Cytologic screening for esophageal cancer: results from 12,877 subjects from a high-risk population in China.

Linxian, China, has one of the highest esophageal-cancer mortality rates in the world. In 1983, esophageal balloon-cytology screening was performed to identify subjects eligible for 2 nutrition-intervention trials in Linxian; 12,877 subjects had cytology slides which were satisfactory for diagnosis. Of the 12,649 subjects with squamous-cell diagnoses, 31% were normal by Chinese cytologic criteria; 38% showed hyperplasia; 21% showed dysplasia 1; 6% showed dysplasia 2; 2% showed near-cancer; and 2% showed cancer. Of the 1,471 subjects with columnar-cell diagnoses, 31% were normal; 44% showed hyperplasia; 16% showed dysplasia 1; 4% showed dysplasia 2; 2% showed near-cancer; and 3% showed cancer. Squamous dysplasia and cancer were more common among females than males, while columnar dysplasia and cancer showed male predominance. The prevalence of dysplasia and cancer of both cell types increased with age. The prevalence of squamous dysplasia was significantly higher than in earlier balloon-cytology screenings in Linxian, probably reflecting changes in cytologic classification.

Germ cell development in neonatal mouse testes in vitro requires müllerian inhibiting substance.

To study the effect of müllerian inhibiting substance on testicular germ cell development, especially on gonocytes, whole testes (156) from newborn mice were cultured for 1 to 7 days in vitro. The synthetic medium contained either 10% fetal calf serum, which itself contains endogenous müllerian inhibiting substance, or transferrin, insulin and retinoic acid. Human recombinant müllerian inhibiting substance, rabbit antiserum against müllerian inhibiting substance and/or normal rabbit serum was added to some cultures. The cultured testes were fixed in Stieve's fixative and stained with hematoxylin and eosin, and the numbers and types of germ cells per tubule were counted under a light microscope. Preliminary studies showed that germ cell development in newborn mouse testes was similar in vitro to that observed in vivo, except for delay in vitro. Normal germ cell maturation from gonocytes to primary spermatocytes occurred in testes cultured with 10% fetal calf serum only (i), 10% fetal calf serum plus müllerian inhibiting substance plus anti-müllerian inhibiting substance antibody (ii), 10% fetal calf serum plus normal rabbit serum (iii) and transferrin, insulin and retinoic acid plus müllerian inhibiting substance (iv). Maturation from gonocytes to A-type spermatogonia was arrested in testes cultured with 10% fetal calf serum plus anti-müllerian inhibiting substance antibody (p < 0.01), transferrin, insulin and retinoic acid alone (p < 0.001) and transferrin, insulin and retinoic acid plus müllerian inhibiting substance plus anti-müllerian inhibiting substance antibody (p < 0.001). The results are consistent with the hypothesis that müllerian inhibiting substance may be involved in postnatal gonocyte development and suggest that it may be useful to treat infertility associated with undescended testes.

Effect of "re du qing" on the activation, proliferation and membrane fluidity of lymphocytes.

Effects of Chinese Medicinal Preparation "Re Du Qing" (RDQ) on the activation, proliferation and membrane fluidity of T lymphocytes from human peripheral blood were studied by means of 3H-TdR incorporation and DPH fluorescence polarization. The results showed that "RDQ" can: 1) significantly inhibit the activation of T lymphocytes; 2) restrain the proliferation of activated T lymphoblasts in the presence of exogenous interleukin-2 (IL-2); and 3) increase the membrane fluidity of T lymphocytes and antagonize the decreased fluidity of lymphocyte membrane mediated by Con A or PHA. The functional abnormalities of T lymphocytes in some autoimmune diseases such as arthritis and the usefulness of RDQ in the treatment of these diseases were also discussed.

Serum lipids and their correlates in Chinese urban and rural populations of Beijing and Guangzhou. PRC-USA Cardiovascular and Cardiopulmonary Epidemiology Research Group.

In 1983 and 1984, surveys were conducted in four Chinese population samples, urban and rural for both Beijing and Guangzhou, as part of PRC-USA collaborative research in cardiovascular and cardiopulmonary epidemiology. Serum total cholesterol (TC), HDL-cholesterol (HDL-C), and triglycerides (TG) were measured in 4280 men and 4695 women aged 35-54 years, in laboratories standardized by the US Centers for Disease Control. Age-adjusted mean serum TC was higher in urban than rural samples and generally higher in Beijing than Guangzhou, ranging from 155 mg/dL for Guangzhou rural women to 187 mg/dl for Guangzhou urban women. Group mean values of HDL-C varied from 48 to 59 mg/dl, higher in Beijing than Guangzhou, and higher in women than men. TC/HDL-C ratio ranged from 3.05 to 3.82. Serum TG values were higher for Beijing than Guangzhou; the lowest group mean values of 78 mg/dl and 75 mg/dl were in rural Guangzhou men and women. Mean body mass index (BMI) was uniformly low, ranging from 20 kg/m2 for rural Guangzhou to 24 kg/m2 for urban Beijing. Multiple regression analyses showed that BMI was positively and independently related to serum TC, LDL-C, TG, and TC/HDL-C, and inversely related to HDL-C. Smoking was positively related in both sexes to TG and TC/HDL-C, and inversely related to HDL-C. Smoking was also positively related to TC and LDL-C in men. In men, alcohol was positively related to TC and HDL-C, and was inversely related to TG and TC/HDL-C. Heavy manual work was inversely related to TC, LDL-C, and TC/HDL-C in men, but not related to lipids in women. Thus, for these Chinese population samples, despite their lower serum TC and BMI, the correlates of serum lipids are similar to those in western populations. These variables accounted for only part of the observed urban-rural and north-south differences in serum lipids among these Chinese population samples. The significance of the relatively low serum TC and TG and high HDL-C in relation to low cardiovascular disease in Chinese populations is the object of further investigation in follow-up studies.

