Composite skin contact smears in multibacillary leprosy patients.

"Composite skin contact smears" technique was used in 20 multibacillary leprosy patients to find out the presence of AFB on their intact skin. One hundred and twenty one AFB were found in 930 sq.cm. area of skin surface examined. Seventy-five AFB were solidly staining and 46 were granular. The bacilli were found in all the sites examined. Immunofluorescent staining showed M. leprae in one out of four patients. The number of bacilli discharged from the intact skin seems adequate for the transmission of leprosy considering the large surface area of the skin and evidence in mouse experiments that multiplication of M. leprae can be obtained after inoculation of very small number of bacilli.

Epidemiology of leprosy in rural population of Pondicherry.

A village having population of 2000 in 350 families and coming under the Villianur Commune of the Union Territory of Pondicherry was subjected to a total population survey. The population examined was 92.2%. A total of 138 cases of leprosy were found in 94 families making a prevalence rate of 6.9%. There were single cases in 61 families and two to four cases in 33 families. Child rate for leprosy was 37%. The male to female ratio showed no sex predilection. Maximum percentage (30.4%) of cases belonged to maculoanaesthetic type, followed by tuberculoid (27.5%), indeterminate (19.6%), borderline (14.5%), polyneuritic (6.5%) and lepromatous (1.5%) types. Nerve involvement was more frequent in multiple skin lesion cases than in single lesion cases.

Quantitative DNCB epicutaneous sensitization in leprosy patients and controls.

Status of non-specific cell mediated immunity in 49 leprosy patients classified according to Ridley & Jopling scale and 16 non leprous controls was studied using epicutaneous sensitization with DNCB and quantitatively grading the degree of sensitization with DNCB and quantitatively grading the degree of sensitization and Mx. test with 1 TU PPD. The effect of dapsone administration on CMI responses was also observed. There was no gross depression of CMI responses as made out by epicutaneous sensitization to DNCB but quantitative grading of responses revealed a subtle depression of CMI responses progressively increasing from II to LL end of spectrum. Mx. testing with 1 Tu PPD did not appear to be a good parameter to study the CMI status. Dapsone administration did not alter the CMI responses.

Knowledge about leprosy among leprosy patients.

A study on "Knowledge of Leprosy in Leprosy patients" was conducted in the Dermatology outpatient department of JIPMER. The study has shown that a good percentage of leprosy patients possess proper knowledge of leprosy in respect of its awareness, nature of treatment, consequences of lack of treatment and prevention of disease. Knowledge regarding the cause and mode of spread of disease and duration of treatment was lacking in majority of patients These areas should be given more concentration in educating the leprosy patients. It was found that literate patients possessed better knowledge than illiterate patients.

Gastro-intestinal side effects of clofazimine.

Three out of 30 patients of leprosy getting clofazimine therapy developed severe gastrointestinal side effects following the therapy in the dose of 100 mg after one month, eight months and five weeks respectively. One of the patients died due to the side effects. Indian patients appear to be more prone to severe gastrointestinal side effects of clofazimine and may develop the same after shorter duration of therapy with smaller doses of the drug.

Further evidence of the efficacy of co-trimoxazole in granuloma venereum.

One hundred and sixteen patients with granuloma venereum were treated with cotrimoxazole two tablets twice daily. All the patients responded; most of them required treatment for 10-15 days. Only two patients had recurrences, which were probably due to reinfections. No side effects of the drug occurred. Co-trimoxazole is considered to be the treatment of choice for granuloma venereum because of its high efficacy, few side effects, easy administration, and absence of any risk of masking concomitant syphilis.

Sulphone induced exfoliative dermatitis and hepatitis.

Exfoliative dermatitis associated with hepatitis has been reported in leprosy patients taking dapsone in daily dosage of 200 mg daily or more previously. This report concerns a 40 year-old-female who developed exfoliative dermatitis and hepatitis six weeks after starting dapsone in daily dosage of 100 mg for treatment of leprosy. She responded dramatically to administration of corticosteroids.

Efficacy of co-trimoxazole in Donovanosis. A preliminary report.

Co-trimoxazole (Septrin, Wellcome) was prescribed in a dose of two tablets twice daily for the treatment of 10 patients with Donovanosis. All the patients responded well to this treatment, and the ulcers healed completely within 10 days in eight patients and within 14 days in the remaining two. Ten days' treatment with 40 tablets of co-trimoxazole is suggested as sufficient to treat Donovanosis. No adverse reactions were noted in any patient.

Post-kala-azar dermal leishmaniasis simulating leprosy. (A case report).

The case of a 40 years male suffering from post-kala-azar dermal leishmaniasis simulating nodular lepromatous leprosy is reported. In countries where leprosy is endemic, other diseases are not infrequently taken to be leprosy (Schaller, 1971). Dermal leishmaniasis is a common cause of confusion in countries where the condition is endemic (Browne, 1964). Dharmendra and Chatterji (1940) discussed in detail the question of differential diagnosis between Leprosy and Dermal Leishmaniasis. The present communication is concerned with a patient suffering from post-kala-azar dermal leishmaniasis whose skin lesions simulated nodular lepromatous leprosy.

Donovanosis (granuloma inguinale) of the oral cavity.

A female patient suffering from donovanosis of the oral cavity without associated lesions elsewhere is reported. The importance of remembering donovanosis in the differential diagnosis of chronic granulomatous ulceration of the mouth is emphasized.
