Ichthyosis. Reliability of clinical signs in the differentiation between autosomal dominant and sex-linked forms.

The reliability of clinical signs to differentiate between autosomal dominant ichthyosis (ADI) or sex-linked ichthyosis (SLI) were assessed. SLI was defined as ichthyosis in combination with steroid sulphatase deficiency. Age of onset and format of the scales were found to be reliable signs to ascertain the diagnosis of ADI or SLI. The ADI-like appearance in histopathology seems to be of little diagnostic value.

Sensory loss following cryosurgery of skin lesions.

Sensory testing by means of a graded bristle technique was performed following cryosurgical treatment of 183 skin lesions in 169 patients. Mild, generally transient, sensory loss was found in 28% of the treated lesions. This sensory loss appeared not to be influenced by differences in the cryosurgical procedures nor by postoperative complications in wound healing, but was dependent on the area of the body where cryosurgery was performed. Treated lesions on the trunk and neck and on the arms and legs showed the most profound and longest lasting sensory loss.

Influence of environmental mycobacteria on the prevalence of leprosy clinical type.

A comparison was made of antibody levels to 16 environmental mycobacteria in leprosy patients and healthy controls. Significant differences in response were found between patients and controls from an area of Zimbabwe with predominantly lepromatous leprosy when compared to an area where more tuberculoid cases were found. The results obtained suggest that exposure to some environmental mycobacteria may influence the type of leprosy developed by susceptible individuals.

Thalidomide therapy. An open trial.

In an open trial, 35 patients suffering from a variety of dermatologic conditions were treated with thalidomide. The drug was shown to be a valuable addition to the therapeutic armamentarium; however, the occurrence of a peripheral neuropathy will limit its use just as its teratogenic side effects have done for many years.

Class specific anti-Mycobacterium leprae antibody assay in lepromatous leprosy (BL-LL) patients during the first two to four years of DDS treatment.

Previously, a slight decrease in antibodies against M. leprae antigen 7 was demonstrated after one year of dapsone (DDS) treatment in 14 of 15 patients with lepromatous (BL-LL) leprosy. The same patients have now received DDS from 11/2 to 4 years (median 3 years) and sera taken at the start, during, and at the end of the observation period have been retested for antibodies against M. leprae antigen 7 by radioimmunoassay and tested for IgA-, IgM-, and IgG-anti-M. leprae antibody activity by solid phase radioimmunoassay. Both IgA- and IgG-anti-M. leprae antibody activity and the activity of antibodies against M. leprae antigen 7 showed a decrease in activity after three years of DDS treatment to a median value of about one third of the activity in the sera taken at the start of the study. A smaller but significant decrease in IgM-anti-M. leprae antibody activity could be demonstrated. A transient increase in antibody activity (both measured by RIA and sRIA) could be demonstrated and related to reactions (reversal reaction and ENL) in five patients. No significant correlation could be found when IgA-, IgM-, and IgG-anti-M. leprae antibody activity was compared with antibodies against M. Leprae antigen 7 in individual sera.
