Assessing altered body image.

Like pain, altered body image remains highly personal, abstract and difficult to describe. It has taken nearly a century to define the concept of body image, and to construe a reasonable working definition of altered body image (ABI). During this time, the assessment of body image has focused upon the researcher and his/her needs, rather than the professional nurse, who must arrive at a practical means to understanding patients' distress, and identify possible solutions. In this paper the author suggests parameters for the assessment of ABI in the practical arena, based both upon ongoing asthma care research and the experience of practitioners developing body image counsellor roles. It is suggested that altered body image should be assessed as part of an integrated approach, and that the complicated risk or cathexis scales developed for psychological research have limited use within the clinical setting.

Education for nurses working abroad.

Studying abroad poses unique challenges. These relate to the course content, materials, study activities and support, as well as assessment and evaluation. Making a careful assessment of needs, study stamina and support can help students make the right choice. As education is often culturally biased, familiarity with the nursing culture from which the course comes may be the most important prerequisite for success.

The right questions to assess materials. Selecting distance learning material.

1. Distance learning provides real educational opportunities, equal to and complementary with, traditional taught programme approaches. 2. Important questions to ask about distance learning include relevance to practice, accessibility, quality assurance, assessment and value for money. 3. Distance learning need not be lonely and isolating, and can provide education that travels with students--important in today's professional context.

How do women choose to breastfeed?

Midwives generally rely on client education to get the message across that 'breast is best'. Research tends to ask why women bottlefeed rather than trying to find out how the decision to breastfeed is reached. The psychological theory of personal constructs might help to shed light on this difficult and frustrating area of midwifery.

The role of suprahyoid block dissection in carcinoma of the floor of the mouth.

The efficacy of suprahyoid block dissection combined with radiotherapy, in the management of neck nodal disease was evaluated in 15 patients with carcinoma of the floor of mouth (abutting on the mandible) between 1983 and 1989. Ten males and 5 females had a mean age of 60.3 years. Suprahyoid block dissection was performed in 10 patients as initial treatment. Nine of these presented with a submandibular mass and one patient with a T4NO lesion had a prophylactic block performed. Suprahyoid block dissection was performed in five patients who developed a submandibular mass after completion of surgery and radiotherapy. Wound sepsis occurred in three patients, but resolved with conservative treatment. Clinical assessment of the suprahyoid mass was accurate in 65% of patients. Nodal recurrence occurred in one patient. Seven patients are alive and disease free after a mean of 64.5 months. Two other patients are alive, one with lung metastases, and one with a supraclavicular mass. Six patients have died, two of whom developed local recurrence, one who developed a supraclavicular mass, one after nodal recurrence, one with lung metastases, and one of an unrelated cause. Suprahyoid block dissection combined with radiotherapy is effective treatment for nodal control of patients with carcinoma of the floor of mouth. This procedure is associated with a low morbidity.

The asthma experience: altered body image and non-compliance.

Asthma remains a frustrating and underestimated threat to health. Non-compliance with medication is a frequent problem, especially when asthma attacks are difficult to predict, and amongst children and young adults. Non-compliance has been attributed to shortfalls in patient education. There may, however, be an attitudinal-altered image dimension to the problem.

Dignity that must be respected. Body image and the surgical patient.

Body appearance is socially valued but at significant risk through surgery and nursing actions. The use of drips, drains and catheters all disrupt body boundaries. It is important to question whether we need to invade such boundaries, and how quickly tubes or lines can be removed to promote dignity. Skin preparation is important for surgical access, infection control and body image. Shaving may not be the soundest approach to depilation.

Cholinesterases in the amyloid angiopathy of Alzheimer's disease.

Vessels affected by amyloid angiopathy in patients with Alzheimer's disease also displayed intense acetylcholinesterase and butyrylcholinesterase activity when examined by light and electron microscopy. The enzymatic properties of the vessel-bound cholinesterases were identical to those of the cholinesterases associated with senile plaques and neurofibrillary tangles. This cholinesterase activity is of unknown origin but represents one of the very few features common to all the major pathological markers of Alzheimer's disease.

Patterns of drug trafficking and countermeasures: the personal view of a veteran.

In the present paper, a law enforcement official with nearly 40 years of experience in combating drug trafficking provides insight that may prove useful to persons in other organizations and/or cultures. He examines, in particular, the development, at the national level, of drug control legislation, a drug law enforcement strategy and a drug intelligence unit.

Effects of a nutrition workshop on paraprofessional assignment of geriatric nutrition services.

The use of dietary assessment techniques by non-nutrition professionals is necessary if the nutrition component of elderly assistance programs is to be fully utilized to the client's advantage. This study examined the impact of a one day nutrition education workshop on community based assessor's evaluation of nutrition needs and services in the home-bound elderly. At the beginning and end of the workshop, changes in a case study nutrition assessment and assignment of services were evaluated. This same procedure was also followed before and three months after the workshop, using client charts from the field. In both the workshop case study and the field chart review there was a significant increase in the detection of nutrition related problems and assignment of services for these problems after the nutrition education workshop.

A model for body-image care.

It is by no means clear that the diagnosis 'altered body image' is sufficiently clear to enable the nurse to plan and deliver a package of body-image care. This paper proposes a model of body-image care based upon five central concepts, and the accepted metaparadigms of nursing. It is suggested that the nurse is uniquely positioned to deliver the best body-image care, and that this model will assist her to assess body-image needs and to then action the altered body-image diagnosis more effectively.

Continuous multiple location body temperature measurement of infants.

This study was done to evaluate the most suitable location of skin temperature sensors to enable long term temperature measurements of infants. A high accuracy eight channel temperature monitor was developed. Linear semiconductor sensor probes were constructed in association with this. System accuracy and stability were tested. Preliminary clinical studies have shown considerable variation from the core temperature for all surface measurement locations.

Signal transduction by p21ras.

Experiments using the physical loading of purified recombinant p21 ras proteins into quiescent normal cells to analyze how the proteins stimulate DNA synthesis and morphological transformation are reviewed. The results indicate that oncogenic p21ras proteins rapidly activate a protein-kinase-C-dependent pathway and a protein-kinase-C-independent pathway. The activation of protein kinase C is absolutely required for p21ras to stimulate DNA synthesis but is not required for morphological transformation.

Cavernous haemangioma of the head and neck in the adult.

This paper reviews a personal experience of 51 cases seen over a 30 year period. Patients were treated in either a combined Head and Neck clinic or a Combined Ophthalmology clinic and a balanced view is thus represented. A method of grading the severity of the disease is described as well as a method of assessing response. This approach has not been proposed hitherto. It is generally concluded that the preferred treatment for small lesions is excisional surgery and for larger lesions carefully planned and highly localized radiation.
