[A new UV-B irradiation unit. Experiences with psoriasis vulgaris with low dosage UV-B irradiation and local cignoline use].

We present technical and clinical experiences with a modified scheme of Ingram treatment using a new UV-B phototherapy unit. In addition to application of anthralin, eleven patients suffering from psoriasis vulgaris have been treated by means of this cabinet equipped with 40 UV-B lamps (Sylvania STF 756085 W UV 6). In a bilateral comparative study, we investigated the therapeutic results of standard increase of the UV-B doses and constant low UV-B dose (0.150 J/cm2), respectively. We discuss our results and therapeutic consequences of this therapy.

[Side-effects of drugs on the male sexual function].

There are many drugs that may influence libido, erection, or ejaculation. In most cases, the sexual function is impaired, whereas enhancement is rarely seen. Such effects most likely occur in the course of treatment with drugs: interfering with the regulation of sexual hormones; exerting effects on central nervous mechanisms; influencing the autonomic nervous system; or causing changes of the peripheral blood flow.

[Side effects of drugs on male fertility].

Side effects of drugs on male fertility occur more often than they are expected to. In general, they are not recognized till during andrological examination on account of a barren marriage. A transient or permanent inhibition of male fertility by drugs is possible, if one of the following functions interferes: spermatogenesis, sperm maturation within the epididymis, sperm transport, sperm metabolism and motility, semen liquefaction, capacitation, acrosomal reaction, or ovum penetration. Best known is the impairment of spermatogenesis by direct inhibition of the germinal epithelium (antispermatogenic agents: cytostatics, immunosuppressive agents, psychopharmacological compounds, antibiotics) or indirectly by influencing the axis between hypothalamus, pituitary gland, and gonads (hormones: estrogens, gestagens, androgens, anabolics, antiandrogens). Sperm transport may be impaired by neuroleptic drugs, tricyclic antidepressants, antihypertensive drugs like guanethidine, and ganglionic blocking agents. Inhibition of sperm motility is possible by antibiotics, psychopharmacological agents, anthelmintics, and beta receptor blockers. In future, it will therefore be necessary to pay more attention to side effects of drugs on male fertility, particularly with regard to patients at the age of reproductiveness.

Polymorphonuclear leukocyte 5-lipoxygenase activity in psoriasis.

5-lipoxygenase-derived products of arachidonic acid are implicated in the pathophysiology of psoriasis, a common hyperproliferative and inflammatory skin disease. We therefore examined whether there is an activation of this enzymatic pathway in extracutaneous tissues. For this purpose, we measured the conversion of 14C-arachidonic acid by polymorphonuclear leukocytes from psoriatic patients and controls. No significant difference in the generation of leukotriene B4 and 5-hydroxyeicosatetraenoic acid by polymorphonuclear leukocytes was noted between the two groups. We conclude that in psoriasis there is no enhanced activity of the 5-lipoxygenase pathway in circulating polymorphonuclear leukocytes.

[Skin testing with the components of analgesics in patients with anaphylactoid hypersensitivity reactions to mild analgesics].

In 282 patients presenting with adverse reactions to mild analgesics, prick tests were performed with components of analgesic drugs; in some of them, commercial preparations were also tested. In 19 patients (7%), a total of 40 conclusively positive immediate reactions was found: there were 22 reactions to pyrazolone derivatives, 14 reactions to commercial preparations, and one singular reaction to phenacetin, phenobarbital, carbromal and vitamin B1, respectively. Cross-sensitivity to different pyrazolone derivatives was observed in only 5 of 15 patients with a positive reaction to at least one of these substances. One of the patients with a positive immediate reaction and 2 further individuals developed positive test reactions after 4 to 24 h. Within 117 patients who gave a clear-cut history of anaphylactoid reactions to mild analgesics, there were conclusive immediate prick test results in 15 cases (13%). In these patients, the diagnostic relevance of the prick test increased with the severity of symptoms in the history, and a conclusive immediate reaction was obtained in 25% of those with full shock in the history.

Cutaneous B-cell lymphoma.