Characterization of external ear impulse responses using Golay codes.

This report explains the use of a complementary series, Golay codes, for probing the impulse response of the external ear. The codes are used to measure both the resonance of the human ear canal, using a sealed sound-delivery system, and to measure the transfer function of the pinna, using a free-field source. With two series of 512 binary numbers, the improvement in signal-to-noise ratio over a single impulse approaches the theoretical value of 30.1 dB [10 log(2.512)]. This technique has many of the same properties as maximal-length sequences [M. R. Schroeder, J. Acoust. Soc. Am. 66, 497-500 (1979)], but it has the added advantage that the sequence length is an integer power of two and is, therefore, particularly convenient to use with modern Fourier transform techniques.

[Pharmacological study on Veronicastrum sibiricum (L.) Pennell].

Six compounds were isolated from Veronicastrum sibiricum and identified as D-mannitol, beta-sitosterol, daucosterine, 3-0-acetyloleanolic acid, 3, 4-dimethoxy cinnamic acid and isoferulic acid. Pharmacological experiment has shown that isoferulic acid is anti-inflammatory and analgesic; 3-0-acetyloleanolic acid is anti-inflammatory; mannitol is analgesic, 3, 4-dimethoxy is also anti-inflammatory and analgesic.

Primary structure of the goat beta-globin locus control region.

The goat beta-globin cluster is composed of a triplicated four-gene set. A locus control region (LCR) containing elements homologous to 5'DNase I hypersensitive sites (HS) 1, 2, and 3 of the human beta-globin LCR has been identified at the 5' end of this locus. We determined 10.2 kb of nucleotide sequence from the goat beta-globin locus control region. Self-comparison of this sequence by dot matrix analysis revealed the presence of six complete and three incomplete artiodactyl repeats. A novel repeated element, termed D repeat, was also identified. Southern blotting analysis demonstrated that these elements exist in the goat genome as a low to medium frequency interspersed repeat family. The absence of any other large region of self-homology (direct or inverted) in the goat LCR suggests that 5'HSs 1, 2, and 3 did not arise through duplication, but rather evolved independently. By comparing goat 5'HS 1 to those of human, rabbit, and mouse, we show a greater than 80% conservation in sequence between the four species. This level of evolutionary conservation suggests that 5'HS 1 plays an important role in the regulation of beta-globin loci.

Effects of genetic factors and dietary electrolytes on blood pressure of rural secondary school students in Hanzhong.

In this study, 24-h urine specimens were collected on 3 consecutive days from 181 adolescents (94 boys and 87 girls), aged 12-16 years, in a rural district--Hanzhong, Shaanxi Province, and sodium, potassium, calcium and magnesium levels were determined. Dietary intake of the four electrolytes and blood pressure were measured in children and their parents on the same consecutive days. The results were as follows: urinary output of calcium and magnesium might not be an appropriate index to reflect intake; after adjusting for age, sex, BMI, heart rate, and parental blood pressure, the ratio of dietary calcium to total calories was negatively related to diastolic blood pressure in the whole group, and the ratio of urinary calcium to creatinine was positively related to systolic blood pressure in girls; not only blood pressure but also its relation to electrolytes are affected by parental blood pressure after adjusting for age, sex, BMI and heart rate.

[A study of textile noise influence on maternal function and embryo-growth].

Ninety hundred seventy-eight female textile workers exposed to noise (greater than or equal to 85 dB (A)) were recruited as the subjects of observation group, and 402 nonexposed female workers were chosen as the controls (less than 75 dB (A)). According to noise level, the observation group was subdivided into three groups: group 1, 100-104 dB (A), 416 workers; group 2, 95-99 dB, (A), 310 workers; group 3, 85-94 dB (A), 252 workers. Non-conditional logistic regression model was used to exclude the effect of mixed factors, such as age and working life. The result showed that noise-exposure could have some adverse effect on reproductive function of women and embryo growth. The workers exposed to noise had significantly increased incidence rates for irregular menstrual cycle, dysmenorrhea, lumbago, hypertension in pregnancy, threatened abortion, spontaneous abortion, premature labour, dystocia and low birth weight of newborns. When the noise intensity heightened, the sorts of abnormal function of reproduction increased and the risk of incidence raised too. These demonstrated a dose-response relationship between the noise intensity and incidence rate. The noise-working life had significant effects on the incidence of hypertension in pregnancy and prematurity. Although the age had some influence, it was not statistically significant. In terms of the maternal function and embryo growth, the factors in order of risk are: noise intensity greater than working life greater than age.
