Laser conization: the results of treatment of cervical intraepithelial neoplasia.

Combination laser conization was performed for the treatment of cervical intraepithelial neoplasia (CIN) in 473 patients, who were followed for a mean period of 70 months. In 16 cases (3.4%) CIN was demonstrated during follow up, and in 10 cases (2.1%) within the first year after treatment. The grade of CIN in the cone specimen was of no predictive value to recurrence. When excisional margins were involved recurrence was observed in 8.7% of cases, compared to 2.3% when margins were uninvolved. The risk of recurrence was significantly higher in patients with involvement of the endocervical cone margin. The only case of invasive disease during follow-up was observed more than 2 years after conization; cone biopsy was without involved margins. The results clearly demonstrate the method to be effective and justify an expectant management of all patients after conization by this method.

A factor encoded by 7q31 suppresses expansion of the 7q- clone and delays cytogenetic progression.

Deletion of part of chromosome 7q (7q-) is a consistent aberration in human acute leukemias and myelodysplasias, especially in patients with a history of genotoxic exposure. The deletion is usually associated with a short survival. Loss of a number of different 7q segments has been described, but still it is not known whether different deletions imply differences in survival. We have investigated the possible importance of loss of different segments in 77 7q- patients studied with high-resolution banding. Thirty-six were examined in our laboratory and 41 were published cases. We found that when the 7q- marker contained the 7q31 band, the 7q- clone was smaller than when the band had been lost (p = 0.011), and the patients survived longer (p = 0.004). Further, karyotypes with complex aberrations were less frequent (p = 0.012). These results indicate that genetic information in 7q31 delays cytogenetic and clinical progression of myeloid disease with 7q-. Our results do not tell, however, whether this is due to direct tumor suppressor activity of a 7q31 gene or due to a more indirect effect.

Budesonide powder administration for the treatment of grass-pollen-induced allergic rhinitis.

The new dry-powder inhaler system, Turbuhaler, has proved to be equivalent to metered-dose inhalers when used in the nose, and the objective of this study was to investigate the efficacy, dose-response effects, and safety of budesonide powder given in the morning during the grass pollen season to patients with grass-pollen-induced allergic rhinitis. Of 190 randomized patients, 186 were treated and 180 completed this double-blind study, which comprised a 4-week treatment period, preceded by a 1-week run-in period. The patients were randomized to three parallel treatment groups: budesonide 400 micrograms, budesonide 200 micrograms, or placebo once in the morning. Assessment of efficacy, by comparing changes in mean scores of nasal symptoms from run-in to treatment, showed a statistically significant effect for all symptoms with active treatments, as compared with placebo. The mean reduction of symptom severity was more pronounced in the 400-micrograms group than in the 200-micrograms group, and this difference was statistically significant for runny nose (P < 0.02) and combined nasal symptoms (P < 0.02). Nasal peak-inspiratory flow improved significantly in both budesonide-treated groups, as compared with placebo (P < 0.01 and P < 0.01). During the treatment period, patients on active treatment showed, on average, a reduction of all nasal symptoms, whereas the placebo-treated patients, on average, showed an increase of nasal symptoms. Approximately 40% in the high-dose group felt total control of rhinitis symptoms, as compared with 26% in the low-dose group. Isolation and culture of human endometrial cells in a three-dimensional culture system.

A cell culture system was established in which endometrial stromal cells were embedded in a collagen matrix and separated from the endometrial epithelium by basement membrane material (Matrigel). The epithelium, seeded on top of the collagen matrix, grew in a monolayer. The cultures were evaluated by light microscopy and transmission and scanning electron microscopy. Light and transmission electron microscopy indicated a polarized columnar epithelium in monolayer with basally positioned nuclei. Scanning electron microscopy revealed a confluent epithelium with an abundance of microvilli and cilia, as well as pinopodes on the apical surface. Immunohistochemical staining for cytokeratin confirmed the epithelial origin of the surface cells, and staining for human collagen IV demonstrated its presence underneath the epithelial cells. This culture system represents a three-dimensional system that imitates the normal endometrium.

Psychosocial aspects of donor insemination. Sperm donors--their motivations and attitudes to artificial insemination.

STUDY OBJECTIVES: To examine sperm donors' motivations and emotional reactions to the role of donors and to measure their willingness to provide information about themselves to recipients and offspring. DESIGN: A prospective trial in which the 26 subjects who were actively functioning as sperm donors in our fertility clinic at the time of investigation were asked to fill in a questionnaire and return it anonymously and voluntarily. MAIN RESULTS: 8% of the donors stated purely altruistic motivation, 32% purely financial and 60% a combination of both. The acceptance rate for providing non-identifying, phenotypic, descriptive information to recipients is 76%, for providing medical information it is 60% and the provision of psychosocial information is accepted by 28% to 40%, depending on the item. 20% of donors are willing to continue donation if the present rules of anonymity are revoked. CONCLUSIONS: Most donors do not seem to feel any close relationship to donor offspring and at least 60% found anonymity to be essential for their further functioning as donors.