The various morphologic and functional subtypes of nodal B-cell lymphomas can also be found in the skin. These reflect the various steps of lymphocyte differentiation including maturation from the pre-B lymphocyte to the well-differentiated B2 lymphocyte or plasma cell in the peripheral blood. The subtypes of cutaneous B-cell lymphomas have been discussed (Kiel classification); the percentages indicate the frequencies of the subtypes among a total of 736 cutaneous lymphomas of both T-cell and B-cell origin: Lymphocytic lymphoma (7 per cent). Immunoglobulin-producing lymphomas, including the rate plasmacytoma of the skin, lymphoplasmacytoid immunocytoma, which represents the largest group of cutaneous B-cell lymphoma (12 per cent), and immunoblastic lymphoma, which is the most aggressive form in this group (8 per cent). Cutaneous B-cell lymphoma arising from or related to follicular center cells, including centrocytic lymphoma (7 per cent), mantle-cell lymphoma, centroblastic/centrocytic lymphoma (6 per cent), the highly malignant centroblastic lymphoma (4 per cent), and lymphoblastic lymphoma, Burkitt type. The Ann Arbor staging system is not applicable to cutaneous B-cell lymphoma; therefore, a TNM staging system has been proposed. The diagnosis of cutaneous B-cell lymphoma is based primarily on cytomorphologic features. Differentiation of cutaneous B-cell lymphoma from pseudolymphoma of the skin cannot be based on a single criterion; a spectrum of characteristic features must be evaluated. Analysis of the infiltrating cells in cutaneous B-cell lymphoma using monoclonal antibodies demonstrates that the proliferation of the neoplastic clone is accompanied by a mixture of accessory cells of various origins, including T cells, macrophages, and dendritic reticulum cells. As in nodal B-cell lymphomas, several factors may be involved in the generation of cutaneous B-cell lymphoma, including persistent antigenic stimulation and loss of regulatory mechanisms for lymphocyte proliferation and differentiation in conjunction with environmental and other factors.

[UV-ray sensitivity of patients with malignant melanoma].

In 77 patients with malignant melanoma and 74 control subjects without skin tumors caused by sunlight, the minimal erythema dosage (MED) in the UV-C and UV-B areas, immediate pigment darkening (IPD), and minimal tanning dose (MTD) were determined according to skin type. Direct (MED-UV-B, MED-UV-C) and inverse (IPD, MTD) correlations were established according to skin type. These parameters showed no differences between patients with malignant melanoma and the control group.

Some statistical data, diagnosis, and staging of cutaneous B-cell lymphomas.

Cutaneous B-cell lymphomas represent a considerable number of skin lymphomas (about one-third) primarily showing skin manifestation without any extranodal involvement detectable using routine and special staging procedures. Differentiation from cutaneous T-cell lymphomas can be achieved clinically; histologically, due to the distinct growth pattern in the skin; and by immuno- or enzymophenotyping of the proliferating cells. Further differentiation of subtypes of cutaneous B-cell lymphomas is based primarily on cytomorphologic phenotyping of the cells. Classification of B-cell lymphomas with "primary" cutaneous manifestation according to the TNM system discloses a more adequate basis for prognostic and therapeutic considerations than the Ann Arbor classification for nodal lymphomas.

Comparison of methods for the macroscopic assessment of epicutaneous allergic contact reactions in guinea pigs.

40 guinea pigs were sensitised with a 50% solution of 2,4-dinitro-1-chlorobenzene (DNCB) and challenged 14 days later with DNCB 0.05%. Four parameters were determined to evaluate the challenge reaction after 24, 48, 72 and 96 h: (a) intensity of erythema, (b) reaction area (product of the largest diameters of the reaction in vertical alignment), (c) increase in skinfold thickness and (d) reaction volume (product of the reaction area and the increase in skinfold thickness). The test reactions were read blind by 2 independent observers, yielding small but significant differences in all methods except determination of the reaction area. Further statistical analysis revealed a linear correlation between the intensity of erythema and the other 3 parameters determined, as well as between the reaction area and the reaction volume. In contrast, the increase in skinfold thickness did not correlate linearly either with the reaction area or the reaction volume. When the results of the 24- and 48-h readings were compared, the characteristic crescendo reaction of contact allergy was demonstrable by all methods except the determination of the reaction area. After the 48-h reading, a continuous decrease of reactions was found with all methods. It is concluded that the determination of the reaction area and consequently of the reaction volume are not suitable for exact measurement of epicutaneous allergic contact reactions in guinea pigs. The most precise results will be obtained by measuring the increase in skinfold thickness, whereas the determination of the intensity of erythema, which is easier to perform, may be sufficient for many purposes.(ABSTRACT TRUNCATED AT 250 WORDS)

Evaluation of the immune status in vivo by the 2,4-dinitro-1-chlorobenzene contact allergy time (DNCB-CAT).

The time interval from starting continuous application of 2,4-dinitro-1-chlorobenzene (DNCB) epicutaneously until the appearance of an allergic contact reaction, i.e. 'contact allergy time' (CAT), was determined. In 34 patients initially 2.0% DNCB ointment was administered for 2 days, then 0.05% DNCB ointment was applied daily (group I). In 39 patients only the 0.05% DNCB preparation was used (group II). The median of DNCB-CAT was 11.2 +/- 0.8 days in group I and 22.3 +/- 3.7 days in group II (p less than 0.01). In patients older than 55 years DNCB-CAT was significantly longer than in younger patients. The medians of DNCB-CAT did not differ in male and female patients. In contrast to the conventional DNCB-sensitization procedures, the CAT method allows one to measure induction and elicitation phase quantitatively.
