CYP1A1, GSTs and mEH polymorphisms and susceptibility to esophageal carcinoma: study of population from a high- incidence area in north China.

AIM: To characterize cytochrome P4501A1 (CYP1A1), glutathione S-transferases (GSTs) and microsomal epoxide hydrolase (mEH) polymorphisms in Chinese esophageal cancer patients. METHODS: Multiplex polymerase chain reaction (PCR) and PCR based restriction fragment length polymorphisms (PCR-RFLP) were used to detect polymorphism changes of CYP, GSTs and mEH on esophageal cancerous and precancerous lesions as well as in case control group. All the examination samples were obtained from Linzhou (formerly Linxian), Henan Province, the highest incidence area for esophageal cancer. RESULTS: The frequency of CYP1A1 3' polymorphism in case control group (26/38, 68 %) was significantly higher than in esophageal squamous cell carcinoma group (ESCC) (29/62, 47 %) (P<0.05). A significant difference in the incidence of mEH slow allele variant was observed between case control group (15/38, 39 %) and esophageal dysplasia group (22/32, 69 %) or ESCC group (39/62, 63 %) (P<0.05). However, no significant difference was observed among different groups in the polymorphisms of CYP1A1 exon 7, GSTM1, GSTT1, GSTP1 and mEH fast allele. CONCLUSION: The present results suggest that CYP1A1 3' polymorphism may be one of the promising protective factors and its wild gene type may be an indicator for higher susceptibility to esophageal cancer. mEH slow allele variant, associated with the progression of esophageal precancerous lesions, may contribute to the high susceptibility to esophageal carcinoma.

[Analysis of p53 mutational spectra of esophageal squamous cell carcinomas from Linzhou, comparison with esophageal and other cancers from other areas].

OBJECTIVE: To study the etiological clues involved (in esophageal cancer in Linzhou, Henan, a high-incidence area for esophageal cancer) by analyzing p53 mutational spectrum from esophageal precancerous and cancerous lesions. METHODS: Using bolt bioinformatic and Monte Carlo methods to analyze p53 mutation spectra from "The IARC Database of Somatic p53 Mutations in Human Tumors and Cell Lines", "p53 Database at Institute Curic" and to establish a local database based on these data using the FileMark Pro 3.0 software to allow fast and off-line analysis on a PC from the authors' laboratory. RESULTS: We found that esophageal squamous cell carcinomas from Linzhou had a lower prevalence of base substitutions associated with strand bias than those from other areas (32.8% vs 39.8%). However, a higher prevalence of G:C-->A:T transitions at CpG site (29.6% vs16.4%) was found. Esophageal squamous cell carcinomas from Linzhou displayed a distinctive profile of mutation hotspots, including codons 273 (covers 11.3% of all missense mutations), 175 (9.7%), 158 (9.7%), 159 (6.5%) and 282 (6.5%), all of which were at the CpG site. Statistical analysis showed that the p53 mutation profiles between esophageal squamous cell carcinomas from Linzhou and those from other areas were different (P = 0.02). The p53 mutation profiles of esophageal squamous cell carcinomas from Linzhou and from other areas were also different from cancer of the head and neck. CONCLUSION: Data showed that the p53 mutational spectrum of esophageal squamous cell carcinomas from Linzhou baring the characteristics of those caused both by endogenous and exogenous mutagenic agents, suggesting the potential involvement of chronic inflammation, unique dietary habits and carcinogen exposure in the pathogenesis of esophageal squamous cell carcinoma in Linzhou.

Simultaneous pancreas-kidney transplantation with enteric drainage of exocrine secretions.

OBJECTIVES: To simplify the enteric drainage (ED) procedure and to decrease surgical and metabolic complications in simultaneous pancreas-kidney transplantation (SPK) patients. METHODS: Between June 2000 and June 2002, nine patients with insulin-dependent diabetes mellitus (IDDM) and uremia underwent simultaneous pancreas-kidney transplantation. The arterial inflow of the pancreas was based upon the right external iliac artery, while venous drainage was systemic via the external iliac vein. The allografts' exocrine secretions were drained into the proximal jejunum via a two-layer hand sewn, side-to-side donor duodenum to proximal small bowel anastomosis after reperfusion. No Roux-en-Y an astomosis of the jejunum was performed. The kidney graft was placed in the left iliac fossa. Quadruple immunosuppressive therapy with antilymphocyte globulin or anti-CD25 monoclonal antibody (Zenapax), tacrolimus, mycophenolate mofetil and steroids was standard treatment in all patients. RESULTS: This procedure was successfully applied in all 9 patients without complication referable to the technique. All patients had achieved euglycemia and excellent renal function, and stopped being dependent on an external insulin source. Fasting serum glucose fell from 9.5 preoperatively to 4.8 mmol/L and remained stable thereafter. At the time this paper was written, the grafts from eight patients were functioning well. CONCLUSIONS: Our primary experience suggests that SPK with ED without Roux-en-Y anastomosis represents a more physiologic milieu, and a viable alternation to replace the bladder (BD) as the primary route of drainage for exocrine secretions of the pancreas. It is a feasible and safer procedure.

