[Current use of statistical methods in five core Chinese medical journals].

OBJECTIVE: To determine the frequency of statistical techniques used in Chinese medical journals, types of statistical errors present, and trends over a 10-year period. METHODS: We reviewed the statistical methods used in all original articles published in 5 leading journals in 1985 (n = 640) and 1995 (n = 954). RESULTS: Compared with 1985, significant improvement was seen in 1995. The proportion of papers using statistical tests increased from 40% to 60% (chi 2 = 60.9, P < 0.01), and more sophisticated statistical methods were used. Of those articles using statistics, the proportion using appropriate methods increased from 22% to 46% (chi 2 = 45.3, P < 0.01). In both years, the most commonly used statistical methods were t tests and contingency tables. The most common errors were presentation of P values without specifying the test used, use of multiple t tests instead of ANOVAs, and use of unpaired t tests when paired tests were required. CONCLUSION: The use of statistical methods in Chinese medical journals is improving, and by 1995, the frequency of using statistical methods in published articles was similar to that in other journals around the world. However, the lack of or inappropriate sue of statistics remains a serious problem.

Supraglottic carcinoma: does preoperative radiotherapy reduce the incidence of cervical metastasis?

OBJECTIVE: To compare surgery (S) alone with combined radiotherapy and surgery (R + S) in the management of patients with supraglottic laryngeal cancer. METHODS: Between 1981 and 1994, patients were stratified according to stage and randomised to either surgery (S) or 4000cGy of radiotherapy and surgery. There were 102 patients in the S group and 99 in the R + S group who completed at least 3-year follow-up. RESULTS: Using Kaplan-Meier survival method showed no significant difference between the two groups. When the patients were grouped according to tumour stage, a significant reduction in the regional recurrence was noted in the R + S group for stage I-III disease (Cox multivariate analysis, P < 0.02). They had an increased relative risk of 1.8 (95% confidence 1.1-2.9) for neck recurrence. There was no significant difference in neck recurrence rates in the two groups for stage IV disease. When Cox proportional hazard model was used, only TNM stage (P < 0.02) and histological nodal status (positive lymph nodes, P < 0.01) were found to be independent risk factors for regional control. CONCLUSION: Preoperative radiotherapy can improve regional cervical control of stage I-III supraglottic cancer as compared with surgery alone.

[The expression of Fas antigen on lymphocytes in the patients with asthma].

OBJECTIVE: To study the expression of Fas antigen on lymphocytes in patients with asthma. METHOD: The peripheral blood lymphocytes (PBL) isolated from patients with asthma were cultured with dexamethasone 10(-5) mol/L, Fas antigen on lymphocytes was detected by flow cytometry at 0, 2nd, 8th, 24th hour respectively. RESULT: The native expression of Fas antigen of PBL isolated from the patients with asthma (5.3% +/- 2.0%, P < 0.05) was lower than that from healthy controls (8.5% +/- 1.8%). Increased expression of Fas antigen in the patients with asthma was observed with induction of dexamethasone (P < 0.01). The percentages of Fas positive cell were 5.3% +/- 2.0%, 33% +/- 5%, 63% +/- 11%, 76% +/- 7% at 0, 2nd, 8th, 24th hour respectively in asthmatic group. While 8.5% +/- 1.8%, 31% +/- 11%, 49% +/- 10%, 20% +/- 4% respectively in normal control group. CONCLUSION: Decreased native expression of Fas on lymphocytes may be related to airway inflammation in the patients with asthma. Increased expression of Fas induced by Dex may be one of the mechanism of lymphocytes apoptosis.

Effect of hypoxia on motion sickness induced by optokinetic stimulation.

The purpose of this experiment was to study the effect of hypoxia on motion sickness (MS) induced by optokinetic stimulation and to observe its character and principle. Twenty healthy male subjects aged 18-25 years received optokinetic stimulus (OS) and hypoxic-optokinetic stimulus (HOS) according to the "Latin Square" arrangement and "Self-control" methods. Motion sickness symptoms (MSS) and optokinetic horizontal nystagmus (OKHN) were recorded and analysed. The results indicated: (1) MS average score for HOS was higher than that for OS (P<0.001); (2) Slow phase velocity(SPV) of OKHN in HOS was lower than that in OS (P<0.001). The results clearly showed that moderate hypoxia could deteriorate the MS induced by OS. It provided a method for the screening of MS sensitivity.

[Effects of processing on toxicity and pharmacological action of flos Genkwa].

