Unrecognised Cushing's syndrome and adrenal suppression due to topical clobetasol propionate.

A 53-year-old man with cushingoid appearance--obesity, osteoporosis causing lumbar and thoracic vertebral collapse and a past history of hypertension and depression presented with symptoms and signs of adrenocortical insufficiency. He denied the use of corticosteroid medication. However, it was eventually discovered that he had used clobetasol propionate (Dermovate), a potent topical steroid cream, for five years. The development of adrenal insufficiency symptoms coincided with the withdrawal of the cream.

Atrial natriuretic factor during the perinatal period: equal depletion in both atria.

Data in the literature concerning atrial natriuretic factor (ANF) physiology in the fetus and newborn, although limited, suggest significant activity during the perinatal period. To characterize further ANF physiology during this time, we documented immunoreactive ANF (IR-ANF) concentrations in the right and left atria before and immediately after birth as well as in the hearts of immature and adult rats. There was a significant decrease in the concentration of IR-ANF in both right and left atria on the d before birth that persisted for the first 48 h of life [d 20 fetal right 570 (106, 90), left 580 (86, 75); d 21 fetal right 270 (70, 55), left 214 (117, 75); 1 d right 206 (39, 33), left 229 (41, 35); 2-d right 352 (35, 32), left 237 (26, 23) [geometric mean (upper SE, lower SE) in ng ANF/mg protein]. Repletion of ANF stores began in the right atrium on d 2 of life and in the left atrium between d 2 and 5. The highest levels of IR-ANF were observed at d 15 [d 5 right 1439 (53,51), left 1547 (83,79); d 15 right 2034 (90,86), left 1943 (108, 102); adult right 1380 (119, 109), left 963 (118, 105)]. In contrast to normal adult animals, factors mediating the observed change affect both atria equally during the perinatal period. The concentration of IR-ANF in the right and left atrium of the fetal, newborn, and immature animals was equal. These data document significant alterations in intraatrial IR-ANF concentrations in the perinatal period.(ABSTRACT TRUNCATED AT 250 WORDS)

Determinants of plasma atrial natriuretic factor concentrations in congenital heart disease.

Seventeen children with congenital heart disease undergoing elective cardiac catheterization were studied to define potential hemodynamic and echocardiographic determinants of plasma atrial natriuretic factor (ANF) concentrations. ANF concentrations in the right ventricle and femoral vein were measured by radioimmunoassay. Right ventricular levels were significantly higher than femoral venous levels (p less than 0.01), but the correlation between the 2 was high (r = 0.87, p less than 0.001). Stepwise multiple linear regression analysis demonstrated that right ventricular ANF concentrations were predicted by a combination of indexed right atrial area measured by 2-dimensional echocardiography, right atrial pressure, pulmonary resistance and heart rate (multiple r2 = 0.83). Femoral venous ANF concentrations were predicted by indexed right atrial area and systolic pulmonary artery pressure (multiple r2= 0.79). It was concluded that there are significant independent predictors of right ventricular and femoral venous plasma ANF concentrations in children with congenital heart disease. The right atrial area indexed by body surface area was the only significant predictor of both central and peripheral plasma ANF concentrations in the multivariate analysis.

Atrial antinatriuretic factor in the developing Dahl hypertensive rat.

In order to determine the developmental pattern of atrial concentrations of atrial natriuretic factor (ANF) in the Dahl hypertension-prone rat, atrial ANF concentrations were measured in inbred hypertension-prone (S/JR) and hypertension-resistant (R/JR) Dahl rats at 5, 15, 25, and 51 days of age. In both strains, atrial ANF concentrations peaked at 15 days of age. Atrial ANF concentrations did not differ between the two strains from 5 to 25 days of age. However, by 51 days of age, atrial ANF concentrations in the S/JR rat were significantly greater than those of the R/JR rat. Combining these data with developmental patterns of plasma renin activity in S/JR rats suggests the possibility that the S/JR rat may become intravascularly volume-expanded between 25 and 51 days of age. This volume expansion may contribute to the etiology of hypertension in this model of essential hypertension.

Carbohydrate and protein contents of grain dusts in relation to dust morphology.

Grain dusts contain a variety of materials which are potentially hazardous to the health of workers in the grain industry. Because the characterization of grain dusts is incomplete, we are defining the botanical, chemical, and microbial contents of several grain dusts collected from grain elevators in the Duluth-Superior regions of the U.S. Here, we report certain of the carbohydrate and protein contents of dusts in relation to dust morphology. Examination of the gross morphologies of the dusts revealed that, except for corn, each dust contained either husk or pericarp (seed coat in the case of flax) fragments in addition to respirable particles. When viewed with the light microscope, the fragments appeared as elongated, pointed structures. The possibility that certain of the fragments within corn, settled, and spring wheat were derived from cell walls was suggested by the detection of pentoses following colorimetric assay of neutralized 2 N trifluoroacetic acid hydrolyzates of these dusts. The presence of pentoses together with the occurrence of proteins within water washings of grain dusts suggests that glycoproteins may be present within the dusts. With scanning electron microscopy, each dust was found to consist of a distinct assortment of particles in addition to respirable particles. Small husk fragments and "trichome-like" objects were common to all but corn dust.

Are HMO enrollees being attracted by a liberal maternity benefit?

A study of recent birth patterns in the Southern California Region of the Kaiser-Permanente Medical Care Program (KPMCP) suggests that individuals may have been attracted to the Program by a liberal maternity benefit. The attraction is reflected in both the type of members joining KPMCP and the birth rate of those members. Although the KPMCP birth rate has been below that of the general population, new members in their first year of coverage delivered approximatey one third of all KPMCP births in 1977, twice the number of births that would be anticipated from an equal number of members in the plan for one year or more. The maternity copayment of up to $350 did not deter women already pregnant from joining KPMCP. Termination rates for new members who gave birth, however, were no higher than expected. The Pregnancy Disability Amendment of Title VII of the Civil Rights Act of 1964, which became effective April 29, 1979, may alter the maternity benefits offered by alternative insurance carriers. The law could impact KPMCP's enrollment and utilization of obstetric services.

A comparison of 1960 and 1973--1974 early neonatal mortality in selected states.

Linked birth and infant death records from six states provided the opportunity for comparison of 1960 and 1973--1974 early neonatal mortality rates by weight, maternal age and live birth order. The data were analyzed by fitting linear models to the logarithm of the ratio of 1960 to 1973--1974 early neonatal mortality rates. Although there was substantial variability in these ratios by maternal age and birth order, the ratios were nearly constant for birth weights between 1001 and 4500 gm. The decline in mortality among low birth weight infants (2500 gm or less) accounted for over half of the overall decline in early neonatal mortality. The results obtained here are compared with earlier national studies and possible explanations for the decline are discussed.

Some cardio-pulmonary effects of infra-renal clamping of the abdominal aorta.

No significant changes were found in heart rate, blood pressure, cardiac output, arterial blood gases or physiological deadspace: tidal volume ratio before and after clamping and unclamping the abdominal aorta below the renal vessels. Nevertheless, drugs should be available to reduct the afterload on the heart if any evidence of myocardial ischaemia appears during the period of aortic clamping. The importance of maintaining an adequate circulating blood volume at all times is stressed.
