The effect of cefuroxime axetil on the faecal flora of healthy volunteers.

The effect on the faecal flora of cefuroxime axetil, an oral ester of cefuroxime, was examined in ten healthy volunteers following a dose of 250 mg twice a day for 41/2 days. The mean blood concentration 2 h after the ninth dose was 3.6 mg/l and the urinary excretion was approximately 40% in 6 h and 50% in 24 h. Two volunteers developed mild diarrhoea and one candida vaginitis. Although there was variation between volunteers in the changes found in the faecal flora, the major effects were a reduction in the total anaerobic bacterial count, particularly where high levels of cefuroxime were present in the faeces and a significant decrease or elimination of strains of Enterobacteriaceae plus an increase in the streptococcal count. Six volunteers showed an increase in the candida count. Clostridium difficile was not isolated from any volunteer and toxin was not present in the two volunteers who developed diarrhoea. Four volunteers showed high levels of cefuroxime in the faeces on the fourth or fifth days of treatment.

Treatment of acute uncomplicated urinary tract infections with single daily doses of cefuroxime axetil.

Cefuroxime axetil in a single daily dose of 250 mg for ten days was given to 75 women with symptoms of acute uncomplicated urinary tract infections. Fifty-nine women were found to have significant bacteriuria but one was excluded as urethral catheterization was required. The dose was taken at night with a milk drink. Ninety-five per cent of women had a clear urine during treatment. Compliance studies showed that antimicrobial activity was detectable in early morning urines up to 8-10 h after the dose. Post treatment, 93% of women were clear of their original infecting organism but five women had become reinfected with a different strain of Escherichia coli. At the six week follow up 98% of women were still clear although one further reinfection had occurred. The overall cure rate was 86%, including 11% reinfection. Adverse events developed in 17 (23%) of the 75 women with candida vaginitis (8) and diarrhoea (4) being most common. Cefuroxime axetil is a valuable therapy for the treatment of urinary tract infection particularly when due to beta-lactamase producing bacteria.

Teicoplanin in infections caused by methicillin-resistant staphylococci.

Serious infections by methicillin-resistant staphylococci, 26 caused by Staphylococcus aureus and two by coagulase-negative staphylococci, were treated with teicoplanin 200-800 mg daily, in eight cases supplemented with another antibiotic. The overall rate of clinical cure or improvement was 75%, and that of bacterial eradication 67%.

Pharmacokinetics of ofloxacin and the effect on the faecal flora of healthy volunteers.

Pharmacokinetic studies using a new oral quinolone ofloxacin were carried out in 12 healthy volunteers. The mean Cmax was 2.7 mg/l after the first dose of 200 mg ofloxacin rising to 3.4 mg/l after the seventh dose. The Tmax was between 1-2 h and the serum half life 5 h. Saliva concentrations matched serum levels but the absolute values were lower. Urinary concentrations ranged from 141 to 330 mg/l and the 12 h excretion was 62% after the seventh dose. Faecal concentrations were high and persisted for up to five days after the last dose. The major effect of ofloxacin on the faecal flora was the rapid and complete elimination of aerobic Gram-negative bacilli. Streptococci were generally increased but there was no change in the total anaerobic bacterial count. Pre-treatment composition of the faecal flora was re-established between 3 and 26 days after the last dose. Ofloxacin was well tolerated by the volunteers and only two complained of significant side effects, gastrointestinal disturbance and insomnia. Ofloxacin is a valuable addition to the range of antimicrobial agents available for the oral treatment of bacterial infection.

Open study of teicoplanin in gram-positive infections.