Daphne genkwa can not be taken unless the toxicity is removed. The study on the effect of processing on the toxicity and pharmacological action of Flos Genkwa has shown that the toxicity of vinegar-processed Flos Genkwa is lower than that of the rude drug, but the pharmacological action is better. Both genkwanin and its vinegar-processed imitation have antitussive and expectorant actions without marked difference in efficacy. Both yuanhuacine and its vinegar-processed imitation could cause contraction of the extracorporeal womb and the intestinal canal, and the efficacy is almost similar in the concentration of 1:10(-5)-10(-6). The gestational womb was observed more sensitive than the normal one.

[Percutaneous autogenous bone marrow grafting for repairing bone defect: an experimental study in rabbits].

OBJECTIVE: To observe osteogenesis of percutaneous autogenous bone marrow into cicatrix of bone defect. METHODS: 18 rabbits were used to observe the osteogenesis of percutaneous autogenous bone marrow into cicatrix of bone defect. A 1 cm bone defect model was made in each side of the radius. And 2 ml autogenous bone marrow was injected in the right radial bone defect after 6 weeks, and 2 ml peripheral blood in the left side. The animals were sacrificed in 1, 2, 3, 4, 12 weeks after injection. Bone formation was studied with histologic technique, X-ray examination and Ca-P quantitative estimates. RESULTS: After 1 week of injection, a big shadow of softtissue was found around the bone defect in the right side(bone marrow). Histologically a lot of new cartilage like tissues were found. In the left side(peripheral blood), no change was seen. After 4 weeks of injection, many of new bone-like shadows were observed around the defect by X-ray examination in the right side. The defect was smaller(average 1.5 mm) and bone formation was found. But in the left side, no signs for osteogenesis were noted. In 12 weeks defect was smaller(average 1 mm). Histologic sections proved well-developed bone tissue. In the left side, no bone formation found. CONCLUSION: Autogenous bone marrow can induce osteogenesis in cicatric bone defect, whereas no osteogenesis is found in the peripheral blood injection side.

[Squamous cell carcinoma of temporal bone: an analysis of long-term treatment results in 33 patients].

OBJECTIVE: To evaluate the long-term results in patients with squamous cell carcinoma of the temporal bone and treated by of mastoidectomy combined with perioperative radiation therapy. METHODS: Thirty-three patients with squamous cell carcinoma invading the temporal bone were reviewed retrospectively. According to the tumor extent, the patients were staged into three subgroups, 3 with lesions confined to the auditory canal(Group I), 17 with involvement of the middle ear or mastoid(Group II) and 13 with more extensive disease(Group III). Two patients were treated by surgery, 11 by irradiation and the remaining 20 by the combined surgery and perioperative radiotherapy. The surgical procedures included sleeve resection for group I patients, mastoidectomy for all other patients except one who had subtotal temporal bone resection. The radiation doses were in the range of 3,500 to 10,000 cGy, with an average dose of 6,560 cGy. RESULTS: The 5-year survival rate for the whole series was 51.7% by the life table analysis. The estimated 5-year survival rates for 3 subgroups were 100%, 68.8%, and 19.6%, respectively (P = 0.012). Radiation alone had a 28.7% survival rate and combined therapy 59.6% (P = 0.80). For patients treated with planned combined therapy, the actual 5-year survival rates were 72.7% (8/11) for Group II and 12.5% (1/8) for Group III (P = 0.02). Twelve patients died of local recurrence (10 cases), cervical metastases(1 case) and liver metastases (1 case) with 67.0% (8/12) succumbed to their diseases within two years. Complications included osteonecrosis (n = 1), osteitis (n = 3), radiation dermatitis (n = 2), facial palsy (n = 2), and delayed healing (n = 2). CONCLUSION: The 5-year survival rates in patients with mastoidectomy with removal of gross tumor, combined with planned perioperative irradiation therapy, are similar or higher than those from literature. This protocol can be used in patients with squamous cell carcinoma confined to the middle ear or the mastoid.

[Inhalation sedation with nitrous oxide in dental extraction].

