The effect of pH on the structural evolution of accelerated biomimetic apatite.

The classic biomimetic apatite coating process can be accelerated by first immersing substrates into concentrated simulated body fluid, 5x SBF (SBF1), at 37 degrees C, to form an initial coating of precursor apatite spheres, and subsequently transferring to a second 5x SBF (SBF2) solution which is devoid of crystal growth inhibitors to promote phase transformation of SBF1-derived precursor apatite spheres into final crystalline apatite plates. Since SBF1 governs the formation kinetics and composition of the initial precursor spheres, we hypothesized that the pH of the SBF1 solution will also influence the final structure of the SBF2-derived crystalline apatite. To test this hypothesis, polystyrene substrates were immersed into SBF1 with different pH (5.8 or 6.5), and then immersed into the identical SBF2 (pH=6.0). The resultant apatites exhibited similar 2 theta XRD peaks; FTIR spectra in terms of hydroxyl, phosphate and carbonate groups; and Ca/P atomic ratio (1.42 for SBF1(5.8) apatite; 1.48 for SBF1(6.5) apatite). SEM, TEM and electron diffraction show that while SBF1(6.5) (pH 6.5) precursor spheres transform into larger, single crystals plates, SBF1(5.8) (pH 5.8) precursor spheres developed minute, polycrystalline plate-like structures over predominantly spherical precursor substrate.

Adrenal cortical cell transplantation.

PURPOSE: The adrenal cortex is a critical component of the hypothalamic-pituitary-adrenal/gonadal axis that coordinates the stress response and maintains homeostasis. The authors hypothesize that adrenal cortical cells can be transplanted in adrenal insufficiency states to regenerate the adrenal cortex. METHODS: Murine adrenal glands were dissociated into adrenal cortical cells. Cells cultured in a collagen matrix were transplanted under the renal capsule. The implants were retrieved 1, 2, 4, and 8 weeks later. Total RNA was extracted from the retrieved specimens and was analyzed by real-time polymerase chain reaction. RESULTS: All animals survived the surgical procedure. At implant retrieval, a distinct organoid could be visualized under the renal capsule. The expressions of adrenal-specific markers including Sf1, Dax1, Star, Cyp11a, Cyp11b1, and Cyp21 were detectable in the retrieved specimens up to 8 weeks posttransplantation. CONCLUSION: Primary adrenal cortical cells retained their gene expressions after heterotopic transplantation. Ex vivo gene transfer followed by adrenal cortical cell transplantation may lead to curative therapy for patients with adrenal insufficiency.

Enterogenesis by mechanical lengthening: morphology and function of the lengthened small intestine.

BACKGROUND/PURPOSE: The purpose of this study is to evaluate the function of the mechanically lengthened small intestine. METHODS: A jejunal segment was separated from intestinal continuity in rats. A screw was inserted into its proximal end, and the distal end was oversewn. The screw was advanced into the jejunal segment by 5 mm every other day. The jejunal segments were retrieved after 2 weeks. The length, weight, muscular thickness, alkaline phosphatase, and lactase activities of the jejunal segments were determined. Comparisons were made among normal jejunum, isolated jejunal segments without lengthening, and lengthened jejunal segments. RESULTS: Jejunal segments doubled in length after gradual mechanical stretching compared with the normal and isolated controls. The thickness of the muscular layer increased in both the isolated and lengthened groups. The total activity of alkaline phosphatase increased in jejunal segments that were lengthened, whereas the total lactase activity remained the same. CONCLUSIONS: Mechanical force is a viable method for increasing intestinal length while preserving the intestinal function. This phenomenon may provide a new method for the treatment of patients with short bowel syndrome.

Stent implantation for coronary aneurysm with edge stenosis: angiographic and intravascular analysis.

The incidence of coronary artery aneurysms is about 1 to 2%, with clinical course dependent on the size of the aneurysm. A case of moderate-size aneurysm in the proximal left anterior descending coronary artery with stenosis at both edges is presented. This was interrogated with intravascular ultrasound (IVUS), and based on the patient's presentation, a single stent, size-matched 1:1 to the proximal reference, was placed across the aneurysm and both lesions. Post-implantation IVUS demonstrated residual stenosis and minimal change in the neck size of the aneurysm. At 4 months, there was no thrombosis or in-stent restenosis, and the aneurysm was almost completely resolved.
