Skin lesions--an indicator of disease activity in systemic lupus erythematosus?

Cutaneous manifestations have great diagnostic value for systemic lupus erythematosus (SLE). In this study we tried to establish a correlation between lupus erythematosus LE-specific and LE-nonspecific cutaneous lesions and disease activity measured by the Systemic Lupus Erythematosus Disease Activity Index (SLEDAI). Sixty-six patients with SLE were evaluated. They were divided into three groups having: (1) only LE-specific lesions (38 or 58.46%); (2) only LE-nonspecific lesions (4 or 6.15%); and (3) both types of lesions (23 or 35.38%). Results were analyzed using the Student t-test. Patients with LE-nonspecific skin manifestations had significantly increased disease activity compared to those with only LE-specific lesions. The number of different skin lesion types also correlated with disease activity. It was significantly increased in a group with three different types of lesion, either specific or nonspecific. Patients with only one type of lesion had mild disease. An intermediate disease activity was found in the group with two different lesion types. Lupus-specific skin manifestations serve primarily as an important diagnostic clue. In conclusion, patients with LE-nonspecific lesions have significantly more active SLE than those with LE-specific lesions and may therefore require more intensive therapy and disease monitoring.

Hyperthermic stress modulates the functions of rat liver glucocorticoid receptor.

A mild whole body hyperthermic stress causes a rapid and reversible reduction of rat liver glucocorticoid receptor (GR) binding capacity and affects the stability of the GR-DNA complexes formed after thermal transformation of the receptor. These changes appear to be physiologically relevant, since they are accompanied by a decrease in dexamethasone induction of hepatic tyrosine aminotransferase (TAT). In spite of the decreased rate of the GR degradation in liver cytosol of hyperthermic as compared to control rats, the total amount of the GR and its proteolytic products recognized by BuGR2 monoclonal antibody was found to be lower in the former cytosol, but higher in the respective nuclei.

[Salivation in the submandibular salivary gland in the prognosis of peripheral facial nerve paralysis. Clinical study].

The relationship between the quantitative submandibular gland secretion and neural motor function was examined in 157 patients with different types of peripheral facial paralysis. These two functions were followed up during the period of one year. In the patients with 40%, or more, of submandibular gland secretion one month after the onset of facial paralysis, the motor nerve function recovery was satisfactory, while, in the patients with submandibular gland secretion less than 20% the recovery was unsatisfying. When the submandibular gland secretion was more than 50% in the early stage of facial paralysis, the complete recovery was within 6 months. On the contrary, when it was less than 50%, the recovery period was much longer.

In vivo effects of cadmium on rat liver glucocorticoid receptor functional properties.

1. Cadmium (Cd2+) administered in vivo induced a 40% reduction of rat liver glucocorticoid receptor (GR) capacity and inhibition of glucocorticoid-receptor complexes binding to mouse mammary tumor virus (MMTV) DNA fragment containing GR consensus sequence. 2. The effect of Cd2+ on the GR binding activity can be reversed with DTT, suggesting Cd2+ interaction with thiol groups. 3. Cd(2+)-related GR modification seems to be mediated by Cd2+ binding to cytoplasmic components included in the regulation of the receptor function, although the direct binding of the metal to the receptor thiols could not be ruled out.

[Sudden sensorineural hearing loss of unknown origin].

The study presents the results of a ten year experience in the treatment of abrupt hearing loss of unknown etiology in 248 patients. More successful treatment was achieved in patients with lesser degree of perceptive hearing reduction and those showing no vestibular symptomatology. A decisive factor for a favourable hearing recovery is time between the onset of the disease and the beginning of the therapy. Having this in mind, the authors insist on early diagnosis and early administration of drugs affecting the inner ear perfusion.

[Personal experience and results in the treatment of hearing loss with myringoplasty].

Within the period of eight years (1973-1981) at the Clinic for ORL Diseases of the Military Medical Academy 150 myringoplasties were performed. The authors have presented the method, choice of grafts for reconstruction, functional success and immediate complications. Improved hearing was achieved in 89,34% of patients while hearing impairment was found in 10,66% of patients. The authors consider that myringoplasty, together with good microsurgical technique choice of grafts and correct indications, presents one of most successful otological operations.