OBJECTIVE: To study the advantages of nitrous oxide sedation in teeth extraction. METHODS: 112 patients were randomly allocated to four groups: 1) 60 mmol/L nitrous oxide, 2) 60 mmol/L nitrous oxide with estazolam, 3) 40 mmol/L nitrous oxide, 4) 40 mmol/L nitrous oxide with estazolam. The nitrous oxide was used for conscious sedation as an adjunct to local anaesthesia. 53 patients of control group underwent dental extraction with local anaesthesia only. RESULTS: The postoperative MDAS scores of experimental group decreased and was significantly superior to control group (P < 0.01). The patients of experimental group showed a certain levels of sedation and partial amnesia. 97 of the 112 patients (86.62%) preferred inhalation sedation with nitrous oxide. There was no difference between the anxiolytic effects, sedative effects, amnestic effects of the four groups (P > 0.05). Estazolam as preoperative medieation did not show obvious coeffect with nitrous oxide. CONCLUSION: Inhalation sedation with nitrous oxide can provide effects of goodquality antianxiery, sadation, amnesia in dental extraction.

[Diagnostic value of serum creatine kinase MB in patients with cardiac damage after electric injuries].

OBJECTIVE: To investigate the incidence of cardiac damage and the diagnostic value of serum myocardial enzyme following electric injuries. METHOD: 32 patients with electrical contact injuries were evaluated by serial electrocardiograms (ECG), creatine kinase (CK) and creatine kinase MB isoenzyme (CK-MB) determinations. The patients were divided into two groups according to the ECG. RESULT: 17 patients showed abnormal ECG (Group A), and 15 normal ECG (Group B). Positive correlations were noted between CK, CK-MB and the size of electric burns. The difference of serum CK-MB and 1.1% of total CK (CK-MB in skeletal muscle) was defined as CK-MB difference. CK, CK-MB, CK-MB/total CK ratio and the CK-MB difference were significantly higher in group A than in group B (all P < 0.05). The CK-MB difference of 88% (15/17) patients in group A was more than 25 U/L, whereas that was 6.7% (1/15) in group B (P < 0.01). Six patients with CK-MB difference greater than 100 U/L had obvious cardiac damage. CONCLUSION: Cardiac damage is a common complication of electric injuries. The cardiac damage could be estimated by serum CK-MB, particularly by the CK-MB difference. The CK-MB difference might be a useful indicator for evaluating the severity of the cardiac damage.

[Repair of articular cartilage defect with a large area: an experimental study].

OBJECTIVE: To compare and evaluate potentialities and biological characteristics of grafts of chondrocytes cultured on fascia, periosteum and articular cartilage in repairing large defects of articular cartilage. METHOD: The large defects of articular cartilage were repaired with grafts of freeze-stored and fresh chondrocytes cultured on fascia, periosteum and articular cartilage, which were evaluated by a number of observation methods. RESULT: The fresh and freeze-stored fascial chondrocytes were superior to free grafts of chondrocytes in potentiality of formation of new cartilaginous tissue, structure and metabolism of newly formed tissue. Relatively good results were also obtained similarly by using fresh autografts of periosteum and cartilage. CONCLUSION: Chondrocytes can be cultured normally on fascia, which can be regarded as an ideal carrier of chondrocytes.

[Tissue engineering of hyaline cartilage].

OBJECTIVE: To study the new way to repair the defect of the articular cartilage. METHOD: The articular-epiphyseal cartilage complexes from 8 newborn rabbits were enzymatically dissolved, and the chondrocytes were seeded onto the fibrous chitin mesh in a 96-well plate in DMEM medium. RESULT: Cultured for 21 days, the "membrane-like cartilage" was strong enough to be transferred. At the 110th day, the semitransparent cartilage discs were 4.4 mm in diameter and 14.8 mg in weightness. Staining of SatranO showed that engineered "cartilage" was bound of GAGs and the chondrocytes expressed the mRNA of type II collagen by in situ hybridization, which is resemble to that of the normal hyaline cartilage beside the biodegraded fibrous chitin embedded in. The "membrane-like cartilage" cultured for 21 days was grafted into the articular cartilage defects of knee of adult rabbits. Seven of 9 defects healed with new cartilage and the structure was well organized in 16 weeks. CONCLUSION: The biodegradable fibrous chitin is a good "matrix" for cartilage tissue engineering. The engineered "cartilage" is true hyaline cartilage, which is a new "alive cartilage tissue" for the repairment of the defect of articular cartilage.

[The detection of circulating hepatocellular carcinoma cells in peripheral venous blood by reverse transcription-polymerase chain reaction and its clinical significance].

