[Prevalence of celiac disease in patients with chronic diarrhea].

The objective of this research was to study the frequency of celiac disease in patients with chronic diarrhea. The biopsy materials of the small intestine and levels of antibodies to alpha-gliadin of class A immunoglobulins (IgA) and tissue transglutaminase were studied in 206 patients with chronic diarrhea. Morphologic celiac-specific symptoms were discovered in 35 (16.9%) patients. Symptoms of the total atrophy were discovered in 28 patients (13.5%); those of subtotal one were found in 7 (3.4%) patients. The increase of antibody levels to IgA alpha-gliadin and tissue transglutaminase was discovered in all 35 patients. Their average level made up 123.7 21.2 units per milliliter and 48.7 11.3 units per milliliter, respectively. It was possible to observe the typical celiac form only in 4 (11.4%) patients; the latent form was found in 30 (85.7%) patients, and the torpid (refractory) form was discovered in 1 (2.8%) patient. The frequency of celiac disease in patients with chronic diarrhea is equal to 16.9%. Patients with the latent form of the disease prevail among patients with celiac disease. Immunological screenings with the subsequent morphologic study of the mucous coat of the small intestine should be prescribed to all patients with the chronic diarrhea syndrome to enable the early diagnostics of celiac disease.

[Heterotopy of the mucous membrane in the fundic part of stomach into an adenomatous polyp of the common bile duct with an ulcerous affection formed in it].

The surgical material of 1,340 cholecystectomies as a result of cholelithiasis was analyzed. Heterotopy of the stomach mucous membrane into gallbladder was discovered in two cases; its heterotopy into the common bile duct with an ulcer formed in the heterotopic section of the mucous membrane in the fundic part of stomach was discovered in one case.

[Clinical-functional and morphological changes in the large intestine in patients with chronic abdominal ischemia].

The article presents a study of clinical variants of ischemic lesions of the large intestine (ILLI) in patients with the chronic abdominal ischemia (CAI) syndrome. There was an examination of 42 patients with putative ILLI, who had a cardiovascular pathology at the background, which led to significant hemodynamic disorders of the blood flow in unpaired visceral branches of the abdominal aorta. The authors performed a complex diagnostics of ILLI including anamnestic and clinical data, laboratory and morphological assessment of the large intestine and vessels providing its blood supply. Based on the data collected, other functional and organic diseases of the large intestine (LI) were excluded. It was revealed that the final result of ILLI differed depending on the ischemia degree--from reversible functional disorders to high-grade organic lesions of the LI. Each clinical variant of ILLI has its own clinical manifestations as well as functional, organic and morphological peculiarities of changes in the LI revealed by a laboratory and histological examination of the LI.

[Changes in the liver in glutenic enteropathy].

AIM: To evaluate frequency of clinical manifestations, biochemical and morphological alterations characteristic of liver affection in patients with glutenic enteropathy (GEP) in correlation with GE duration and severity of malabsorption syndrome (MAS). MATERIAL AND METHODS: 116 GEP patients have undergone clinico-laboratory examination, intestinoscopy with biopsy of jejunal mucosa, puncture biopsy of the liver with morphological examination of the specimens, ultrasonography of the liver. In addition, the immune status was assessed and tests for markers of hepatotropic viruses B and C were made. RESULTS: Clinical symptoms, typical for hepatic lesion in GEP patients, such as yellowness of the skin and scleras, palmar erythema, vascular stella and hepatomegaly were found in 7.2% cases. Hyperaminotransferasemia was detected in 48.4% of new cases of celiac disease, in 18.1% of patients on strict aglutenic diet. Aminotransferase hyperactivity detection was associated with severity of MAS. Morphological alterations in the liver in celiac disease are characterized by fat dystrophy, portal and periportal hepatitis, steatohepatitis or fibrosis. Combined therapy of celiac disease including strict aglutenic diet, correction of metabolic disturbances was not always effective in correction of metabolic processes in the liver. In new GEP cases early adequate therapy was most effective. CONCLUSION: The course of glutenic enteropathy is accompanied with clinicomorphological alterations in the liver. This necessitates use of drugs improving liver function.

[Incidence of celiac disease in patients with chronic diarrhea].

AIM: To study prevalence of celiac disease (CD) among patients with chronic diarrhea (ChD). MATERIAL AND METHODS: Serum levels of IgA-antibodies to gliadin, endomysium, reticulin and tissue transglutaminase were examined in 206 patients with CD. Biopsies were obtained from a distal portion of the duodenum or a proximal portion of the jejunum. RESULTS: CD was diagnosed in 35 (16.9%) of 206 patients with ChD. The disease was typical in 5 (2.4%) patients and was latent in 30 (14.5%) patients. Antibodies to gliadin, endomysium, reticulin and tissues transglutaminase in diagnostically significant titers were detected in the serum of all the examinees with CD. Formation of the antitissue antibodies occurred because of destructive-dystrophic alterations of the connective tissue of the lamina propria mucosae pointing to the autoimmune nature of a pathological process in celiac disease. CONCLUSION: To diagnose CD in ChD patients, it is necessary to supplement standard examination with duodenobiopsy, tests for antibodies to gliadin, tissue transglutaminase and to tissue structures--endomysium and reticulin.

[Hemorrhagic shock. A new concept of endoscopic hemostasis and prospects of the non-surgical treatment of ulcer and gastroduodenal hemorrhages complicated by hemorrhagic shock].

A new direction with the use of endoscopic hemostasis (EH) was offered in the complex program of metabolic rehabilitation of patients with ulcerous gastroduodenal bleedings (UGDB) and hemorrhagic shock (HS). There was a study of the results of treatment of 44 patients (29 men and 15 women at the age from 61 to 77) with severe concurrent diseases and UGDB complicated with HS. Recurrences of UGDB in case of HS are a factor of high operative-anesthetic risk. Endoscopic preventive metabolic hemostasis is a prospective direction in rehabilitation of patients with UGDB complicated with HS. It is necessary to improve the EH methods for treatment of continued arterial bleedings from giant gastroduodenal ulcers.

[Role of stress in the etiopathogenesis of duodenal ulcer in young patients].

The analysis of the acid-forming function of the stomach, both basal and stimulated with insulin, atropine test, tests for the content of the adrenocorticotropic hormone, somatotropic hormone, thyrotropic hormone, triiodothyronine, thyroxin, cortisol in the blood and the content of gastrin in the blood and gastric juice, activity of the processes of lipid peroxidation, content of ceruloplasmin in the blood, content of Helicobacter pylori (Hp) in the gastric mucosa were conducted in 68 men and 64 women at the age from 18 to 40 years suffering from gastroduodenal ulcer in order to examine the role of stress in ulcer formation. 38 patients with duodenal ulcer took proton pump blockers and antioxidants for 3 years whenever they were in a stress situation. The research revealed that young patients with duodenal ulcer show enhanced sensitivity of parietal cells and sympathoadrenal system to stressful situations, and stimulation of the acid-forming stomach function takes place under the effect of both catecholamines and gastrin. Men can have both Hp-dependent and Hp-independent duodenal ulcers, while women have mostly Hp-dependent duodenal ulcers. Prescription of proton pump blockers of and antioxidants for patients with duodenal ulcer, who are in a stressful situation, reduces the frequency of duodenal ulcer recurrences.
