Impaired antibody-dependent cell-mediated cytotoxic activity in patients with the acquired immunodeficiency syndrome.

Since the acquired immunodeficiency syndrome (AIDS) is characterized by opportunistic infections and malignancies indicative of a profound suppression in cell-mediated immunity, we investigated the antibody-dependent cell-mediated cytotoxicity (ADCC) of peripheral blood mononuclear cells of patients with AIDS against chicken red blood cells (CRBC). A marked decrease in ADCC-CRBC activity was observed from patients with AIDS as compared to healthy controls. Furthermore, suppression in ADCC activity was seen when mononuclear cells from healthy subjects were assayed using media containing 25% or 40% sera from AIDS patients. Two of two patients with AIDS and impaired ADCC-CRBC activity were also found to have in vivo impaired reticuloendothelial system Fc-specific clearance of 51Cr-labelled, anti-Rho (D) IgG-sensitized autologous erythrocytes. These data provide further evidence of monocyte-macrophage dysfunction in AIDS and help explain the widespread occurrence of opportunistic pathogens in AIDS.

Phenotypic expression of natural killer cell associated membrane antigens and cytolytic function of peripheral blood cells from different aged humans.

The function and phenotypes of peripheral blood NK cells from different aged adults were examined using a 4 hr 51Cr release NK assay and laser flow cytometry utilizing the Leu-7 and Leu-11 monoclonal antibodies. It was found that there was no difference between aged and young adults in the expression of Leu-7+, Leu-11+, Leu-7-11+ or Leu-7+11+ cells. A significant decrease, however, was seen in both percent representation and absolute number of Leu-7+11- cells in older subjects. When examined by age and gender, women aged 17-59 yr had a significantly lower percent representation and absolute number of Leu-11+, Leu-7-11+ and Leu-7+11+ cells and baseline NK activity than did men of similar age. No differences were seen between men and women aged 60-84 yr. No differences were seen in the degree of augmentation by interleukin-2 of NK function in any group. Our data indicate that NK function is correlated with cells bearing the Leu-11 surface antigen and that the gender of the cell donor is a significant factor in the representation of NK cells and function in persons under 60 yr of age. Studies on age differences in NK number and function must therefore take into account the percent of women in the populations studied.

Disseminated Curvularia infection. Additional therapeutic and clinical considerations with evidence of medical cure.

A previously reported case of cerebral infection due to Curvularia lunata is more fully described. Medical cure was apparently achieved after 30 months' treatment with amphotericin B. Success was achieved only when the drug was given in a dose of 40 mg, three times per week, and was continued for six months after enhanced computed tomographic scans no longer showed cerebral lesions. Immunologic studies suggested the infection was accompanied by an unexplained defect in cell-mediated immunity.

Hematologic abnormalities in the acquired immune deficiency syndrome.

Blood and marrow specimens were evaluated from 12 patients with the acquired immune deficiency syndrome (AIDS). Ten patients were anemic, eight leukopenic, and three thrombocytopenic. Pancytopenia was present in two patients and subsequently developed in two others. Reticulocyte counts were not increased in the anemic patients. The most common peripheral blood abnormalities were a left shift in the granulocyte series, lymphopenia, atypical lymphocytes, and vacuolated monocytes. Marrow cellularity was increased in five patients and reduced in three. Marrow reticulin was increased in 10 patients; in three of these, marrow could not be obtained by aspiration. Plasma cells were increased in number in every marrow aspirate, and there was a left shift in the myeloid series in most. Aggregates of atypical lymphocytes or a diffuse increase in marrow lymphocytes occurred in seven patients. An increase in histiocytes was observed in seven marrow aspirates; in five of these, the histiocytes were phagocytizing red cells, white cells, and platelets. Necrosis was present in four marrow specimens. These hematologic abnormalities reflect, in part, the presence of systemic infection, inflammation, and the inanition associated with them. However, the high incidence of myelofibrosis, alterations in marrow cellularity, pancytopenia, and hematophagic histiocytosis indicates that the bone marrow is a target organ in AIDS.

Analysis of immune status of hemodialyzed adults: association with prior transfusions.

Peripheral blood leukocytes of 29 hemodialyzed adults, 19 transfused and 10 nontransfused, were studied using immunofluorescent staining with monoclonal antibodies and in vitro measurement of natural killer (NK) cell activity. When compared with control subjects, the absolute number of leukocytes in transfused hemodialyzed patients was significantly reduced (P less than 0.01), as were the absolute numbers of OKT11+ cells (P less than 0.01), and OKT4+ cells (P less than 0.0001). The percent representation of OKT11+ and OKT4+ cells was also significantly lower among transfused hemodialyzed patients (P less than 0.01 and 0.001, respectively), and this loss of OKT4+ cells resulted in a decrease in the ratio of OKT4+/OKT8+ cells (P less than 0.01). The absolute number of Leu-7+ cells was also decreased in the transfused group (P less than 0.05). A decrease in in vitro NK cell activity was present in both transfused and nontransfused hemodialyzed subjects. Whether these differences in peripheral blood lymphocytes were induced by the erythrocyte transfusions could not be determined; however, if they reflect changes in central lymphoid tissues, then these results may help explain the prolonged survival of renal allografts in transfused individuals.

Systemic anaphylaxis caused by parenteral spectinomycin.

Spectinomycin is commonly used for the treatment of uncomplicated gonorrhea, and is considered to be a safe drug. We have described a case of anaphylaxis characterized by peripheral vascular collapse after intramuscular administration of spectinomycin.

Role of the mononuclear phagocyte system in the immunopathogenesis of human immunodeficiency virus infection and the acquired immunodeficiency syndrome.

In studies aimed at defining monocyte and macrophage function in patients with human immunodeficiency virus (HIV) infection, we found impaired in vivo Fc receptor-specific clearance in 20 of 25 patients with acquired immunodeficiency syndrome (AIDS) and in five of 13 patients with AIDS-related illnesses. The in vivo function of macrophage C3 receptors was also found to be abnormal: AIDS patients had a relatively large release of cell back into the circulation, suggesting failure of macrophage phagocytosis. The antibody-dependent cell-mediated cytotoxicity of circulating mononuclear cells was significantly lower in AIDS patients than in healthy controls. Monocyte nonspecific phagocytosis and surface marker expression were intact. Defective monocyte and macrophage function is an integral part of the immunopathology of AIDS, leading to the failure to control opportunistic pathogens. Whether these defects are due to intrinsic infection of the mononuclear phagocytes with HIV or are secondary to other events in the network of HIV infection remains to be determined.
