Twenty-five non-smokers and 35 smokers who had abstained overnight from cigarette smoking and were undergoing minor elective orthopaedic surgery were studied in order to compare gastric volume and pH. There was no significant difference in aspirated volume and pH both just after intubation and prior to extubation between the two groups, thus indicating that habitual smokers undergoing elective surgery in general anaesthesia do not have increased risk of acid pulmonary aspiration compared with non-smokers.
Paracetamol 20 mg kg-1 dissolved in 200 ml of water was given by mouth to seven patients undergoing minor orthopaedic surgery in spinal analgesia, and again 2 weeks later. The rate of paracetamol absorption was significantly delayed by spinal analgesia and surgery, indicating an inhibition of peroperative drug absorption and gastric emptying. The effect of spinal analgesia on gastric emptying was not able to normalize the delayed peroperative gastric emptying produced by surgery, and suggests that surgery is an important factor in peroperatively delayed gastric emptying.
Significant aspiration may occur around correctly inflated high volume, low pressure endotracheal tube cuffs. The prevention of silent aspiration due to leaks around cuffs of endotracheal tubes was investigated during general anesthesia for hip replacement in 47 patients. The patients were randomly assigned to one of three groups, in which one of three endotracheal tubes of different designs were used for intubation. The following three tubes were used: the red rubber Rüsch tube with low residual volume cuff, inflated to minimum occluding volume; the Mallinckrodt Hi-Lo tube with high residual volume cuff; the NL tube with high residual volume cuff and automatic cuff pressure regulation. The cuffs on the Mallinckrodt and the NL tubes were inflated to 29-31 cm H2O. One hour before termination of the surgical procedure, 1 ml methylene blue dye was injected into the trachea just above the cuff through a thin channel built into the tubes. At termination of the operation, the trachea below the cuff was inspected with a fiberoptic bronchoscope. Aspiration was found in 12.5% with the Rüsch tube, in 31.2% with the Mallinckrodt tube, and in 0% with the NL tube. Our results show that silent aspiration is still a problem with standard endotracheal tubes, but that it may be minimized by use of appropriate tubes, cuffs, and control of cuff inflation.

Seventy-four habitual smokers who had refrained overnight from cigarette smoking and were undergoing elective orthopedic surgery were allocated randomly to two groups in order to investigate the effect of smoking two cigarettes before premedication on volume and pH of gastric contents. The smoking group was allowed to smoke two cigarettes while the non-smoking group was not allowed to smoke. There was no significant difference in volume or pH between the groups, either just after intubation or prior to extubation, thus indicating no decreased risk of acid pulmonary aspiration because of refraining acutely from cigarette smoking.

The results of a follow-up investigation of 12 patients with ameloblastomas of the mandible, treated by conservative surgical means, are presented. The age of the patients at the time of operation ranged from 8 to 77 years, the average age being 41 years. Seven of the tumors had a unilocular and five a multilocular appearance. The operation technique varied somewhat with the individual case. An attempt was made in all cases to remove the tumor in toto. In cases with multilocular appearance or where the tumor was in connection with the surface epithelium, a resection of the roof of the lesion was made. Microscopic examination of the tumors revealed that they all could be classified as plexiform or folicular but with various degrees of differentiation. Seven of the patients were observed for more than 5 years, the others for less. Three instances of recurrence were diagnosed and were in all cases less than 1 cm in diameter. The recurrences were found 1, 3, and 20 years after the primary treatment. Although the number of patients and the observation period are limited, the preliminary results seem encouraging.

During the five-year period 1984-1988 we received 192 specimens from 180 patients infected with the human immunodeficiency virus (HIV) for investigation of Legionella infection. The majority of specimens were bronchoalveolar lavage (BAL) fluids (84%), but tracheal suctions and lung tissue from autopsies were also examined. The diagnostic methods used were a direct immunofluorescence assay (DFA) for the detection of Legionella antigen, and culture on buffered charcoal yeast extract (BCYE-alpha) media. All specimens were also examined for the presence of other bacterial lung pathogens, and all BAL specimens additionally for Pneumocystis carinii and mycobacteria. Legionellosis was not found to be common among HIV-infected patients, as only six specimens (3%) from six patients were found positive by DFA, and no specimens were culture-positive for Legionella species. Dual infection with Legionella and P. carinii occurred in two patients. Clinical data of the six patients are presented, and currently used methods for diagnosing legionellosis are discussed.