Serum eosinophilic cationic protein and lactoferrin related to smoking history and lung function.

Some smokers have an accelerated loss of lung function, possibly due to a chronic bronchial inflammation in which granulocytes are involved. Eosinophil cationic protein (ECP) and lactoferrin (LF) are granule proteins in the eosinophil and neutrophil granulocyte, respectively. We wanted to investigate the relationship of serum (s) ECP and LF concentrations to smoking history and lung function alteration. This partly cross-sectional and partly prospective study included 98 nonatopic smokers and 31 lifetime nonsmokers. As participants in a smoking cessation programme, 50 of the 98 smokers ceased smoking for > or = 1 year. Smoking history, lung function and blood samples were obtained at the start of the study, and smokers and ex-smokers also gave blood samples 3, 6 and 12 months later. s-ECP and s-LF were elevated in smokers compared to people who had never smoked. s-ECP was linearly associated with daily cigarette consumption and forced expiratory volume in one second (FEV1) residuals. In a multiple linear regression analysis, low s-ECP and high s-LF were associated with decreased FEV1 residuals. s-ECP and s-LF together accounted for 10.2% of the variation in FEV1 residuals. After smoking cessation, s-ECP and s-LF decreased within 6 months. s-ECP and s-LF are raised in smokers, and may serve as indicators of granulocyte activation. We speculate that they might contribute to prediction of accelerated lung function loss in smokers, but this question needs further investigation in a prospective study.

[Foot deformities and relation to the length of leg muscles in Danish children aged 3-17 years].

Seven hundred and sixty-nine children and adolescents from three to seventeen years of age, being 98% of the pupils in different classes in seven schools and ten kindergartens on Funen, Denmark, were examined in 1991. The Triceps Surae muscle was found to be too short, if the corrected foot can only be pressed slowly to the right angle. The 1520 footprints showed an increase in the number of highly-arched feet through the age groups from 2.3% to 7.3% and a decrease in flat-footedness from 41.8% to 5.7%. In the two oldest age groups, 39% of those with a high arch, 56% of those with a normal arch and 77% of those with flat feet had short calf muscles. We therefore conclude, that it is necessary that a few minutes be spent daily in Danish kindergartens and schools on stretching the triceps surae.

[Incidence of short hamstrings and leg muscles at ages 3-17 years].

In 1991, 769 children and adolescents, 98% of the pupils in the various age groups in seven schools and ten kindergartens on Funen, were examined. Seventy-five percent of the boys and 35% of the girls over the age of ten proved to have such short hamstrings that one knee had a extension deficit without violence of > or = 40 degrees when the hip was flexed 90 degrees. More than 10% of the boys over the age of ten had an extension deficit of > or = 60 degrees. Forty-five children had a difference > or = 15 degrees between the two legs. The triceps surae muscles are too short if the corrected foot can only be pressed slowly to the right angle. This was found to be the case for 25-64% of the children from the age of three to 17. Therefore, we must conclude that it is necessary to allow time in kindergarten and school for the children to stretch their muscles a few minutes daily.

[Changes of sexual behavior among Danish homo/bisexual men under the impact of HIV/AIDS epidemic].

In October 1991, 1.567 gay/bisexual men throughout Denmark returned an anonymous questionnaire enclosed in the magazine of the National Gay and Lesbian Association. Most were younger, well educated men living in the Copenhagen area. 76% had been tested for HIV, 15% were infected. As in 1988, the median number of partners in 1991 was 2-5. Eighty-two percent of the sexually active men engaged in anal sex, of these half used condoms consistently. Anal sex was practised more with steady partners and condom use was more common with casual partners. One out of eight had engaged in unprotected anal intercourse with a partner with discordant HIV-serostatus, down from one in three in 1988. It is concluded, that Danish gay men have not further limited their sexual life during the later years. The majority of the respondents have increased sexual safety, but risky sex still prevails among a substantial minority, and safe sex campaigns should continue.

[Spinal stenosis. A review].

Spinal stenosis is characterized by a broad spectrum of symptoms which are often vague. Therefore, patients suffering from this condition are typically seen by various specialists over a long period of time. Among the symptoms are various degrees of low back pain, neurogenic claudication, parasthesias, muscle weakness and reflex abnormalities. When spinal stenosis is suspected, CT-scanning seems to visualise stenosis well. Because the localisation of the stenosis is often unknown, myelography is still the first choice. CT-scanning can then provide supplementary information regarding the specific location. MR-scanning is not as yet used as a routine procedure, but will in time replace both procedures. Conservative treatment consisting in analgesics and physiotherapy is sufficient in some cases. When conservative treatment is not enough, surgical intervention consisting in decompression by broad laminectomy is performed. Fusion is performed when there are signs of vertebral instability or in young patients. Operational results are often good. Failure is seen following insufficient decompression.

The effect of salmeterol on the early- and late-phase reaction to bronchial allergen and postchallenge variation in bronchial reactivity, blood eosinophils, serum eosinophil cationic protein, and serum eosinophil protein X.