[Effect of Fos expression induced by spinal nitric oxide on the nociception of formalin in rats].

OBJECTIVE: To assess the effect of s.c. injected formalin and clarify the possible mechanism of nociception in SD rats. METHODS: Sixty-four healthy male rats were randomly distributed into the control groups (group NS, group formalin) and test groups (group LaF, group LnF) with correponding numbers. Group NS served as normal control. Group formalin was treated with 5% formalin 100 microliters, s.c. Groups LaF and LnF were treated respectively with L-Arg 4.7 mumol ith and L-NAME 370 nmol ith before receiving 5% formalin 100 microliters, s.c. like group formalin. 1/2 h and 0-1 h after formalin treatment, spinal cord segments were removed for expression of NOS, and behavior responses were observed. RESULTS: The flinching and licking time of group formalin was longer than that of group NS, and NOS expression of group formalin was stronger than that of group NS. Pretreatment with L-Arg potentiated formalin-induced response, but pretreatment with L-NAME reduced the response. CONCLUSIONS: Spinal nitric oxide release might be one of the nociception mechanisms in the formalin test.

[Effect of the number of CD34+CD38+ cells in cord blood on hematopoietic reconstitution in patients with acute leukemia after umbilical cord blood transplantation].

The objective of this research was to explore whether the number of CD34(+)CD38(+) cells infused affects hematopoietic reconstitution after cord blood transplantation. The number of CD34(+)CD38(+) cells in cord blood was analysed with flow cytometry after freezethawing. The body weight and time for neutrophil and platelet recovery were measured in 20 children with acute leukemia. The results showed that the median number of CD34(+)CD38(+) cells infused was 29.47 (9.85 - 325.71) x 10(4)/kg. A median time for neutrophil recovery (> 5 x 10(8)/L) in 20 patients was 18.5 (11 - 32) days, and time for platlet recovery (> 2 x 10(10)/L) in 19 of 20 patients was 45 (12 - 118) days. The number of CD34(+)CD38(+) cells infused correlated with time to neutrophil and platelet recovery (r = -0.577, P < 0.01 and r = 0.503, P < 0.05, respectively). In conclusion, the number of CD34(+)CD38(+) cells infused is correlated with the time for hematologic recovery.

[Production of transgenic mice by in vivo spermatogonia-mediated gene transfer].

The present study examined if injection of DNA into the testis tabular could generate transgenic mice via transfecting spermatogonia. The 0.9 kb Bcl-2 cDNA was fused to the promoter region of mouse whey acid protein gene and the SV40 polyA. A 3.0 kb fragment of WAP-Bcl-2-SV40 was mixed with cationic liposome and one side tabular of 3 mice testis was injected with the fragment, the other side was ligatured. Two out of the 3 males were used to mate with the female 4 days later. Twenty pups were produced and 3 of which were proven to be gene integration positive by PCR detection. Two, 1 male and 1 female, were further confirmed to carry the transgene by Southern blot analysis. The male died by accident during its feeding. The female was demonstrated to express Bcl-2 protein in its mammary glands by Western blot assay. Seven out of 45 F1 mice were proven to be transgenic by PCR. It is concluded that transfecting spermatogonia in vivo can produce transgenic mice.

[Population-based study of human papillomavirus infection in high-risk area for cervical cancer in Shanxi Province, China].

