Tentative protocol for the evaluation of coagulometers, based on one-stage prothrombin time.

Information on performance is invaluable to those who have to decide on the choice of an instrument. More interest is being shown in the use of coagulometers. Accordingly, a tentative protocol for the evaluation of coagulometers is presented for discussion. While it is based on the use of one-stage prothrombin time, principles of evaluation are laid down which should make it generally applicable. It is hoped that the tentative protocol will form the basis of a reference protocol and to this end comments are invited from users of coagulometers and suppliers. Comments for publication may be sent to the Editor of Clinical and Laboratory Haematology or to the Secretary of the Working Party.

In vitro studies on optimum preparation of coagulum for surgery of renal calculi.

In renal calculus surgery, human dried fibrinogen produces a coagulum with a much greater tensile strength, stickiness deformability and extractability than cryoprecipitate, which produces a relatively weak and friable clot. A useful coagulum may be prepared at room temperature at any convenient time prior to use if a mixture of 19 ml human dried fibrinogen solution and 1 ml thrombin solution is used. If mixed in a single syringe and injected within 35 s through a butterfly needle into the renal pelvis, complete mixing of the solutions is guaranteed. By using this simple method, the unpredictability of the coagulum can now be eliminated and a strong and elastic clot can be produced.

Regional co-ordinator for haemophilia in domiciliary practice.

The North-east Thames Region has appointed a nursing sister to co-ordinate the organisation of care for haemophiliacs in the region. As a result of the appointment, facilities for home treatment have expanded rapidly. Several associate centres providing care to haemophiliacs have been set up around the region in addition to the four main haemophilia centres, which are all in the south-west corner of the region. As well as providing support and supervision of patients on home treatment, the co-ordinator helps to place haemophiliac children in suitable schools, maintains the regional register of haemophiliacs, and has a more general role in ensuring that services are available where they are needed throughout the region.

Severe coagulation defect due to a dietary deficiency of vitamin K.

A patient with multiple injuries developed a severe coagulation defect due to a deficiency of vitamin K. None of the usual predisposing factors was present and the patient's diet was very poor. Dietary deficiency of vitamin K and the importance of antibiotic therapy in its production are discussed.

Role of plasma-exchange in the management of patients with factor VIII inhibitors.

Plasma-exchange is one of many treatments available for the prevention and management of hemorrhage in patients with factor VIII inhibitors. Details of exchange technique and the frequency of treatment are determined by the clinical situation, venous access and the availability of equipment, staff and blood products. In view of the many difficulties associated with plasma-exchange, other treatment options are likely to be preferred in most circumstances. The technique remains attractive as a means for reducing inhibitory activity before elective surgery and during life-threatening hemorrhage.
