[Posterior dislocation of the scaphoid: report of one case (author's transl)].

The authors have observed one case of dislocation of the scaphoid posterior to the radius. There had been a complex trauma of the wrist with disruption of the palmar and dorsal ligaments, the capitate bone being interposed between the scaphoid and the lunate. A good result was obtained by a closed reduction with fixation by two pins through the scaphoid, lunate and capitate.

[The treatment of bone lesions in myelomatosis (author's transl)].

41 patients suffering from a bone lesion due to myelomatosis were treated (32 multiple myelomata and 9 solitary myelomata). Diagnosis was established by biopsy in 9 cases. In 8 cases there was compression of the spinal cord or of a spinal root. 15 pathological fractures were treated by fixation and bony union was always obtained. In 7 other cases, fixation was used to prevent fracture and 2 patients had a total joint prosthesis. It is concluded that the treatment of the bony lesions of myelomatosis is comparable to that of bony metastases but the average survival is much longer.

[Treatment of Ewing's sarcoma by radiotherapy and adjuvant chemotherapy : results at 3 and 5 years (author's transl)].

The local treatment of Ewing's sarcoma by radiotherapy is classically linked to a very poor prognosis : 10 to 15% of 5 years survival, 75 to 95% of distant metastasis in 2 years. The combination of local radiotherapy to a systemic adjuvant chemotherapy have been recently shown to give a real benefit to this prognosis. The therapeutic trial of the EORTC and GETO, presented here allows us to hope a disease free survival at 5 years for 50% of our patients and a complete survival at 5 years for 56% of them.

Clear-cell chondrosarcoma: a report of five cases including ultrastructural study.

Five cases of clear-cell variant of chondrosarcoma (Unni et al.) are reported. The tumors occurred in the epiphyseal region of long bones; three in the femoral head. Roentgenographically, the lesion was usually a well-defined and benign appearing one, either purely lytic (3 cases) or with central radiodensity (2 cases). Histologically, all five cases had areas of conventional chondrosarcoma; however, the greater portion of the tumor was made up of sheets of clear-cells intermixed with nonneoplastic bone trabeculae but devoid of chondroid matrix. Electron microscopic studies showed that these clear-cells possess cytoplasmic microvilli, abundant glycogen particles and prominent golgi complexes, like normal or tumorous chondroid cells usually have. In our experience, the best treatment seemed to be en bloc resection with joint replacement; indeed, despite the fact that they are true chondrosarcomas, these tumors usually have a very slow rate of growth.

[Bone metastasis simulating a chondrosarcoma (author's transl)].

The authors report three cases of bone tumours, two in the scapula and one in the pelvis, which simulated chondrosarcoma on radiological examination. After biopsy it was concluded that they were metastases secondary to carcinoma of the bladder in one case and of unknown origin in the other two.

[Pantalar arthrodesis. A follow up study of 45 operations (author's Transl)].

The authors have studied the long term results of 45 Pantalar arthrodesis realised either in one stage, or in two stages. Technical details are given for obtaining a good positioning during the surgical procedure. It was found that post operative complications were more frequent than after more limited operations. The long term results were satisfactory, provided a good positioning. Residual equinus deformities were better tolerated than deformities in pronation or supination. It was noted no consequences on neighboring joints.

[Phalangeal osteoid osteoma (13 cases)].

Thirteen cases of osteoid osteoma of the phalanxes are reported: 10 fingers and 3 toes. Any phalanx may be affected, with no preference for a particular site, in the young adult. Segmentary tumefaction of the soft parts near the lesion is a more consistent sign than pain, which is often quite minimal. Radiographies demonstrate the increase in the size of the phalanx, whose matrix is condensed. All radiographic techniques must be used to determine the exact location of the nidus, that is always very small (1 to 2 mm), and whose total removal is the only guarantee of cure.

[Resection-arthrodesis of the knee for bone tumours (author's transl)].

The authors describe the technique, devised by Juvara and modified by Merle d'Aubigné of resection-arthrodesis of the knee with fixation by medullary nailing and using massive bone grafts. They report 38 cases operated on for various conditions (20 giant-cell tumours, 7 chondro-sarcoma, 6 juxta-cortical sarcoma, 3 other types of sarcoma, one desmoid fibroma, one chondroma). The best results were obtained in cases in which surgical excision had been sufficiently wide and when very thick nails were used. The use of homografts increased the rate of infection. Resections of the lower and of the femur resulted in more complications than tibial resections. In femoral lesions, the part of the tibia that is transplanted should not be turned upside-down but should be mobilised upwards to provide better bone contact. The indications are discussed and compared with those of total replacement. The authors believe that, despite the number of complications, this technique is more practicable than total knee replacement.

[Clear-celled chondrosarcoma or chondroblastic sarcoma. A new type of chondrosarcoma (author's transl)].

The authors report five cases treated by wide resection. Pathological studies were made of the whole of the lesion. Three tumours were situated at the upper end of the femur, one at the upper end of the humerus and one at the level of the tibial plateau. The specific features were localisation in an epiphysis and histological areas of clear cells whose cytoplasm contained glycogen. From the anatomical and radiological points of view, these tumours were like chondroblastomata but their progress was more like that of chondrosarcomata. The author concludes that they should be distinguished from both types of tumour.

[Functional study of a double arthrodesis of the foot].

The authors have reviewed 190 feet treated by double arthrodesis for a variety of lesions including post-traumatic deformities, flat foot, paralytic lesions, club foot and other conditions. Follow-up was more than two years in 101 cases, more than five years in 73 cases and more than ten years in 30 cases. The importance of obtaining a good correction of both hindfoot and forefoot is emphasized. The long-term results of feet correctly aligned was good. The fate of the neighbouring joints and the functional adaptability of the foot have been studied. Details of surgical technique are given.
