The use of nurses' holding power in a large psychiatric hospital.

The use of nurses' holding power is a poorly explored aspect of psychiatric inpatient care. This study examined the use of Section 5(4) at the Maudsley Hospital, London, since the introduction of the 1983 Mental Health Act. Between 1983 and 1996 nurses' holding powers were used on 25 occasions. Seventy two per cent of patients detained under Section 5(4) of the Mental Health Act 1983 were subsequently detained under Section 5(2). Results were broadly consistent with previous studies, although some significant inconsistencies were observed which can probably be attributed to small sample sizes. Further, multicentre research is needed in order that substantive conclusions can be drawn.

Research and audit in effective health services.

This article explores the role of research and audit in the pursuit of effective health services, the differences between the audit and research function, the importance of a multidisciplinary approach to evaluating a service and the impact of an evidence-based approach on patients.

The Internet: an effective tool for nursing research with women.

This article outlines the methodology of using the Internet to survey an international population of women about their perceptions of breast health education and screening. Issues to consider in planning and implementing the research project by Internet are presented. A large population of women from North America and elsewhere was reached through the establishment of a website with linkages to other sites frequented by women. Women who visited the website were asked to complete a questionnaire. Anonymity was guaranteed and simple instructions were provided at the site. Benefits, limitations, and tips for success in using the Internet as a research tool are presented. These investigators found the Internet to be an appropriate medium for health-related research that also garnered national and international media interest. Hypertrophic cardiomyopathy: therapeutic approaches.

Patients with hypertrophic cardiomyopathy can have varied clinical pathophysiologic courses, and there is no optimal treatment modality. The mechanisms that explain clinical symptoms in a patient are not well understood. Treatment modalities include drugs, pacemakers, and operative approaches. Genetic analyses ultimately may help identify patients at high risk for complications who will require a particular treatment strategy.

Pacemakers for refractory heart failure: the road not taken?

Dual chamber pacemaker implantation has been investigated as a possible treatment modality for patients with advanced congestive heart failure (CHF) who do not respond to current pharmacologic approaches. By optimizing atrioventricular contraction with pacing, hemodynamics have been shown to improve in short-term studies. The long-term benefits of therapy have not been demonstrated conclusively, and clinical trials are now in progress to study pacemaker implantation with and without implantable cardioverter defibrillators as a possible therapeutic option.

Left ventricular reduction surgery.

When medical therapy is unsuccessful in improving the functional class in patients with heart failure, surgical options must be considered. Although transplantation may be considered the gold standard, numerous factors limit the number of patients who can benefit from it. Therefore, other surgical methods have been devised as alternatives to transplantation. Left ventricular reduction by surgical resection of a piece of myocardium was introduced in Brazil a few years ago, and enthusiasm for the procedure is increasing because encouraging results have been published by various groups. It is hoped that this procedure will form part of the treatment for congestive heart failure in the future.

Challenges for teachers of women's health.

Until recently, nursing literature had not addressed feminist issues and nurse educators have had little experience in programs of women's studies. The author discusses some implications for nurse educators as they relate to the new scholarship of women and a new consciousness of women's health. Factors contributing to the need for particular ways of teaching when dealing with women as students and with women's health experiences as subject matter are addressed. Using specific examples from her experience in teaching an open enrollment course in women's health, the author describes the use of some feminist pedagogical approaches and the students' response to them.

Spurious absence of signal on 3D time-of-flight MR angiograms on 1 and 3 tesla magnets in cerebral arteries associated with a giant ophthalmic segment aneurysm: the need for alternative techniques.

There are many high-resolution MR angiographic techniques available today for the evaluation of circle of Willis. We report a case of bilateral aneurysms of the carotico-ophthalmic segment, the left one being a giant aneurysm, in a 41-year-old woman. She underwent different MRA sequences on both 1 and 3Tesla magnets and a digital subtraction angiography. 3D time-of-flight angiograms can give spurious information on giant aneurysms and other techniques like phase contrast and dynamic contrast enhanced angiographies can supplement if not replace it. High field strength improves the objective quality of MR angiograms, but in the case of large aneurysms, intra aneurysmal and distal vessel signal loss can be a significant problem.

Promoting creativity in nursing education.

Shortly after the University of Iowa implemented a new, integrated curriculum in 1974, a study of creativity was initiated because one goal of the process curriculum is to foster creativity. Hypotheses of no difference between beginning and graduating students and between old and new curriculum students were tested by means of Student's t test with Activities 5, 6, and 7 of Verbal Form A, Torrance Tests of Creative Thinking used as the criterion measure. The null hypotheses were rejected in favor of the beginning students and the old curriculum students, respectively (p less than .05). Hypotheses of no relationship between creativity and American College Testing scores and admit grade point average were tested by computing Pearson product moment correlation coefficients. All values of r were nonsignificant. Further research on a number of aspects of creativity in nursing is suggested.
