[Evaluation of experimental arterial occlusion, with special reference to its complications (micro-spheres, Histo-Acryl, Paladur, alternating and direct currents (author's transl)].

Permanent arterial occlusion can be obtained experimentally by using plastic micro-spheres, polymerising materials (Histo-Acryl, Paladur) and direct current thrombosis. Certain limitations in their use arise from the properties of the substances. In our experience, plastic microspheres are not suitable for blocking entire organs or large vascular territories. They are more appropriate for occlusion of individual, small vessels by super-selective injections. Sole use of polymerising materials is not possible; they are best used in combination with other techniques (magnets, balloon catheters). Induction of thrombosis by small direct currents (50 volt, 10 mA). produces permanent and complete occlusion. The method can be used selectively or super-selectively. Unwanted thrombosis can be avoided by using catheters with the smallest possible external diameters.

Posttraumatic bilhemia: conservative management by percutaneous drainage.

Posttraumatic bilhemia complicating liver trauma has been managed surgically so far. A case report of a biliary-venous fistula resulting from blunt hepatic trauma managed successfully by percutaneous drainage is presented. A catheter was placed in the right hepatic lobe for reducing intrahepatic pressure. This procedure was followed by a dramatic decrease of bilirubin blood level. This management emphasizes the role of interventional radiology in the treatment of complicated blunt hepatic trauma.

Value of 3D CT imaging in fractured os calcis.

The authors report their experience with 3D computed tomography (CT) in 23 patients with fractured os calcis (several types of fractures) in comparison to classical 2D CT. The diagnostic accuracy was not found to be improved by the three-dimensional representation. 3D CT is affected by reconstruction artifacts and strongly depends on the choice of threshold. However the 3D visualization is appreciated by the surgeons.

[Demonstration of subchondral density pattern using CT-osteoabsorptiometry (CT-OAM) for the assessment of individual joint stress in live patients].

The researches of Pauwels and those following him have demonstrated that the subchondral bone density distribution below the surface of a joint is a metrical parameter which mirrors the predominant stress acting on that joint. Their technique of x-ray densitometry cannot, however, be used during life. By employing computer tomography, a new method has been developed--CT-osteoabsorptiometry (CT-OAM)--which can be used to obtain the density distribution pattern in the living subject. By means of a comparative investigation on specimens which were examined both with traditional x-ray densitometry and with CT absorptiometry, it has been shown that the new method can produce the same results, but providing the great advantage to be used on the living. In addition, the density distribution pattern of the glenoid cavity has been examined in patients with various shoulder conditions, and also in gymnasts who exercise on the rings. The distribution of subchondral bone density showed different patterns in the different groups examined. Both the comparison of the methods and the subsequent study confirmed that CT osteoabsorptiometry enables assessment to be made of the individual long-term stresses acting on a living joint.

[Expert evaluation in bony injuries of the spine].

The conventional way of appreciating working capacity is no longer satisfying considering the improved diagnostic tools with increasing frequency of various deformities, instability and ankylosis and their combinations. As in the evaluation of peripheral joint function a concept is proposed, which is based on the structural changes in the motion segments. The concept is practicable in all parts of the vertebral column and allows a differentiated judgement within the usual percentages.

[Clinical and computer tomography results following Chiari pelvic osteotomy].

22 hips in 20 patients operated with Chiari's pelvic osteotomy between 1970 and 1980, were examined with clinical methods and computerized tomography. With computerized tomography one is surely able to see if the femoral head is completely covered with pelvic bone or not. It was completely covered in 36.4% only. Especially the dorsal part of the femoral head often was not. We report on the influence of former and contemporaneous intertrochanteric osteotomy on the result. The influence of the degree of covering of the femoral head upon absence of postoperative pain, motility of the hip, Trendelenburg's sign, and contentment of the patient is discussed.

Imaging in cardiac mass lesions.

In 26 patients with cardiac mass lesions confirmed by surgery, diagnostic imaging was performed preoperatively by means of two-dimensional echocardiography (26 patients), angiography (12 patients), correlative computed tomography (CT, 8 patients), and magnetic resonance imaging (MRI, 3 patients). Two-dimensional echocardiography correctly identified the cardiac masses in all patients. Angiography missed two of 12 cardiac masses; CT missed one of eight. MRI identified three of three cardiac masses. Although the sensitivity of two-dimensional echocardiography was high (100%), all methods lacked specificity. None of the methods allowed differentiation between myxoma (n = 13) and thrombus (n = 7). Malignancy of the lesions was successfully predicted by noninvasive imaging methods in all six patients. However, CT and MRI provided additional information concerning cardiac mural infiltration, pericardial involvement, and extracardiac tumor extension, and should be integrated within a preoperative imaging strategy. Thus two-dimensional echocardiography is the method of choice for primary assessment of patients with suspected cardiac masses. Further preoperative imaging by CT or MRI can be limited to patients with malignancies suspected on the grounds of pericardial effusion or other clinical results.

Malignant triton tumor of the thyroid gland.

A case of a malignant triton tumor, a malignant schwannoma with a rhabdomyoblastic component is described. The tumor appeared in the right thyroid gland with no relationship to a peripheral nerve. Beneath pulmonary metastases the tumor produced bone metastases and showed responsiveness to radiotherapy and treatment with ifosfamide. These findings are uncommon in malignant triton tumors.