Eighteen patients with documented Gram-positive infections which included osteomyelitis, prosthetic infections, endocarditis, skin and soft tissue, and urinary tract infections were treated with teicoplanin. The organisms involved included Staphylococcus aureus (15 isolates of which six were methicillin-resistant-MRSA), Staphylococcus epidermidis (two), Streptococcus faecalis (one) and Streptococcus milleri (one). Clinical success occurred in all seven patients with skin and soft tissue Streptococcus milleri (one). Clinical success occurred in all seven patients with skin and soft tissue infections (with bacterial persistence in three out of the seven), in three patients with bacteraemia endocarditis, and in one of the three patients with chronic osteomyelitis. In four patients with prosthetic bone and joint infections, clinical improvement followed removal of prostheses. Adverse effects occurred in two patients and these included one patient with a rise in serum aspartate aminotransferase and bilirubin and one patient with a rise in blood urea, both of which returned to within normal limits on discontinuing the drug. The study showed that teicoplanin is a safe and effective antistaphylococcal agent.

The effect of transdermal estradiol on hormone and metabolic dynamics over a six-week period.

Seventeen healthy postmenopausal women who had subjectively noted eight or more hot flashes per day and who objectively demonstrated four or more vasomotor flushes of 1.0C or more during eight hours of continuous thermography were studied. They were randomly allocated in a double-blind fashion to either 50 micrograms/day of transdermal estradiol (E2) patch or placebo. Application of the first patch was followed immediately by repeat eight-hour thermography, with hourly measurements of E2 and luteinizing hormone (LH). In the transdermal E2 group only, significant elevations of E2 (mean 91 pg/mL) were noted at two hours, and LH was suppressed after eight hours (P less than .05). There was no immediate effect on vasomotor flushes. Treatment was continued for six weeks, with daily subjective recording of hot flash frequency. Patients on transdermal E2 reported a significant (P less than .001) fall in hot flashes over four weeks, after which the rate stabilized. An initial decline in the placebo group was not statistically different from baseline. Eight-hour thermography was repeated after six weeks of treatment. Patients on transdermal E2 demonstrated an 85% decrease from baseline in vasomotor flushes (P less than .01). No effect on total cholesterol or its subfractions, renin substrate, or aldosterone was found. Serum E2 levels fell by 50% in 24 hours after patch removal. Endometrial histology and vaginal cytology showed an estrogen effect.

The effects of unstable clients in adolescent treatment: contagion versus continuity.

This study examined the effects of integrating an unstable group of seriously disturbed adolescents into an existing day treatment program. Behavioral data were collected for an established group of subjects who were in treatment one year prior to and one year after the arrival of a new group of severely disturbed adolescents. It was found that the disruptive behavior displayed by the new admission subjects was not imitated by the subjects of the established group. Rather, there was a significant decrease in aggression and in the overall number of psychiatric events for the established group. The treatment milieu was seen to be resistant to the effects of the new admissions largely due to the structured form of treatment.

The effect of handcream on the antibacterial activity of chlorhexidine gluconate.

A series of handwashing experiments compared the residual effect on Escherichia coli of non-medicated soap, chlorhexidine detergent followed by an anionic handcream, chlorhexidine detergent followed by a non-ionic handcream and chlorhexidine detergent alone. Handcreams containing an anionic emulsifying agent reduced the antibacterial effect of chlorhexidine.

Nocardia asteroides osteomyelitis.

Nocardia asteroides osteomyelitis has previously been described only in acutely ill patients. It may occur as an isolated event or part of the disease spectrum of disseminated nocardiosis. An 84 year old immune competent man with N. asteroides right tibial osteomyelitis had an unusual presentation, presumed mechanism of disease and outcome. His course affords an opportunity to review the literature on N. asteroides osteomyelitis and to discuss the unique features of his case.

Krypton-81m imaging of the right ventricle.

We report the development of a method for obtaining right-ventricular radionuclide angiograms using continuous peripheral intravenous infusion of the ultra-short-lived nuclide krypton-81m. This tracer has a half-life of 13 sec, emits a single 190-keV photon, and is extremely insoluble. During infusion into a peripheral vein, Kr-81m achieves stable count rates over the right heart, and it is essentially completely cleared by the lungs during its initial pulmonary transit. Thus no interfering activity is present in the systemic circulation. Initial studies provide excellent data on right-heart anatomy and function.