OBJECTIVES: To investigate the prevalence of oncogenic type of human papillomavirus (HPV) infection and identify the high risk population for conducting immuno/chemoprevention of cervical cancer. METHODS: All married women aged 30 to 50 with no history of hysterectomy, pelvic radiation and non-pregnant from certain villages of Xiangyuan and Yangcheng County were invited. This study was conducted through two phases. In phase one, subjects sampled the vaginal secretions using the collectors after signing the informed consent. And physicians sampled exfoliated cells from cervix in the phase two. All the specimens were tested with the Hybrid Capture 2 test. The data was managed and analyzed by VFP and SPSS software. RESULTS: There were 9,683 women participated in this study. Local women welcomed this study and population compliance rate was 75.4%. In tested population, we found 2,666 subjects of HPV DNA positive and HPV prevalence was 27.5%. The rates of different age group were 24.5% (30-34 yrs), 27.4% (35-39 yrs), 28.2% (40-44 yrs), 27.4% (45-50 yrs) respectively and had no significant differences (P = 0.604). The rates were slightly increased with the higher education level and had no differences (P = 0.106). The rate in mountain areas was higher than that in half-mountain areas (P = 0.001). CONCLUSIONS: The prevalence of HPV infection is indeed high in this region. Local women and health professionals welcome the activities of cervical cancer screening and prevention. It is an emergent task to improve their sanitary condition and prevent them from cervical cancer in these women. A women health cohort is established successfully among high HPV exposed women in rural China. The extensive biologic specimen repository has been successfully established to simultaneously study the etiology, early detection, and immuno/chemoprevention of cervical cancer.

[Cyclooxygenase-2 expression in esophageal cancer cells and induction by mitomycin C].

OBJECTIVE: To study the relationship between COX-2 expression in esophageal cancer cell (EC/CUHK-1) and mitomycin C (MMC) treatment. METHODS: 2 mg/L of MMC was added to the well-grown EC/CUHK-1 cells cultured in RPMI-1640 including 10% FCS, and the medium was totally changed after 0.5, 1, 2, 4 and 8 h treatment, respectively. Cells were collected after another 24 h culture. Protein expression of COX-2, Bcl-2, Rb, p53 were examined by Western blot, and RT-PCR method was used to confirm the COX-2 expression in mRNA level. Cell cycle analysis for cells collected at 0, 0.5, 2, 4 and 8 h was performed on an EPICS profile analyzer. RESULTS: The cell cycle analysis showed that the percentage of apoptosis cells were (4.12 +/- 0.83)%, (1.00 +/- 0.11)%, (4.32 +/- 0.99)%, (9.46 +/- 2.11)% and (31.10 +/- 3.57)%, respectively. COX-2 mRNA expression were 2.60, 1.70, and 0.08 times, COX-2 protein expression were 2.0, 3.1 and 2.8 times, Bcl-2 protein were 3.6, 14.0 and 12.0 times, p53 protein were 1.8, 0.5 and 0.2 times, hyperphosphorylated form Rb were 8.2, 8.4 and 6.2 times, underphosphorylated form Rb were 1.8, 0.5 and 0.2 times in 0.5, 2 and 4 h after MMC treatment, respectively, as compared with the control group. CONCLUSIONS: The COX-2 expression showed coincidence up-regulation according to the MMC-induced anti-apoptosis function activation in esophageal cancer cells, and the process was at least partly associated with Rb phosphorylation and p53 accumulation. It is implied that COX-2 may be a protecting factor in MMC induced esophageal cancer cell apoptosis, and the use of COX-2 inhibitor as an enhancer for esophageal cancer chemotherapy may be reasonable.

[Diagnostic value of serum-ascites albumin gradient].

OBJECTIVE: To evaluate the diagnostic value and efficacy of the serum-ascites albumin gradient (SAAG). METHODS: One hundred and thirty six patients with ascites fluid were divided into 5 groups: cirrhosis group (Group A, 42 cases), hepatocellular carcinoma group (Group B, 20 cases), spontaneous peritenitis group (Group C, 10 cases), tuberculous peritenitis group (Group D, 40 cases), and periteneal carcinomatosis group (Group E, 24 cases). Group A, B, and C all had portal hypertension. Ascites fluid from paracentesis was analyzed before the treatment. RESULTS: The ascites total protein (ATP) concentration in Group A, B, and C was less than 25 g/L but was more than 25 g/L in Group D and E. SAAG was more than 11 g/L in Group A, B, and C but less than 11 g/L in Group D and E. There was significant difference between the high SAAG group (> or = 11.0 g/L) and the low SAAG group (P < 0.01). PMN count was less than 250 x 10(6)/L in Group A, B, and E but more than 250 x 10(6)/L in Group C and D. There was no significant difference between Group C and D (P = 0.662). CONCLUSION: SAAG demonstrates that patients with ascites fluid possess the basis of portal hypertension. PMN count represents infective ascites. SAAG combined with PMN can effectively enhance the diagnostic value of ascites fluid tests. SAAG classification can be considered to be a novel standard in ascites fluid analysis.

