Balance and skeletal alignment in a group of elderly female fallers and nonfallers.

The purpose of this study was to determine whether sagittal plane posture differed between fallers and nonfallers and to explore the relationship between skeletal alignment and balance in elderly females. Forty-eight women > 65 years of age were recruited from various medical and senior citizen centers. Thirteen of these women were classified as fallers. Spinal posture was measured in standing using an inclinometer, and lower extemity joint angles were assessed in standing using a universal goniometer. The Berg Balance Scale, the Functional Reach Test, end a modified Timed Get Up and Go Test were used to measure balance. Spinal alignment did not differ significantly between fallers and nonfallers; however, knee joint angle was significantly greater in fallers compared to nonfallers. Significant, but low, correlations were found between the inclination of the upper thoracic spine and all three balance measures. Lower thoracic slope and knee joint angle in standing were also weakly related to two of the three balance measures. This study supports the hypothesis that a significant but weak relationship exists between balance and skeletal alignment in elderly females.

Bone-marrow pressure and bone strength.

The pattern of bone-marrow pressure changes differed with the mode of stress application. Intra-medullary pressure remained steady during most of the slow loading. During rapid dynamic loading, however, a slight rise in intra-medullary pressure was observed. Contraction of the femoral muscles also resulted in a greater bone-marrow pressure increase. A correlation of 0.98 (P less than 0.001) between stimulus strength and intra-medullary pressure was obtained. The rise in intra-medullary pressure with femoral muscle contraction is suggested to have a possible role under extreme stresses in living conditions.

Reliability of goniometric measurements of hip motion in spastic cerebral palsy.

Reliability of goniometric measurements of ranges of motion in the right hip of four children with mild or moderate spastic diplegia was studied by comparing results when physical therapists used specific and non-specific measurement instructions. Measurements of hip extension, abduction and external rotation were made. The use of specific measurement instructions improved inter-rater reliability only in the case of external rotation. Inter-session reliability was not improved by the use of specific measurement instructions. It is concluded that goniometric measurements have such a low level of reliability that they can only be used to assist clinical judgment, and that they are not sufficiently reliable to be used in studied of cerebral palsy.

Community care for Canadian seniors: an exercise in educational planning.

This paper outlines the demography of disability among Canada's elderly population, and examines whether services such as Home Care and the Extramural Hospital programmes are meeting the needs of disabled, as opposed to sick, housebound elderly. In the future, an increasing number of rehabilitation professionals will provide services in community-based programmes rather than in hospitals and other health care institutions. A description is given of planned changes in occupational therapy and physical therapy curricula designed to prepare students to work effectively with disabled older persons in the community.
