Treatment of HBsAg-,HBeAg-positive chronic active hepatitis with adenine arabinoside. A case report with clinical remission and seroconversion to anti-HBs.

A 26-year-old patient developed HBsAg-, HBeAg-positive chronic active hepatitis arising from an acute infection 6 months earlier. Treatment with adenine arabinoside (Ara-A), 15 mg/kg/day administered as overnight infusions for three weeks, resulted in clinical and complete biochemical remission with normalisation of aminotransferases, loss of HBsAg, HBeAg and DNA polymerase, and appearance of Anti-HBs during follow-up for 9 months. Treatment of chronic hepatitis B infection using Ara-A for several weeks may be especially effective when initiated early in the course of the disease.

The effect of cholecystectomy on the induction of colorectal tumors in mice by 1,2-dimethylhydrazine.

The influence of previous cholecystectomy (CCE) on the 1,2-dimethylhydrazine (DMH)-induced colorectal tumorigenesis was examined in 497 female NMRI mice after 14, 20, 24, 28, 33, and 36 weeks of induction time. There were only minor differences between the cholecystectomized and the non-cholecystectomized animals with regard to the percentage of tumor-bearing animals, number of adenomas, and location of the tumors. Only after an induction time of 20 weeks, the operated group had significantly more carcinoma-bearing animals than the non-operated group (P less than 0.01, chi 2-test). The implication of these results on known data of epidemiologic studies in human colorectal cancer are discussed.

Adenomas of the large intestine after cholecystectomy.

The frequency of adenomas of the large intestine in 331 cholecystectomised patients who underwent total colonoscopy was compared with that of a control group of patients with asymptomatic cholelithiasis who were matched for age and sex. Whereas no significant difference in the frequency of adenomas was found between two groups, a subgroup of patients aged 60-80 years with a postcholecystectomy interval of 10 years or greater exhibited a significantly (p less than 0.05) greater frequency of adenomas (38.5%) than matched patients with a postcholecystectomy interval of less than 10 years (21.8%) and matched controls with cholelithiasis (23.7%). This increase in the frequency of adenomas was primarily accounted for by an increase in the percentage of tubular adenomas (p less than 0.05) and corresponded to an increase in the frequency of cancer (p less than 0.05) of the large bowel.

[Cholecystectomy, cholelithiasis and cancer of the large intestine].

In a retrospective post-mortem study the connection between the post-cholecystectomy state, cholelithiasis and carcinoma of the large intestine was evaluated in 333 cases and compared with an age and sex matched control group and a group with gastric carcinoma. It was established that cholelithiasis (without cholecystectomy or contracted gallbladder) occurred significantly more frequently (24.9%) in patients with colonic carcinoma than in the control group (15.3%) or the gastric carcinoma group (12.6%) (P less than 0.001). In contrast the rate of cholecystectomy in patients with colonic carcinoma was not significantly increased (6.3%) when compared with both control groups (4.6 and 4.8%). Separate assessment of males and females showed that females had cholecystectomy and contracted gallbladder significantly more frequently (P less than 0.05). These figures suggest that the clinically observed association between cholecystectomy and colonic carcinoma reflects only a - not necessarily causative - connection between gallstone disease and colonic carcinoma.

Investigations of hyperplasiogenous gastric polyps by partial reconstruction.

Four focal foveolar hyperplasias (hyperplastic polyps) and 41 hyperplasiogenous polyps were studied by serial sections and partial graphic reconstruction. Position of mitoses and the border between foveolar epithelium and glandular epithelium were recorded. The results show that the hyperplasiogenous polyp essentially is a hyperplasia of the gastric mucosa in which the foveolar epithelium always plays a major role. The zone of mitotic activity, especially the transitional zone between proliferating and differentiated cells, is widened. The glands usually show atrophy, but also may be normal or hyperplastic. An overaging of foveolar epithelia may contribute to development of erosions frequently seen. Splitting of the muscularis mucosa seems to be a secondary phenomenon which in turn may contribute to cystic dilation of foveolae. Because of its characteristics in structure and cell proliferation the term "hyperplasiogenous polyp" seems to be justified and preferable as compared to the term "hyperplastic polyp" (WHO-nomenclature; Oota, 1977).

[Behavior of resorbable and nonresorbable suture material in lymph vessel suture].

After transverse division of the abdominal thoracic duct of the rat, 14 anastomoses were performed using synthetic absorbable (Polyglactin 910, Vicryl) and synthetic non-absorbable (Polyamid 6.6, Ethilon) suture material. The anastomoses were achieved by means of a tension-free technique using interrupted sutures. The follow-up period was from 28 to 133 days. Clinical observation showed that all anastomoses were patent. However, with the aid of staining methods only five out of seven anastomoses were shown to be patent. Using absorbable suture material, a lumen was demonstrable at all anastomoses, while using non-absorbable suture material this could be proved in only four out of seven anastomoses. The foreign body reaction diminished with time when absorbable material was employed, whereas it persisted with non-absorbable material.

[Malignant polyps and early recognized carcinomas of the rectum. Morphological aspects and selective surgical treatment (author's transl)].

The lymphatic drainage of the colon and rectum originates in the submucosal layer. Therefore metastases do not arise from carcinomas confined to the mucosa, and for the same reason the WHO lately recommended to call these lesions severe atypism. The term adenocarcinoma should be reserved in the case of infiltration of the carcinoma into the submucosa. According to the features outlined above adenomas of the rectum showing severe atypism can be removed by careful excision of the mucosa. Even adenomas containing an early diagnosed adenocarcinoma up to 2.5 cm in diameter often can be cured by a local resection of the rectal wall, if no metastases have occurred. From 1971--December 1, 1977, 12 rectal adenomas with severe atypism and 14 rectal adenomas with adenocarcinoma had been removed locally without complications. After 6 years there was no recurrence of carcinoma.
