Importance of histologic grading in the prognosis of epithelial ovarian carcinoma.

Ovarian epithelial carcinomas still have a poor prognosis. Great efforts are being made to improve treatment modalities; chemotherapy in particular is in a period of dynamic development. A basic requirement for the necessary research is a thorough knowledge of the natural history of ovarian cancer and its prognostic influence. In this report the importance of tumor stage, histologic type, and degree of differentiation is analyzed in an unselected and well-controlled Swedish ovarian cancer series comprising 501 cases. The great prognostic significance of tumor grade for survival is demonstrated and it is proposed that this histopathologic factor, as well as tumor stage and histologic type, should be included in the international classification of primary epithelial tumors of the ovary.

Actuarial Analysis↗

The nephrotic syndrome in the Sudan with special reference to schistosomal nephropathy. A preliminary morphological study.

The morphology of 68 renal biopsies from the Sudan is analysed. Schistosomal infection was more common among the nephrotic patients than in the average population (32% against less than 1%). In the schistosomal group the high incidence of focal sclerosing glomerulonephritis (28%) and of secondary amyloidosis (38%) was remarkable. The history is described of a boy in whom repeated biopsies showed the decrease of glomerular amyloid depositions accompanied by mesangial cell proliferation following treatment for schistosomiasis.

Morphology of the spleen and lymph nodes in fatal visceral leishmaniasis.

Histological appearances of the spleen and lymph nodes were analysed in twenty fatal cases of human visceral leishmaniasis from Sudan. Marked atrophy of the splenic white pulp was associated with necrosis and fibrosis of thymus-dependent area, accumulation of parasite-containing histiocytes and plasma cell hyperplasia. Depletion of small lymphocytes in the paracortical areas of the lymph nodes was accomplained by proliferation of plasma cells and histiocytes in the paracortex. Depletion of small lymphocytes in thymus-dependent regions of lymph nodes and spleen is viewed as arising from immune suppression associated with antigen overloading or other factors, which may impair those aspects of lymphocyte-macrophage cooperation that are presumably necessary to kill the invading parasites.

[Vascular lesions in mucosal leishmaniasis].

In 20 our of 52 cases of mucosal- leshmaniasis vascular lesions of the exudative type were observed. Arteria and vena were equally involved. These lesions are characterized by oedematous imbibition of the vascular wall, formation of fibrinoid and fibrotic proliferation of the intima. In addition fibrin thrombus and inflammatory vascula lesions were also detected. In the development of vascular lesions authors emphasize the role of immunological processes in which immune-complexes and/or membrane-reactive autoantibodies have an importance.

Burkitt's lymphoma in the Sudan.

During the period 1962-73 twenty-nine cases of Burkitt's lymphoma were seen and examined histologically in Khartoum, Sudan. Burkitt's lymphoma formed 20% of the cases of childhood lymphomas. The average age at presentation was 6.8 years and the male to female ratio was 3.8:1. The clinical features, the anatomical localization and histological appearances were similar to those reported in the literature. The geographical distribution of the tumour showed close correlation to that of holo- or hyperendemic malaria.

Gastric cancer in Sudan.

All gastric tumours received in the University Department of Pathology and the National Laboratory between 1970-1974 were re-examined. Of 35 such tumours, 28 were carcinomas, 6 malignant lymphomas and 1 leiomyosarcoma. Histological grading showed that the majority of the patients reported in the very advanced stages of the disease. Accordingly, the resectability rate was low. The incidence of primary malignant lymphoma of the stomach (16.6%) is much higher than in Western countries but is in accord with reports from countries in the Mediterranean basin.
