including CGL in lymphoid transformation. My9 was negative in ALL except for an unusual case. The phenotype My9+, 3C5+ was seen exclusively in M1 (69 per cent) and M2 (14 per cent) AML. Ultrastructural analysis with the immunogold method in combination with the myeloperoxidase (MPO) reaction showed that expression of My9 increased in parallel with MPO activity whereas 3C5 was expressed mainly in myeloblasts with little MPO content. We conclude that the use of these two McAb will contribute to the diagnosis and classification of AML and may throw some light to the pathogenesis of biphenotypic acute leukemias, including TdT + AML.

Mechanical work as a determinant of prey-handling behavior in the tokay gecko (Gekko gecko).

In this study an in vitro analysis of the force and mechanical work required to bite prey items of different size and physical character is combined with an in vivo analysis of prey-handling behavior in the tokay gecko (Gekko gecko). The force required to bite and the work of biting increase with prey size, but the rate of increase is prey specific, with crickets (Acheta domestica) requiring substantially more force and work per bite than larvae (Galleria mellonella and Manduca sexta) for all but the smallest prey. Prey-handling behavior is also prey specific. Geckos exert more bites per feeding event on small crickets than on small insect larvae, but the number of bites increases faster with prey mass for larvae than for crickets. Combination of the in vitro mechanical measurements with the in vivo behavior analysis allows the calculation of total mechanical work per feeding event and indicates that total work increases with prey size but that the difference between prey types is far less than predicted from the differences in structural properties of the prey. This occurs because the number of bites and work per bite relationships tend to cancel the differences in the total work necessary to process each prey type. Thus, when considering the effect of prey size, a 13-fold greater rate of increase in bite force and an 18-fold greater rate of increase of work per bit for crickets over larvae was partially compensated for by a threefold increase in the number of bites used on larvae relative to crickets. These results can be interpreted in two ways. The effect of mechanical work in feeding behavior suggests that the energetics of jaw adductor musculature could play a greater role in governing the feeding behavior of this lizard than has previously been expected. Alternatively, the scaling of work in feeding over a range of prey sizes suggests distinct differences in the geometric features of the prey that determine how they are processed.

Clinical studies of pneumococcal vaccines in infants. II. Efficacy and effect on nasopharyngeal carriage.

Two polyvalent pneumococcal polysaccharide vaccines were evaluated in about 200 infants. Vaccination did not affect nasopharyngeal carriage of pneumococci nor was titer of type-specific antibody, as determined by radioimmunoassay, predictive of subsequent carriage of individual types. Otitis media occurred with equal frequency in vaccinated and in control children. One of four episodes of documented pneumococcal bacteremia occurred after vaccination and was caused by a type contained in the vaccine. No indication of either an efficacious or detrimental effect of pneumococcal polysaccharide vaccines administered to infants was seen in this study.

Emerging role of the pediatric nurse practitioner in acute care.

The pediatric nurse practitioner role in the tertiary setting is one of several emerging roles gaining recognition in the various groups of advanced practice nursing programs today. The advanced practice nurse (APN) who is a pediatric nurse practitioner (PNP) in an acute care setting helps to provide cost-effective, quality patient care for critically and chronically ill children who are in these settings. The foundation of advanced practice nursing in this role incorporates the general role expectations of advanced nursing preparation, including case management, clinical pathway development, consultation and education, research, and collaboration, with the specific knowledge and skills of the pediatric nurse practitioner to function effectively with sick children in the acute care areas.

Ulcer-healing drugs and their actions and side-effects.

Peptic ulcer disease (PUD) is a common cause of admission to hospital. Every year, there are approximately 30,000 admissions to hospital for upper gastrointestinal tract (GT) haemorrhage. Older people are at risk due to tissue changes in the tract associated with age. Younger sufferers may find that peptic ulcer disease is strongly related to lifestyle. Contributory factors include the level of emotional stress experienced, the type of food eaten and the variety/amount of stimulants taken. Peptic ulcers are medically treated with drugs from three different groups: antacids, mucosal protective agents and inhibitors of gastric secretion. Ulcers often recur after treatment, perhaps because of noncompliance with treatment or because a stressful lifestyle is resumed after healing has occurred. To achieve freedom from peptic ulcer disease, patients require from nurses not only information about their drug regime to encourage compliance, but health education to help them eliminate stressors that may contribute to the persistence of PUD.
