Metabolic products of microorganisms 167. Cyclopaldic acid from Aspergillus duricaulis. 1. Production, isolation and bioloical properties.

In the course of a screening for new metabolites from fungi we isolated a substance with antimicrobial activity from cultures of Aspergillus duricaulis (CBS 481.65) (Tü 679). It was antagonized by putrescine, spermidine, spermine, arginine, citrulline, lysine, ornithine, in higher concentration by aspraagine and glutamine too. The effect of ethylenediaminetetraacetate on the susceptibility of Streptomyces viridochromogenes (Tü 57) and Bacillus subtilis ATCC 6051 to this antibiotic has been studied. The substance was characterized and identified as cyclopaldic acid.

[The syndrome of the fifth day (author's transl)].

After operation of uncomplicated forms of appendicitis in children postoperative complications characterised by sudden vehement stomach-ache, quickly increasing tension of the abdominal wall, rise of temperature, pulse and white blood cells as well as vomitus, mostly on the fifth post-operative day could be observed. French authors call it "the syndrome of the fifth day". We observed this syndrome in 12 patients of 2.814 appendectomies in infancy. The treatment demands an immediate introduction of an intensive care by gastric suction intravenous fluid replacement and high doses of antibiotics. If these measures are not followed by an improvement within a few hours, a relaparotomy will be indicated. Then liquid pus is to be found in the abdomen and only a fibrin film in the primary operation field. In single cases we found a partial suture dehiscence. The aetiology of this complication has not been cleared yet.

Endocrine function after antineoplastic therapy in 22 children with acute lymphoblastic leukaemia.

In 22 children who were in complete remission after acute lymphoblastic leukaemia endocrinological investigations were performed 5-12 weeks after cessation of therapy. The children had received central nervous system irradiation (tele-Co60, 850-1800 rad), and long term, aggressive cytostatic drug therapy during 21 to 36 months. Growth hormone, TSH, thyroxine, LH, FSH, cortisol secretion, and urinary concentrating capacity were found to be normal, with a few exceptions where borderline results were obtained.

[Therapy of periodontal diseases].

An analysis of general periodontal treatment carried out in the Departement of Cardiology, Periodontology and Preventive Dental Medicine of the University of Zurich, Dental Institute was undertaken with the aid of 50 randomly selected patient case histories. Immediately following therapy, it was found that a normal marginal periodontium had been established in all cases. A prerequisite for this success was effective patient oral hygiene. The duration of total treatment depended upon diagnosis and treatment plan and ranged from 18 to 62 h. Documentation of each phase of treatment was responsible for a significant part of the total therapy time. In view of the relatively large amount of time necessary to treat periodontal disease, future emphasis should be placed upon the refinement and propagation of preventive measures.

Investigatiaons on drug-resistance of Shigella sonnei bacilli isolated in Poland in the years 1959-1972.

2538 strains of Shigella sonnei bacilli, isolated from patients in the years 1959-1972, have been tested for drug-resistance. The strains examined came from different regions of Poland. Drug-resistance has been determined by the method of "multi-arm" disks of filter-paper. In investigations 12 chemotherapeutics have been employed. The tests performed have shown that 63% of strains were resistant to one or more chemotherapeutics. The percentage of streptomycin-resistant strains, found in the investigations, was higher than that found by other authors. In the material examined 24% of strains were resistant to antibiotics, and sensitive to sulfathiazole. No strains resistant to eight or more chemotherapeutics applied have been found.

[Outpatient management of diabetic children and adolescents: control of fractional 24-h glucosuria (author's transl)].

The optimal ambulatory treatment of diabetic children requires the cooperation of the patient and his family. Following a careful and extensive information about the course of diabetic mellitus, its management, and its possible complications, the patients and their parents have to take charge of an important part of the metabolic control. Fractionating 24-h-urines in three portions and estimation of the glucose excretion using the Clinitest-method proved to be a valuable tool to judge the metabolic control and to adjust the insulin doses. The comparison of the glucose excretion in 595 fractionated urines of 30 juvenile diabetics estimated with both the Clinitest-method and polarimetry in the clinical laboratory showed good agreement for low glucose conentrations. The discrepancies between the two methods increased with rising glucose concentrations and increasing urine volumes. 139 insulin-dependent diabetic children and adolescents were presented to the Children's Hospital outpatient clinic every 6-7 weeks for a physical examination and measurements of the 24-h-glucose excretions as well as the postprandial blood glucose concentrations. The average glucose excretion in two groups of patients with one or two daily injections of insulin was 24 and 30 g/day, respectively, which is higher than the recommended limit of 20 g/day. The glucose excretion was not significantly influenced by the frequency of insulin administration (one or two injections per day), by the age of the patient or by the duration of the diabetes mellitus. The good agreement of the evaluations of the 24-h-glucosuria using Clinitest on one side and polarimetry on the other proves regular controls at home by the patients and their parents to be useful and to facilitated the outpatients management of juvenile diabetics. Measuring the 24-h-glucose excretion at least 2-3 times a week seems desirable.

Development of subcutaneous fat in infancy. Standards for tricipital, subscapular, and suprailiacal skinfolds in german infants.

In order to assess the nutritional status of healthy infants in Berlin, the tricipital (TRI), subscapular (SCA), and suprailiacal (SIL) skinfolds were measured at several instances (2--14 times) during regular presentations at the well baby clinic offices in 265 infants of normal gestation, 140 boys and 125 girls, using the Holtain skinfold caliper. The characteristic pattern of skinfold development in infancy includes a rapid increase in width of all diameters until 3--5 months of age, and a gradual decrease thereafter. This diminution of skinfold thickness is more pronounced at the trunk (SIL and SCA) than at the limbs (TRI), indicating a change in distribution of subcutaneous tissue during infancy. Compared to the present study, previous investigations in Great Britain and Sweden have shown a maintenance of maximal skinfold values rather than a decrease during the second half of the first year. It is suggested that differences of feeding habits and calorie intake may be responsible for these discrepancies. The results of this mixed longitudinal study, performed in 1974/1975, were computed to calculate centile curves of the skinfold development in infancy, which may serve as standards for infants living under similar socioeconomic and nutritional conditions.