The late asthmatic response to bronchial allergen challenge and the associated increase in nonspecific bronchial reactivity provides a model for studying the acute inflammatory mechanisms in asthmatic airways. In 12 asthmatic patients (aged 22-37 years) with known dual reaction to allergen challenge, salmeterol 50 micrograms, 100 micrograms, or placebo was administered as a single dose 10 min before allergen challenge in a double-blind, randomized order on three different study days 2 weeks apart. The bronchial reactivity (BH) to histamine was measured the day before and 24 and 48 h after allergen challenge. Salmeterol significantly inhibited the early (P < 0.02) and the late (P < 0.05) asthmatic reactions. After placebo, mean BH was significantly increased above base line at 24 and 48 h (P < 0.02). After 50 and 100 micrograms salmeterol, BH was less than base line at 24 h and returned to prechallenge values at 48 h. Blood eosinophils increased significantly (P < 0.05) 24 and 48 h after allergen challenge, and no difference was found between treatments. After pretreatment with placebo, serum eosinophil cationic protein (s-ECP) and serum eosinophil protein X (s-EPX) increased significantly (P < 0.05) 24 and 48 h after allergen challenge. After treatment with salmeterol 50 micrograms, s-EPX, but not s-ECP, increased significantly 24 h after challenge, but was normal at 48 h. After salmeterol 100 micrograms, no change in s-EPX or s-ECP was found. The results showed that salmeterol eliminated the allergen-induced dual asthmatic reaction and gave protection against increased BH.(ABSTRACT TRUNCATED AT 250 WORDS)

Therapeutic experiences with 947 epileptic out-patients in oxcarbazepine treatment.

Efficacy and tolerability of the new antiepileptic drug oxcarbazepine, was evaluated in a retrospective multicentre study. The records of all 947 epilepsy patients treated with oxcarbazepine in the eight participating centres from 1981 through 1990 were examined. The median daily dose of oxcarbazepine was 30 mg/kg in children, 18 mg/kg in adults, and 15 mg/kg in elderly patients, given b.i.d. or t.i.d. The mean plasma levels of the main active metabolite of oxcarbazepine was 88, 79, and 68 mumol/l in children, adults, and elderly, respectively. In patients shifted to oxcarbazepine treatment, seizure frequency was unchanged in 51-66%, 32-48% had a decrease, and 1-10% an increase in seizure frequency, considering the individual seizure types separately. Adverse events were reported in one third of patients, most frequently affecting the CNS (dizziness: 6%; sedation: 6%; fatigue: 6%). Rash was reported in 6% of patients, half of these patients had previously had an allergic reaction to carbamazepine. Hyponatremia was found in about a quarter of the patients from whom data were available. No congenital malformations were seen in nine live-born, first trimester oxcarbazepine-exposed children.

The effect of single doses of inhaled beclomethasone dipropionate on the circadian rhythm in serum osteocalcin in normals.

A single dose of 2.5 mg prednisone leads to a significant transient decrease in serum osteocalcin, which is only demonstrated by frequent serum sampling. The aim of the present study was to evaluate whether a single dose of inhaled beclomethasone dipropionate causes transient changes in serum osteocalcin, indicating a systemic effect on bone cells. In a double-blind, placebo controlled, cross-over design we evaluated the effects of single doses of 250 micrograms and 1000 micrograms beclomethasone on the circadian rhythm in serum osteocalcin. Fifteen normal subjects aged 23-38 years were studied twice with an interval of one week with hourly blood sampling from 16.30 until 17.00 the following day; 1000 micrograms beclomethasone, but not 250 micrograms, suppressed serum cortisol by 14.4 +/- 6.7% (p = 0.03). Neither of the beclomethasone doses significantly altered the time pattern of serum osteocalcin. We conclude that a single inhaled dose of beclomethasone in the therapeutical range does not acutely influence osteoblastic activity as judged from serial measurements of serum osteocalcin.

Allergy caused by light-cured ear-moulds.

This investigation was conducted after the occurrence of allergic contact dermatitis in three laboratory technicians all engaged in the production of UV-light-cured acrylate ear-moulds. Ninety patients fitted with such moulds claimed discomfort to the same extent as patients with traditionally made acryl moulds. Of thirteen patients seen for clinical examination, seven were tested for allergy in the same way as the technicians and four were found to be sensitized to monomere metacrylates in the wax. The handling of the base materials in the laboratory as well as the UV-light-curing procedure needs further investigation.

Prevalence of oromandibular dysfunction in a general population.

The prevalence of oromandibular dysfunction was studied in 735 subjects from a random sample population of 1,000 subjects aged 25 to 64 years. A diagnosis of oromandibular dysfunction was based on criteria established by the International Headache Society, as a subgroup to tension-type headache. Tenderness in pericranial or jaw muscles was not included. The most common symptoms were clenching (22%) and grinding of teeth (15%). The most common sign was irregular jaw movements on opening and closing (29%). The ratio of men to women for most symptoms and signs ranged from 2:3 to 1:3. This study serves as a base in evaluating the importance of oromandibular dysfunction as a causative factor for tension-type headache.
