Ventilation and acid-base recovery following exhausting activity in an air-breathing fish

The effects of exhausting activity in normoxic (PO2=20.7 kPa) and hypoxic (PO2<2.7 kPa) water on ventilatory, metabolic and acid-base variables were examined in spotted gar (Lepisosteus oculatus) to determine the role of the air-breathing organ in supporting active metabolism and recovery. The level of aquatic hypoxia used effectively eliminated the gills as a site of O2 uptake, forcing the fish to respire as a unimodal air-breather. Swimming duration (until exhaustion) was not significantly different in normoxic and hypoxic water. Blood gas, acid-base, cardiovascular and ventilatory variables were monitored at intervals from 15 min to 24 h post-exercise. Fish survived exhaustive exercise using a combination of anaerobic metabolism and increased ventilation (aerial and aquatic), despite respiratory and metabolic acidoses. The cardiovascular effects of exercise (heart rate and dorsal aortic blood pressure) were minor. The metabolic effects of exercise were similar to those in unimodal water-breathing fish; however, even hypoxic animals recovered from exhaustive exercise by 24 h. Thus, the results of this study show that air breathing in L. oculatus allows gar to exercise to the same extent in normoxic and hypoxic water and enables them to re-establish blood gas and acid-base balance after exhaustive activity even in hypoxic water.

Impressions for oculofacial prosthetics.

Primary to the development of an acceptable and functional facial prosthesis is the impression procedure. The fabrication of a facial prosthesis combines the art and science of anatomy, cosmesis, and function, to develop a nonliving substitute to replace altered, missing, or defective regions of the head and neck area. Success depends on patient's cooperation, motivation, and commitment to treatment as well as the technical and artistic scope of the facial prosthetic service.

Impression making, sculpting, and coloring of orbital prostheses.

The replacement of any anatomic structure by artificial means remains a challenge. This is particularly true in the facial area. The replacement must be one which blends with the adjacent tissues as well as replaces the missing structures. Careful planning and meticulous attention to detail in fabrication of a prosthesis can enable the maxillofacial prosthodontist to make a major contribution in the rehabilitation of the patient with an orbital defect.

Flasking technique for large facial prostheses.

By utilizing the giant press and the plastic pipe flask, large facial prostheses can be flasked and processed to allow thin, well-adapted margins without distortion. The plastic flask is easily stored for a remake of the prosthesis at a future date if it should be necessary.

Clinical evaluation of oral Candida in cancer chemotherapy patients.

No overt clinical oral candidiasis was noted upon either direct oral examination or review of medical records. No significant change in percent of Candida species as compared to other populations is seen in this study. Varying percents of individual species do differ from other reports. Alimentary and digestive tract tumors represented over 56% of the primary malignancies. There was a significant correlation between decrease in salivary pH and increase in percent of Candida species growth. Most significantly, however, the preliminary data in this study might eventually contribute to continuing investigation of Candida species in human cancer patients.

Altered growth and development of lower teeth in children receiving mantle therapy.

The improvement in survival and local control rates in children with neoplasms in the head and neck region can be expected to demonstrate increased iatrogenic adverse effects of treatment. The authors report on mandibular growth and developmental abnormalities in 3 children with Hodgkin disease treated by external beam megavoltage therapy. Radiations to the mandible during mantle field therapy were measured and found to be significant. Possible methods of dose reduction are discussed.

Occlusally oriented impressions for large maxillary stomas.

A stable record base is critical to the making of an accurate centric jaw relation record. This is attained by meeting three criteria: (1) a well adapted, adequately extended record base, (2) adequate support of the alveolar bone to prevent excessive movement of the record bases, and (3) accurate tissue integrity to allow good base adaption, maximum tissue contact, and minimum movement. If these criteria cannot be met--and they cannot in the bilateral subtotal maxillectomy patient--then an alternative method must be used. The combination jaw relation record-impression technique using the patient's pre-existing occlusion is an alternate method for the fabrication of a stable prosthesis. This method has been tested in several maxillofacial prosthodontic centers and has proved to constitute a realistic approach to the management of the subtotal maxillectomy patient.
