A method is described for preparing fibrin glue from fibrinogen obtained by double cryoprecipitation of plasma collected by plasmapheresis. Plasma (average volume, 528.8 +/- 86 mL), collected from 12 plasmapheresis donors was cryoprecipitated twice. When the cryoprecipitated pellet was resuspended in 0.5 to 2 mL of saline (total volume of saline-resuspended fibrinogen: 5 mL), the average yield of fibrinogen was 78.4 +/- 18.3 mg per mL. This is comparable to commercial preparations, is suitable for clinical use, and offers greater safety at a reduced cost.
The author reports an open controlled trial on 72 patients affected by different types of skin ulcers, in which a heterologous lyophilized collagen sponge was compared with Dextranomer as healing adjuvant. The rationale of use and pharmacological properties of this collagen are outlined and explain its outstanding results.
A further case of gastric leiomyoblastoma in a 53 year old woman is presented. Pathology, diagnosis, and therapy of this rare neoplasm are discussed.
The Authors describe a case of subcutaneous emphysema following rectosigmoidoscopy in a 72 year old woman. Guidelines of preliminary conservative treatment and eventually careful surgical approach are suggested.

ATN is a deleterious problem in the outcome of kidney transplantation. This complication is usually related to multiple factors including donor parameters, surgical technique, ischemic time, and recipient variables. In order to develop prophylactic measures, out of 430 kidney transplants performed in our Department, a series of 90 consecutive cadaveric renal allografts has been considered in this study. The overall incidence of IGNF was 23/90 (25.5%). Kidneys from MOD revealed a lower rate of IGNF (7/35 = 20%) when compared with organs from SOD (16/55 = 29%, P = NS). No difference was noted when kidneys were removed together with heart and/or liver and/or pancreas. Out of the donor factors, only CID was significant (17 +/- 9 hours in IGNF v 11 +/- 10 hours in patients with IGF, P = less than .05). Analysis of data concerning the fate of paired kidneys revealed two cases of IGNF in both kidneys from the same donor v 14 cases of IGNF in only one of the two paired grafts (P = NS). We conclude that: 1. Donor factors are clearly associated with a minority of IGNF. 2. The introduction of multiorgan procurement programs does not complicate early function. 3. Recipient factors (immunological events and intraoperative fluid management) provides important additive effects on initial graft nonfunction.
The Authors report the results obtained within a multicentrical trial, weighing the immunostimolant effect of bacterial lysate on 157 patients. The drug (bacterial lysate) has induced an immunitary reaction, making significantly higher either the salivary IgA values or the serum IgA, IgG and IgM values. Also excellent tolerability is peculiar to this new immunostimolant.
Preliminary data of a kindred with multiple endocrine neoplasia type IIa (MEN IIa) are presented. The study concerns 20 subjects belonging to 4 generations. The first is a man, previously adrenalectomized for pheochromocytoma, suffering from thyroid node with high serum calcitonin (CT) and normal tests for parathyroid function, who underwent total thyroidectomy, removal of a grossly enlarged parathyroid and subsequent autotransplantation, because the other glands seemed to be macroscopically uninvolved. In 4 further patients total thyroidectomy and removal of the parathyroid glands with autotransplantation were performed. In all patients, despite biochemical tests had disclosed parathyroid hyperfunction in only one, one or more enlarged parathyroid glands were found, subsequently diagnosed as adenomatous or with focal areas of "chief-cell" hyperplasia. In 3 out of 4 patients, a single test among those performed to detect the presence of a pheochromocytoma was positive. However, because of the lack of a clear correlation among data from biochemistry, stimulation tests and morphological investigations, adrenalectomy has not been performed. Nevertheless these patients will be closely followed up, in order to promptly state when andrenalectomy has to be performed.

106 liver resections have been performed at the Department of Surgery. 42 of these were liver segmentectomies and other minor liver resections. Such kinds of resective procedure have been adopted in 40 patients affected by benign or malignant lesions in whom the normal liver parenchyma had to be preserved. Indications and technical aspects of hepatic tissue preservation in liver resection are discussed. Liver segmentectomy required a transparenchymal approach, by means of finger fracture technique, through scissural planes to the deeply located segmental pedicle. It should be noted that blood loss and complication rate may be minimized when the technical procedure is carefully performed. A satisfactory survival rate in patients with hepatic involvement from colonic cancer is reported.

We report the results of a study carried out with cephalexin and bromhexine in a group of 100 patients at risk for bronchopulmonary complications following abdominal surgery. The treatment achieved very good activity and excellent tolerance. We consider this therapy very useful for prophylaxis and treatment of bronchopulmonary complications in patients who have undergone surgery.