OBJECTIVE: To detect circulating hepatocellular carcinoma by demonstrating hepatocellular carcinoma cells-associated mRNA in the nuclear cell component of peripheral blood (PBL). METHOD: Peripheral blood (5 ml) samples were obtained from 93 patients with hepatocellular carcinoma (HCC) and from 37 controls (15 controls with liver cirrhosis after hepatitis B, 12 chronic hepatitis B, and 10 normal liver function). To identify HCC cells in peripheral blood, liver-specific human alpha-fetoprotein (AFP) mRNA was amplified from total RNA extracted from whole blood by reverse transcription-polymerase chain reaction. RESULT: AFP mRNA was detected in 50 blood samples from the patients with HCC (53.8%). In contrast, there were no clinical control patients whose samples showed detectable AFP mRNA in PBL. The presence of AFP mRNA in blood seemed to be correlated with the stage (by TNM classification) of HCC, the serum AFP value, and the presence of intrahepatic metastasis, portal vein thrombosis, tumor diameter and/or distant metastasis. AFP mRNA was detected in the blood of 21 patients showing metastasis at extrahepatic organs (100%) in contrast to 29 of 72 patients without metastasis (40.3%). CONCLUSION: The presence of AFP mRNA in peripheral blood may be an indicator of malignant hepatocytes, which might predict hematogenous spreading metastasis of tumor cells in patients with HCC.

[Relationship between dental severe attrition and the temporomandibular glenoid morphology].

This study used the indices of the temporomandibular glenoid and posterior slope of the eminence by tomography to determine the effect of severe dental attrition on temporomandibular glenoid morphology. The results showed: the temporomandibular glenoid indexes in the group with dental severe attrition and in the control group were respectively 91.49 +/- 8.20 and 92.54 +/- 8.38 (P > 0.05), and the indices of posterior slope of the eminence were 49.79 +/- 5.84 and 50.38 +/- 4.58 (P > 0.05), the difference was not statistical. It was concluded that severe dental attrition would not certainly cause abnormal temporomandibular glenoid.

Ultrastructural observations on the formation and metabolism of calcareous corpuscles in Cysticercus cellulosae.

AIM: To study the formation and metabolism of calcareous corpuscles from Cysticercus cellulosae at the ultrastructure level. METHODS: Transmission electron microscopy. RESULTS: The developmental processes of calcareous corpuscles could be divided into two stages: the intracellular formation stage and the extracellular metabolic stage. The calcareous corpuscles were formed in a cell which we named calcareous corpuscle forming cell. At the early stage of the formation, the corpuscles appeared to be secretory granules in the cells. With the development of the corpuscles, they became bead-shaped and lamellae-like, then the calcareous corpuscle forming cell enlarged and the organellae degenerated. Finally the corpuscles gathered to form particle substances with black dense background, while the nucleus and organellae of the forming cell all disappeared. There were 1-3 or 10-20 calcareous corpuscles in a mature forming cell. Then, the corpuscles were released to the parenchymal tissues and gradually appeared to be concentric lamella or an empty cavity during the metabolic process. CONCLUSION: The calcareous corpuscles were formed in calcareous corpuscle forming cell and consumed in metabolic process in the parenchymal layer of Cysticercus cellulosae.

[Study on the Rheum palmatum volatile oil against HBV in cell culture in vitro].

The antiHBV effect of Rheum palmatum Volatile oil was studied by using 2215 cell line transfected with HBV DNA. At the same time MTT method was applied for the detection of cytoxicity of drugs, selecting acyclovir(ACV) as control medicine. It turns out that the toxic concentration of Rheum palmatum Volatile oil for 50% cells was (CD50) > 1.25 x 10(-1) g/L. When concentration was below 0.625 x 10(-1) g/L, the survival rate of cells was over 90%. The maximum inhibitory rates for HBsAg and HBeAg were 70.71 +/- 5.4% and 30.99 +/- 5.3% respectively. This shows Rheum palmatum Volatile oil possesses the effect of antiHBV in vitro.

[Direct collection with graphite probe filters and determination of trace cadmium in atmospheric particulate matter by graphite probe furnace atomic absorption spectrometry].

After atmospheric particulate matter was directly collected with graphite probe filters, the trace cadmium in APM was determined by graphite probe furnace atomic absorption spectrometry. The results showed that the absorbance (peak-area mode)and the concentration of cadmium had a excellent linearity in the range of 0-50 microg/L. The characteristic mass and detection limit (3 sigma) were 2.48 and 1.86 pg, respectively. Its relative standard deviation was 2.06% (n = 11). The results of analysis of the National Bureau of standards of America, Standard Reference Material (SRM) No. 1648, Urban Particulate Matter, gave a recovery of 90.15-99.29% with RSD of 4.09%. The method proved to be accurate, sensitive, rapid and simple.
