Attempted suicide with a pesticide mixture.

An attemped suicide with a pesticide mixture containing pentachlorophenol, benzene hexachloride, dieldrin, metaldehyde phenoxyacetate esters, paraquat, diquat and petroleum distillates, is reported. The prognosis initially seemed hopeless, but the patient was treated successfully by intensive care with early respiratory assistance.

Extreme pulmonary hypertension caused by mitral valve disease. Natural history and results of surgery.

Five hundred and eighty six patients with mitral valve disease were studied with cardiac catheterization between 1961 and 1972; 48 (8.2%) had extreme pulmonary hypertension (resting systolic pulmonary artery pressure of 80 mmHg or above and pulmonary vascular resistance of 10 units or greater) and of these patients, 27 underwent cardiac surgery. The operative mortality for mitral valvotomy was 11 per cent and for mitral valve replacement 56 per cent. The overall mortality was 31 per cent. The risks of operation were increased in those with a long history of cardiac symptoms, those over 50 years of age, and in the presence of associated aortic valve disease. The mean survival for those patients not having operation was only 2.4 plus or minus 0.5 years. The mean follow-up period for those surviving operation has been 5.8 plus or minus 0.6 years, and symptomatic improvement has been good.

cutaneous sarcoidosis in blood donation venepuncture sites.

Six patients were studied in whom sarcoidosis first showed itself by the development of granulomata at the site of previous venepunctures. The diagnosis was histologically confirmed in all cases. The treatment of the lesions with triamcinolone did not seem to hasten resolution.

Leucocyte migration inhibition and leucocyte adherence inhibition studies, using ferritin as 'antigen', in patients with malignant lymphoma.

Isoelectric focusing of ferritin prepared from three spleens involved by Hodgkin's disease demonstrated only minor differences from normal spleen ferritin. The leucocyte migration inhibition (LMI) and leucocyte adherence inhibition (LAI) assays were used to assess sensitization to Hodgkin's disease spleen ferritin and its component acidic and basic isoferritins in patients with malignant lymphoma compared to patients with other malignancies and control subjects. A difference in response to ferritin was demonstrated in both tests with patients with malignant lymphoma compared to controls, and in LMI test compared to patients with other malignancies. There were also significant differences in responses in, patients with malignant lymphoma compared to controls, against acidic and basic isoferritins in both tests, in lymphoma patients versus patients with other malignancies against acidic isoferritins in both tests, lymphoma versus other malignancy against basic isoferritins in the LMI test only. However, no significant differences, within each group, between responses with acidic and with basic isoferritins were demonstrated, and there was no correlation between individual patients' responses in the two tests.

The clinical presentation, treatment, and outcome of patients diagnosed with possible ectopic molar gestation.

The objective of this study was to determine the clinical presentation, treatment, and outcome of patients diagnosed with possible ectopic molar gestation registered with the Trophoblastic Disease Screening and Treatment Centre, Weston Park Hospital, Sheffield between 1986 and 2000. From the 5581 women registered, those with a diagnosis of ectopic molar pregnancy were identified from a computer database. Information regarding the relevant history of each patient and the clinical presentation, treatment, and outcomes of gestational trophoblastic disease (GTD) was determined by reviewing referral forms, case notes, and pro formas completed by the referring gynecologist. Histological review of the cases was undertaken where possible. Suspected ectopic molar gestations comprised 31/5581 (0.55%) of registrations. Known risk factors for ectopic pregnancy were identified in 79% of cases. Central histological review confirmed only six cases of GTD: three choriocarcinoma and three early complete moles. Four patients subsequently required chemotherapy. All patients are now in complete remission. We conclude that ectopic GTD is uncommon, with a UK incidence of approximately 1.5 per 1,000,000 births. Initial management is usually surgical removal of the conceptus, pathological suspicion of the diagnosis and registration with a screening center. Chemotherapy may be required and the prognosis is excellent.
