Evaluation of polymerase chain reaction, tuberculostearic acid analysis, and direct microscopy for the detection of Mycobacterium tuberculosis in sputum.

Tuberculosis remains a major global cause of morbidity and mortality. There is an urgent need for improved bacteriologic diagnosis of Mycobacterium tuberculosis infection. Three methods for rapid identification of M. tuberculosis in sputum samples (direct microscopy, gas chromatography-mass spectrometry [GC-MS], and polymerase chain reaction [PCR]), were compared with culture on Lowenstein-Jensen medium. Growth of M. tuberculosis was observed in 38 of 145 sputum samples. Detection of acid-fast bacilli by direct microscopy gave a sensitivity of 66% and a specificity of 100%. Detection of tuberculostearic acid by GC-MS gave a sensitivity of 55% and a specificity of 87%. Amplification by PCR of a fragment of the insertion sequence IS6110 gave a sensitivity of 95% and a specificity of 93% compared with culture and a corrected specificity of 99% compared with both culture and clinical data. This study indicates that PCR can be adapted for clinical use and is the method of choice for rapid diagnosis of pulmonary tuberculosis.

[Characteristics of sexual offenders in our data].

By analyzing two groups of sexual delinquents: those which have committed rape, attempted rape or committed rape with murder (the first group) and the commiters of indecent acts (the second group), it was established that the named groups are substantially different in relation to some of the studied parameters. In the first group of delinquents, there is considerably less of those who are married (35.48%), in relation to the second group (73.68%). Indecent acts are most often committed in the apartment of the sexual offender (63.16%), while this is the case in 22.58% of the offences from the first group. The victims of indecent acts were only minors. In both groups the most frequent psychiatric diagnosis was psychopathy. Alcoholic state at the time of committing the offence was considerably greater in the first group (87.10%), in relation to the other one (47.37%). There were no important differences relating earlier criminal offences, as well as the psychiatric estimates of accountability between the groups.

[Disease of metalgrinding workers (author's transl)].

It is a specific clinical picture with extensive soft tissue gas and swelling of the forearm. Usually, the clinical course is without any other symptoms. Therapy is not necessary because gas disappears in few days. The etiology of the disease is not clear. Gas can be generated by the chemical reaction between a metallic foreign body and tissue fluid. Spread of benzine evaporation has been considered as a possible source as well. The disease is very rare and its knowledge is important in differential diagnosis of gas gangrene.

[Ulcerative tracheo-esophageal fistula--a complication of longterm respiratory support (author's transl)].

A case of ulcerative tracheoesophageal fistula following long term respiratory support is described. After establishing the diagnosis of TEF, an attempt at surgical repair should be made as quickly as patient's condition permits it, ideally immediately. We suggest right side thoracotomy with the resection of the fourth rib, thereby facilitating preparation of a pedicled flap which should be interponated between trachea and esophagus after the closure of the esophagus defect by sutures has been performed.

[Surgical interventions in pacemaker patients (author's transl)].

In this paper 44 operations in patients with cardiac pacemakers have been reported. Electrosurgical instruments were used in almost all cases. It was noted, that neither a disturbance of the pacemaker nor of the heart stimulated by it occured during or after operation. A premature exhaustion of the battery was not observed.

[Homotransplantation of the spleen].

A report of experiences and results on 63 homotransplantations of spleen in dogs is given; 50 of the transplantations were performed in an orthotopic and 13 in a heterotopic way. 10 of the latter had undergone an earlier splenectomy and 3 an earlier orthotopic transplantation, followed by a complete resorption of the first transplant. The course of the transplant was observed by scintigraphy, the function of the anastomosis was controlled by angiography. The immunological and hematological changes were analyzed together with the changes of the protein-electrophoresis and those of the fibrinogenic factors. Scintigraphy has proved to be very useful for the diagnosis of transplant rejection; the measurement of the activity of the lysozyme in the serum compared with the pathoanatomical findings showed a characteristic increase of the activity of the lysozyme during the rejection crisis.

[Diagnosis and therapy of pulmonary sequestration (author's transl)].

Pulmonary sequestration is a rare malformation which is characterised by lacking connection to the bronchial system and by atypical arterial blood supply. Unidentifiable tumorous, cystic or infiltrative lung diseases, especially of the lower lobes, should always lead to aortography to exclude pulmonary sequestration.

[Biochemical characteristics of concentrated and rinsed erythrocytes].

The biochemical characteristics of concentrated and rinsed out erythrocytes in time function have been analysed. It has been concluded that five days after conservation and during the same day of preparing there was no evidence of significant differences between complete blood erythrocytes and concentrated and rinsed out erythrocytes. In course of preparing concentrated erythrocytes the number of leucocytes declined rapidly, but not completely. By triple rinsing of erythrocytes the number of leucocytes is somewhat less than in the samples of concentrated erythrocytes, but some leucocytes are left.
