A total of 781 non-polypoid colorectal neoplasias harvested at 4 main Hospitals in Tokyo, Japan (n = 420) and at 4 different time-intervals at the Karolinska Hospital, Stockholm, Sweden (n = 361) were reviewed. By applying strict histologic definitions, the lesions were classified into adenomas with low grade dysplasia (LGD), with high grade dysplasia (HGD), intramucosal carcinomas (IMC) or submucosal carcinomas (SMC). Of the non-polypoid neoplastic lesions reviewed in Sweden, 82.8% (n = 299) had LGD. In Japanese patients only 42.6% (n = 179) had LGD (p < or = 0.001). On the other hand, as many as 42.4% (n = 178) of the non-polypoid lesions in Japanese patients had HGD, but only 14.1% (n = 51) of those in Swedish patients (p < or = 0.001). Whereas 15.0% (n = 63) of the non-polypoid neoplasias seen in Japan were IMC or SMC, only 3.0% (n = 11) of those seen in Sweden were IMC or SMC (p < or = 0.001). The cause(s) for these differences remains unclear. In Japan, however, a marked increased incidence of colonic cancer has been recorded in later years. Whether the "catching up phenomenon" by the Japanese with western colonic cancer incidence includes increased histologic aggressiveness of non-polypoid neoplastic polyps--as found in this survey--remains to be elucidated.
Although gastric carcinoma is one of the most common cancers worldwide, only a few histologic proximate precursors have been demonstrated. Several authors have found that foci of misplaced gastric mucosa with cystic dilatations (s.c. heterotopic mucosa) are often associated with gastric adenocarcinomas. However, adenocarcinomas originating within heterotopic gastric mucosa have never been reported. In present work, the review of 213 consecutive gastrectomy specimens in Japanese patients showed heterotopic gastric mucosa in 20.1% (n = 43). Up to 18 foci per gastrectomy were present. The heterotopic mucosa was surrounded by invaginations of the muscularis mucosae which showed strong positivity for smooth muscle actin. In 3 of the 213 specimens, an adenocarcinoma was found within a focus of heterotopic gastric mucosa. All 3 adenocarcinomas had cystic dilatations lined by neoplastic columnar epithelium with polymorphic nuclei, irregular nuclear membrane, large irregular nucleoli and pathological mitosis. The tumors had lateral bundles of smooth muscle (smooth muscle actin positive), regarded as invaginations of the muscularis mucosae. The p53 protein was strongly overexpressed in all 3 tumors. The heterotopic gastric mucosa may be one mucosal locus from which gastric adenocarcinomas may originate.

The exfoliating epithelial surface in cone specimens of the human cervix with histologically normal epithelium, squamous metaplasia, dysplasia, carcinoma in situ and in cervical biopsies with invasive squamous carcinoma was studied by scanning electron-microscopy (SEM). A cobblestone-like surface with anisovillosis seems to herald the appearance of malignant cells on the exfoliating epithelial surface. Post-scanning histologic examination served as a diagnostic reference for the SEM findings.
The frequency of intraepithelial lymphocytes (IEL) was investigated in 255 colorectal lesions: in 18 metaplastic polyps, in 159 tubular, serrated and villous adenomas, in 28 incipient adenocarcinomas and in 50 overt adenocarcinomas. > or = 21 IELs were found in 13.6% (12/88) of the adenomas with high grade dysplasia (HGD) but in none of the 18 metaplastic polyps, the 71 adenomas with low grade dysplasia (LGD), or the 50 overt adenocarcinomas. CD3-T, MHCII and TIA1 cell antigens were positive in IELs from tubular adenomas, villous adenomas, incipient carcinomas and overt adenocarcinomas. In the normal colorectal mucosa and in metaplastic polyps, CD3-T was positive whereas MHCII and TIA 1 were not expressed. On the other hand, Fas cell antigen was expressed in the normal mucosa and in metaplastic polyps but it was downregulated in neoplastic lesions. The upregulation of CD3-T, MHC class II molecules, and TIA 1 in IELs of neoplastic colorectal lesions suggests that they are activated and cytotoxic; however, they undergo apoptosis due to the downregulation of the FAS molecules.
The anatomy of the epithelial surface of the uterine cervix was investigated in 31 mice with scanning electron microscope (SEM). The cervox of 20 mice was painted twice a week during five months with 3,4-benzpyrene (BP) in acetone and in 11 mice with acetone alone. Only two of the 15 acetone or BP treated animals with histologic normal epithelium showed similar structures as those described earlier for the normal cervical epithelium of untreated mice. The remaining 13 animals had regular mosaics, intermediate or cobblestone structures covered with irregular microvilli with or without long, finger-like protrusions. Irregular mosaics, intermediate or cobblestone structures were seen in atypical epithelium. In atypias Grade III and in invasive carcinoma irregular cobblestone structures with cellular overlapping were present. Atypical and invasive carcinoma were usually furnished with irregular microvilli, irregular fragmented microrugae and finger-like protrusions. The proportion of mice having anisovillosis with finger-like protrusions increased with increasing degree of epithelial severity. Since pathologic SEM changes were observed in some treated mice having normal histology the possibility was entertained that alterations at the cellular membrane level may preceed the nucleocytoplasmic changes required for the recognition of atypical epithelium in conventional histologic preparations.