[Reparative and therapeutic effect of rhBMP-2m on mice injured by chemotherapy].

AIM: To probe the therapeutic effect of recombinant human bone marrow morphogenetic protein-2 maturation peptide(rhBMP-2m) on mouse bone marrow injury caused by cyclophosphamide (CTX). METHODS: 18 mice were divided randomly into 3 groups, namely CTX injection group(CTX group), BMP therapy group(BMP group) and PBS control group(Control group), 6 mice each group. CTX of 200 mg/kg per mouse was intraperitonealy(IP) injected at a time to BMP group and CTX group so as to establish the experimental model of mouse bone marrow injury. In BMP group, the therapy started from the second day after injection of CTX by using IP injection of 0.5 mg BMP per mouse each day. In control group, PBS was injected only. Changes of peripheral blood leukocyte numbers in 3 groups were observed. On the 5th and 8th day after CTX injection, DNA content in mouse bone marrow karyocytes and variation of cell cycle were analysed by flow cytometry(FCM). Colony forming unit granulocyte-macrophage(CFU-GM) was cultivated simultaneously. RESULTS: On the 4th day after injection of CTX, the leukocyte number in mouse peripheral blood of CTX group dropped to the lowest level, and then picking up gradually. In respect to the variation of the leukocyte number, there was no significant difference between BMP group and CTX group (P>0.05). On the 5th day, the ratio of G(0)/G(1) phase cells in BMP group was notably higher than that of CTX group, and the necrotic and apoptotic rates decreased markedly (P<0.01). On the 8th day, the number of karyocytes in mouse bone marrow of BMP group was obviously more than that of CTX group (P<0.01). CONCLUSION: BMP has some therapeutic effect on mouse bone marrow injury caused by CTX.

End-group analysis of blue light-emitting polymers using matrix-assisted laser desorption/ ionization time-of-flight mass spectrometry.

An analytical method based on matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF MS) has been applied to provide information on the structure of a copolymer, e.g., repeat unit and end group. Seven conjugated polymers, which have been demonstrated as the active component in blue light-emitting diodes, were synthesized through Suzuki polycondensation reaction in the presence of Pd(PPh3)4 catalyst. Their molecular weights were obtained using gel permeation chromatography analysis. MALDI-TOF MS was used to investigate the structure information in detail. The proposed end-group structures were confirmed by the identity between the observed and the simulated isotopic distribution of each polymer. The results demonstrate that these synthetic polymers possess various end groups and even contain macrocycles. The catalyst Pd(PPh3)4 was found to introduce phenyl end groups via aryl-aryl exchange between the catalytic palladium intermediate and the triphenylphosphine ligand. All these results are based on the analysis of the mass spectrum data, which suggests that MALDI-TOF MS is an extraordinarily strong tool in synthetic polymer structure analysis.

[Effect of local mild hypothermia on treatment of acute intracerebral hemorrhage, a clinical study].

OBJECTIVE: To observe the effect of local mild hypothermia on treatment of acute intracerebral hemorrhage. METHODS: Forty patients with acute intracerebral hemorrhage were matched according to age and state of illness at a ratio of one to one and then randomly divided into two groups of 20 patients: routine group (dehydrant, antihypertensive agent, etc were given) and hypothermia group (in addition to the general treatment, controllable semiconductor brain-protecting freezer was used to produce local hyperthermia at temperature of 6 degrees C for 48 hours). Immediately after admission and one week and 2 weeks after treatment cranium CT was conducted and the volume of cerebral edema was calculated by Tada's formula. RESULTS: There was no significant difference in the volume of edema and scores according to European Stroke Scale between the two groups at admission (P > 0.05). The volumes of edema one week and 2 weeks after therapy were 17.4 +/- 6.2 and 13.1 +/- 5.8 milliliter respectively in hypothermia group, both significantly lower than those in the routine group (33.8 +/- 16.0 and 22.4 +/- 12.2 milliliter respectively, P < 0.05). The scores according to European Stroke Scale one week and 2 weeks after therapy were 62.1 +/- 10.8 and 70.3 +/- 10.7 in hypothermia group, both significantly higher than those in the routine group (52.8 +/- 10.9 and 60.5 +/- 10.9 respectively, P < 0.05). No side effect caused by local hyperthermia was observed during the therapy. CONCLUSION: A safe and reliable treatment, local mild hypothermia combined with routine therapy is effective in protecting the brain of patients with intracerebral hemorrhage.
