[The optic and trigemino-facial reflexes in the diagnosis of multiple sclerosis].

The optic facial reflex (ofr) has been studied in a group of subjects affected by multiple sclerosis (ms) and compared with the trigemino-facial reflex (tfr) as well as clinical data. From the literature it appears that the afferent pathway of the ofr, after the optic chiasma crosses the midbrain tegmentum and from there goes directly to the pons. The ofr was impaired in 14 subjects out of 20 (70%), the tfr only in 7 (35%). This difference is attributed to the fact that the visual pathways are frequently affected by demyelinating processes. The ofr in contrast with the tfr, because of anatomophysiological differences, does not supply the exact localization of the damage. In our series of patients affected by ms the data from ofr reflected exactly the signs of optic nerve lesions. When these were not found, ofr abnormality was attributed to lesions located behind the chiasma or in the midbrain. A close correspondence between data supplied from the ofr and the diagnosis of ms according to Mc Alpine is emphasized: this makes the ofr a very useful diagnostic test of ms. The combined use of ofr and tfr, in addition to fulfilling the criteria of detecting multiple sites of demyelination, can be useful to investigate the brain stem.

[A case of xanthogranulomatous pyelonephritis. Diagnostic and therapeutic problems].

The use of the modern diagnostic imaging techniques allows in most cases a diagnosis of nature of the lesions involving parenchymal organs. The authors report a case of renal xantogranuloma, rare pseudotumoral inflammatory pathology, treated by radical extended nephrectomy on the basis of a pre-operative diagnosis of malignant tumor. Despite the risk of false positives, in front of a renal mass whose nature is sufficiently characterized by the imaging techniques, the routine use of more invasive investigations such as arteriography and percutaneous needle biopsy seems not justified.

Causes of the instability of R3 component of electrically evoked blink reflex: role of the attention to the stimulus.

The causes of the spontaneous sharp amplitude variations of R3 component of electrically evoked blink reflex have been analyzed, in 10 subjects 24-30 years, in different experimental conditions: a) neutral condition, b) selective attention to the stimulus; distraction from the stimulus: c) by looking at a poster rich with images and d) by signaling from sequences of numbers three targets. In the situations where the subject does not play attention to the experimental set up, (a), or is distracted from it (c), the R3 response shows the maximal amplitude, whereas in condition of greater attention to the stimulus (b), R3 appears sharply reduced in amplitude. Where attentive levels are high but not focused on the stimulus (d), the reduction of the third reflex response is lesser. The reciprocal modifications with respect to the other responses R1 and R2 are also discussed.

CD3-stimulated Jurkat T cells mediate IL-1 beta production in monocytic THP-1 cells. Role of LFA-1 molecule and participation of CD69 T cell antigen.

In this study we investigated the T cell signals required for monocyte activation. We used an in vitro co-culture system involving two human cell lines: Jurkat T cells and THP-1 monocytes. Monocyte activation was monitored by measuring IL-1 beta production, whereas IL-2 secretion reflected Jurkat activation. We showed that CD-3 -stimulated Jurkat cells delivered an IL-1-inductive signal to THP-1 cells through a cellular contact which was independent of THP-1 Fc receptors cross-linking. Stimulation of IL-1 beta production did not appear to require lymphokine secretion by T cell since a lymphokine defective mutant of Jurkat cell was able to deliver the stimulatory signal. The LFA-1 molecule was clearly shown to participate in the cooperation process, but its role was likely to be restricted to mediating initial adhesive interaction rather than to transducing the IL-1 -inductive signal. Interestingly, the co-culture stimulated by (Fab')2 fragments of CD3 mAb displayed an enhanced IL-1 beta production without any increase of IL-2 secretion. This result indicated that Jurkat cells could stimulate THP-1 cells even when they were only partially activated. The kinetics and conditions of IL-1 beta production called our attention to the early T cell activation antigen CD69. We then showed that CD69 mAb interfered with transmission of the IL-1 inductive signal (40-50% inhibition of IL-1 production). Our results are suggestive of a new role for CD69 molecule intervening in the T lymphocyte-dependent monocyte activation process.
