Clinical experience with the REDY dialysis system.

363 dialysis runs with the REDY sorbent dialysis system have been carried out in 3 patients with acute uraemia, and 14 with chronic uraemia, during a period of 19 months. Nine of the patients with chronic uraemia received from 15 to 87 dialyses, and 6 patients were treated at home. It is concluded that both clinical, biochemical, and rehabilitational results are comparable to those obtained by treatment with conventional techniques. Technical problems have been few and surmountable.

Kidney transplantation and donor age.

The cumulative graft survival after 355 cadaver kidney transplantations was analyzed with special reference to the age of the donors. A significant decrease in cumulative graft survival with increasing donor age was found.

Thermoregulation during static work with the legs.

Sustained static work with the legs, i.e., holding a weight of about 10% of maximal isometric strength for 25 min was compared to dynamic exercise on a bicycle ergometer causing the same rate of heat production. In the static work the subjective feeling of exertion was very high and the effort maximal. The pronounced increase in heart rate and blood pressure and a typical flush of the face and chest (flush areas) indicated a high sympathetic tonus. Plasma catecholamine levels were 1.3 times higher (significant at the Pless than or equal to 0.05 level) after static work than after dynamic work. Although the sustained static work was nearly maximal, the rate of increase in sweating and the change in core temperature during work were not different from the responses to dynamic work.

On the exudation of plasticizers from PVC haemodialysis tubings.

Prompted by a series of hepatitis-like cases in patients on regular haemodialysis treatment, the sets of polyvinylchloride (PVC) test tubings used were submitted to perfusion experiments. The perfusates from four commercially available tubings were also investigated. Diethylphthalate (DEP) could be identified by by IR and PMR spectrometry in the perfusate from the test tubings. UV spectrometry proved the presence of other non-identified substances in the perfusate from both the test tubings and from some of the other tubings.

Pharmacokinetic studies with mecillinam and pivmecillinam.

Mecillinam (FL 1060) is a new beta-lactam antibiotic particularly active against gram-negative organisms. When given intravenously, very high serum levels were maintained for a short period of time. Lower peak levels but comparable bioavailability were obtained after intramuscular administration. Gastrointestinal absorption of mecillinam is poor, and for effective oral therapy the drug must be given in the form of its pivaloyl-oxymethyl ester pivmecillinam (FL 1039) which is well absorbed and rapidly transformed to mecillinam by enzymatic hydrolysis in the body. Urinary recovery of mecillinam after orally administered pivmecillinam was 45% in the first 6 h compared with 55 and 59% after mecillinam given by the intravenous and intramuscular routes, respectively. By increasing the dose the orally active ester produced proportionally higher levels of mecillinam, and the area under the serum curve was doubled with the dose. Higher peak levels and prolonged maintenance of high serum concentrations were seen after administration of pivmecillinam with probenecid. The presence of food in the stomach did not influence the absorption of pivmecillinam to any great extent.

Deposits of immunoglobulins and complement in the dermoepidermal junction of patients with anaphylactoid purpura.

Skin biopsies from 3 patients with anaphylactoid purpura examined by an immunofluorescence technique revealed deposits of immunoglobulins, complement C3, and fibrinogen in the dermo-epidermal junction and in vessel walls of clinically involved as well as uninvolved skin. The deposits in the dermo-epidermal junction are similar to those seen in the skin of patients with systemic lupus erythematosus.
