Effect of a lipid load on blood and urinary carnitine in man.

Blood and urine carnitine contents have been determined in patients before and after a lipid load and in patients on haemodialysis. Oral and intravenous lipid administration significantly depressed blood carnitine content and after 500 ml intravenous Intralipid urinary carnitine excretion fell by 43%. Blood carnitine was reduced by 50% by haemodialysis and returned to the pre-dialysis value within 20 h in 5 out of 8 patients. It is concluded that the blood carnitine level is normally controlled over a narrow range. The fall in blood carnitine concentration and urine excretion which follows a lipid load indicate a limiting role for carnitine in lipid utilization in man, and suggest that carnitine supplements could be of value during parenteral nutrition with fats.

Inhibition of active-transport sodium-potassium-A.T.P.ase by myeloma protein.

Patients with multiple myeloma excrete immunoglobulin light chain (Bence-Jones protein) in the urine. Light chain accumulates in the nephron, and this may result in renal disease, which accounts for the death of between 1 in 4 and 1 in 7 patients with multiple myeloma. Myeloma light chain inhibits transport of paraaminohippurate in the kidney. Evidence is presented here that light chain affects renal transport by inhibiting the active-transport enzyme, ouabain-sensitive Na-K-A.T.P.ase. As a direct consequence, renal A.T.P. content and the maximum rate of gluconeogenesis from lactate are enhanced in the presence of light chain.

A modified Rutter children's behaviour questionnaire. Its reliability and validity in screening for behaviour disturbances in part-Aboriginal children.

The Rutter Children's Behaviour Questionnaire was completed by the teachers of 108 Aboriginal pupils at two rural schools in the far west of New South Wales. The questionnaire ratings were compared with a contemporaneous psychiatric study of behaviour disturbance for each child. Some modification of the questionnaire was required because of cultural factors. The modified questionnaire was found to be of acceptable reliability and validity in screening for emotional disturbance in Aboriginal children. It is suggested that it would be a useful case finding instrument for teachers of part-Aboriginal children.

Evaluation of renal preservation using the isolated perfused rat kidney. Ischaemic damage and the effects of inosine.

An isolated perfused rat kidney has been used to determine the response to prolonged warm ischaemia and to examine renal preservation procedures. All measurements of function declined to near zero after 90 min of warm ischaemia. Measurement of total sodium reabsorption (TNa) was the most sensitive indicator of renal damage. Inosine was without effect on subsequent renal function after 60 or 90 min of warm ischaemia, or after 24 hr of cold ischaemia. Surface cooling or hypertonic citrate solution, in the cold or at 37 degrees C, significantly improved renal function, and their use in clinical renal preservation rather than that of inosine was supported by the present results.

Giant sigmoid diverticulum: clinical and radiological features.

Two case reports of giant sigmoid diverticulum associated with diverticular disease of the sigmoid colon are presented. The clinical and radiological features of 30 similar cases found in the literature are reviewed. Our two cases represent the largest recorded diverticulum and the oldest recorded patient with this condition.
