Dissociation of biochemical and hypotensive effects of debrisoquine in hypertensive patients.

The 24 h urinary excretion of adrenaline, noradrenaline, metadrenaline, normetadrenaline and vanillylmandelic acid, plasma renin activity and plasma and urinary debrisoquine were measured before and during chronic treatment with oral debrisoquine in 14 in-patients with essential hypertension. There was a significant fall (mean +/- SD) in the 24 h urinary excretion of vanillylmandelic acid (15.3 +/- 2.8 to 6.7 +/- 1.9 micronmol) noradrenaline (199.0 +/- 105.8 to 125.2 +/- 43.3 nmol) and plasma renin activity (0.71 +/- 0.47 to 0.40 +/- 0.20 pmol Angio I ml-1 h-1) while the urinary normetadrenaline/noradrenaline ratio increased (10.4 +/- 6.1 to 17.1 +/- 5.1). No significant change was seen in the output of adrenaline or of O-methylated metabolites. Debrisoquine produces extensive noncompetitive inhibition of platelet monoamine oxidase in vivo at low therapeutic plasma concentrations. These changes support the view that treatment with debrisoquine produces intraneuronal inhibition of monoamine oxidase and post-ganglionic blockage. There was a significant correlation between the change in standing diastolic blood pressure and the daily dose (rs = -0.52), pre-dose plasma concentration (rs = -0.85) and mean daily urinary recovery (rs = -0.80), of debrisoquine. The full extent of the biochemical changes were seen at low dose and low plasma concentration and were not directly correlated with the fall in standing or supine blood pressure.

Parainfluenza virus type 3: isolation from CSF of a patient with Guillain-Barré syndrome.

We report the isolation of parainfluenza virus type 3 from the CSF of a 19-year-old man with Guillain-Barré syndrome. Although parainfluenza viruses are usually associated with respiratory tract illnesses, some strains show a neurotropism not previously appreciated. Thus, parainfluenza viruses may be responsible for a portion of cases of Guillain-Barré syndrome.

Legionnaires' disease in Vermont, May to October 1977.

Thirty-two confirmed and 24 highly probable cases of Legionnaires' disease occurred in Vermont between May 1 and Oct 15, 1977. Confirmed cases had positive results for direct fluorescent antibody testing of lung tissue or fourfold rise in antibody titer. Highly probable cases had one elevated titer (greater than or equal to 1:256) and a compatible illness. Forty-eight (86%) had underlying chronic disease, and 22 (39%) were immunocompromised. Prominent early symptoms were fever, cough, chills, and malaise. All but one patient had verified pneumonia. Courses ranged from a pneumonia not requiring hospitalization to respiratory failure necessitating support with mechanical ventilation. Seventeen patients died. Although the clinical presentation was variable, rapid development of high fever and leukocytosis together with negative cultures of lower respiratory tract secretions strongly suggested the diagnosis in an epidemic setting.

Continuous-flow size selection of Tetrahymena pyriformis ST: changes in volume, DNA, RNA and protein during synchronous growth.

The cell cycle of Tetrahymena pyriformis ST was studied using synchronous cultures prepared by continuous-flow size selection. These organisms had cell cycle times (mean values 151 and 186 min in the first and second cycles respectively) similar to but not identical with the mean generation times of exponential cultures from which they were selected (mean value 165 min). The median volume of cells averaged over the duration of the cell cycle in synchronous cultures was similar to that found in exponential cultures. Total cell DNA, RNA and protein doubled during the cell cycle: there was one discrete S phase which occupied 40 to 50% of the cell cycle time during the period between the end of the selection procedure and the first synchronous division. Total cell protein and RNA increased continuously.

Acebutolol in hypertension--double-blind trial against placebo.

1. Eleven hypertensive patients were studied in a double-blind comparison of acebutolol, a beta-adrenoceptor blocker, and placebo. 2. Optimum dosage defined in an open assessment varied from 200--600 mg twice daily. 3. Blood pressure and pulse rate fell significantly while patients were lying, standing and after exercise. 4. Blood pressure remained as well controlled on each patient's optimum daily dose when taken once daily, and assessed 24 h post dose. 5. There was no correlation between blood pressure reduction and changes in heart rate. On once daily therapy while blood pressure remained unchanged at 24 h post dose there was a signficant reduction in beta-adrenoceptor blockade as measured by percentage reduction in exercise tachycardia. 6. There was no significant change in urinary catecholamine excretion or echocardiographically estimated cardiac output. 7. A correlation was found between the change in plasma renin activity (log transformed) and blood pressure reduction.

The effect of haematoporphyrin derivative on the uptake of 67Ga, 59Fe and 65Zn in tumours.

The effect of prior administration of haematoporphyrin derivative on the uptake in tumours of 67Ga, 59Fe and 65Zn has been studied in tumour-bearing rats and mice. An approximately two-fold increase in the uptake of 67Ga was observed in the August 15 rat tumour when the nuclide was administered 17 to 24 hr after haematoporphyrin. No increase in the uptake of 67Ga occurred in three mouse tumours. Haematoporphyrin administration did not affect the uptake of 65Zn and 59Fe in any of the tumour systems. It is concluded that the presence of haematoporphyrin does not markedly increase the ability of tumours to accummulate metallic radionuclides.

Cytomegalovirus encephalitis in immunologically normal adults. Successful treatment with vidarabine.

Acute cytomegalovirus (CMV) encephalitis developed in two immunologically normal adults. The diagnosis was confirmed by isolation of CMV from the CSF and urine in one case and from temporal lobe biopsy tissue, CSF, and urine in the second case. Both patients were treated with vidarabine and showed dramatic clinical improvement. Virus excretion, which had been chronic in one case, cleared after therapy. To our knowledge, these are the first persons with CMV encephalitis evidenced by isolation of the virus from the CNS. The response to vidarabine was impressive and warrants further evaluation.

Isolation of reovirus-like agents from patients with Crohn's disease.

220 nm filtrates of intestinal resections from 6 of 10 patients with Crohn's disease produced a cytopathic effect in WI-38 tissue-culture monolayers, whereas controls had no effect. Characterisation of the virus responsible has been completed in 3 of the 6 positive isolates and indicates that it is an R.N.A. virus, 55-60 nm in diameter, heat, ether, and acid stable, and antigenically related to Nebraska calf-diarrhoea virus. It therefore belongs to the Reoviridae family.
