[Assessment of the left ventricular function in patients with pacemakers VVI and DDD, using non-invasive methods (systolic time)].

OBJECTIVE: Determination of left ventricular function, in patients with ventricular inhibited (VVI) and atrioventricular universal (DDD) pacemakers, by measurement of systolic time intervals. DESIGN: Twenty six patients were studied (six with DDD pacemakers and twenty with VVI pacemakers). In all of them systolic time intervals measurements were obtained the day before and one week after the implant. PATIENTS: We have studied twenty six patients consecutively, and all of them completed the study. INTERVENTIONS: The measurement of systolic time intervals was obtained, by the simultaneous recording of ECG, phonocardiogram and carotid arterial pulse. The recordings were obtained at a paper speed of 100 mm/sec. MEASUREMENTS AND RESULTS: The values obtained before the implantation were similar in both groups, but one week after implantation they show a closer to normal ventricular function, in patients with DDD pacemakers. CONCLUSIONS: The measurements of systolic time intervals, is a very easy method, to determine left ventricular function non invasively. In patients with DDD pacemakers, the left ventricular function, was better than in patients with VVI pacemakers.

Polyelectrolyte and rheological studies on the polysaccharide welan.

This paper investigates the polyelectrolyte behaviour of welan in aqueous solution. From conductivity and activity measurements it is demonstrated that a double-helical conformation is adopted irrespective of pH and ionic strength. The intrinsic pK of its carboxylic groups is found to be abnormally low (pKO = 2.2) and no conformational transition was observed during neutralization. The viscometric behaviour was studied as a function of degree of neutralization, polymer concentration, shear rate and ionic strength. The linear relation [eta] = f(Cs-1/2) allows the determination of B, the stiffness parameter from Smidsrød, whose value characterizes a very stiff molecule.

True hallucinations in non-psychotic states.

While hallucinations are known to occur as conversion symptoms in non-psychotic psychiatric states such as hysteria and grief, there is a lack of distinction between true and pseudo-hallucinations in such cases. We discuss the rationale which has necessitated this distinction, affirm that true hallucinations can occur in non-psychotic states, discuss the characteristics of such hallucinations, and postulate certain factors which may lead to hallucinations in non-psychotic states being judged as "true."

[Non-invasive evaluation of left ventricular function (systolic time interval) in patients using double-chamber pacemakers programmed on DDD and VVI].

OBJECTIVE: To evaluate changes on left ventricular function as assessed by systolic time intervals (STI), by programming "physiological" pacemakers either on VVI or DDD mode. STUDY DESIGN: Comparison of STI values--electromechanical systole (QS2); left ventricular ejection time (LVET); pre-ejection period (PEP); PEP/LVET--in patients submitted to double chamber pacing programmed in DDD or VVI mode. SETTING: Cardiac Pacing Center in a Department of Cardiology at a University Hospital. PATIENTS: We have studied 12 patients (3 men, 9 women), mean age 56.16 years (range 25-76 years) treated with double chamber pacing. INTERVENTIONS: Initial measurements of STI in DDD or one week in VVI mode, and again after one week in DDD mode. RESULTS: Significant changes were found after the first week in VVI pacing, versus baseline determination in DDD pacing, for LVET, PEP, and PEP/LVET (p less than 0.05). At the end of the second week (DDD pacing), there were also statistically significant differences for QS2 (P less than 0.05), LVET (P less than 0.01) and PEP/LVET (P less than 0.05) values when compared with the ones obtained in VVI pacing, at the end of the preceding week. No significant changes were found between the initial and the final STI determinations (both in DDD pacing). CONCLUSION: We concluded for a better performance of the left ventricle at rest when "physiological" pacing is compared with VVI pacing.

[Diabetes mellitus and coronopathy].

Diabetes Mellitus represents an important public health problem in the most developed industrialized countries. Clinical presentations of diabetes are strongly related to the cardiovascular system, namely, coronary disease and angiopathic renal failure. Diabetes modifies the clinical course of arteriosclerosis by carrying the angiopathic process to a microvascular level, where typical microangiopathic lesions can be observed. The risk of developing atherosclerotic disease is 2-3 fold higher in diabetics than in nondiabetics and arterial hypertension reaches a prevalence of 40 to 80%. Authors analyse Arterial Hypertension in the context of Diabetes putting focus on the underlying pathophysiological mechanisms. Where considering the coronary disease (CD), its high prevalence among the diabetics is also emphasized, which is expressed by an increase of morbidity and mortality when compared to normal subjects. In diabetics not only the incidence of Acute Myocardial Infarction is higher, but also the long term prognosis is more complicated, a reality that the authors try to explain by anatomic and metabolic factors. The association of Diabetes plus hyperlipidemia represents undoubtedly one of the major factors that justify the worsening and progression of CD. Briefly, some interesting points that allow the understanding of this topic are described, pointing the pathogenic differences of types I and II and the clinical implications of their knowledge. Finally, the approach of Diabetes as a cardiovascular risk factor is discussed in a prophylactic perspective.

True hallucinations as a culturally sanctioned experience.

Earlier, we reported that true hallucinations may present as a conversion symptom. We here reiterate this and suggest that cultural sanction may be one factor that can lead to hallucinations being classified as true in non-psychotic psychiatric states.

[Virologic studies in patients with amyotrophic lateral sclerosis].

The effects of intracerebral inoculation of cerebrospinal fluid in mice from 9 patients with amyotrophic lateral sclerosis are reported. There were 704 animals inoculated. The results were considered positive when the animals presented difficulties in walking and equilibrating. Cases 1, 2, 6, 7 and 9 were positives in the first passage. In case 1, the positivity was reached from first to fourth passage and in case 6, to third passage. At sight of this results, the possibility of that disease to be caused by slow-virus is discussed.

Autosomal dominant system degeneration in Portuguese families of the Azores Islands. A new genetic disorder involving cerebellar, pyramidal, extrapyramidal and spinal cord motor functions.

We studied 40 patients in 15 families from the Portuguese Azores Islands. Each family was affected by an autosomal dominant genetic disorder. Symptoms began between ages 20 and 50. Ataxia, ophthalmoplegia, pyramidal signs, dystonia, rigidity, and distal atrophy were the major clinical findings. Evidence suggested that this was a single genetic disease, with variable phenotypic expression. Machado disease, nigrospinodentatal degeneration with nuclear ophthalmoplegia, and autosomal dominant striatonigral degeneration may be variations of this same genetic disease.

Peripheral nerve fibre changes in asymptomatic children of patients with familial amyloid polyneuropathy.

Sural nerve biopsies of 5 children of patients with familial amyloid polyneuropathy were studied by electronmicroscopy. The subjects were 14-17 years old and were in normal health. Neurological examination was negative. In none of the sural nerve specimens were there any amyloid deposits. In Case 4 no fine structural changes were detected. In the remaining 4 cases, there were frequent deposits of glycogen and clusters of multimembranous bodies in the Schwann-cell crescents of large myelinated fibres which presented infoldings of the myelin sheath, irregular myelin lamellation and a great number of Schmidt-Lanterman incisures. In one nerve fasciculus of Case 5 the myelin sheaths of some large myelinated fibres were extremely thickened; the axoplasm exhibited dilated vesicles and disordered microfilaments. These findings were taken as evidence of the occurrence of nerve fibre lesions at the pre-symptomatic stage of the disease, which seem to precede the appearance of amyloid deposition. The lesions affected primarily the Schwann cell and myelin sheath, and spared the unmyelinated fibres.
