Prognosis of post-streptococcal glomerulonephritis: Chandigarh study.

144 patients (107 adults and 37 children) belonging to a homogenous North Indian population who were hospitalised for acute post-streptococcal glomerulonephritis have been followed up for a period of ten years. The majority of these belonged to the lower socio-economic group. Varying degrees of proteinuria was present in 88.2%, hypertension in 62.5%, renal insufficiency in 61% and hematuria in 51.4% of the patients at the onset of the disease. By the end of the two years, 25 patients were dead because of rapid progression to end stage renal failure and 6 patients had mild to moderate renal insufficiency. Of the 119 patients who were alive at the end of the two years, 32 patients (who had shown a clinical recovery) were not available for further regular follow up and were excluded from the long term study, 87 patients were followed up beyond 2 years. In addition to 6 patients who were already in renal insufficiency at two years, 6 more developed renal failure on further follow up. 3 out of these 12 patients progressed to end stage renal failure 5 to 10.5 years after the onset of the disease. Persistence of nephrotic state, hypertension and renal insufficiency following an acute attack, and the presence of glomerular crescents on renal histology were associated with a poor outcome. Whereas only 2 out of 37 children died of end-stage renal failure, the majority of the deaths were in the adults (26 out of 107). Thus, 17% of the patients of acute post-streptococcal glomerulonephritis progressed to end stage renal failure within 2 weeks to 2 years, another 2% within 10 years and more than 6% will ultimately progress failure beyond 10 years.

Clinico-pathological study of testicular involvement in leprosy.

Twenty five patients of lepromatous leprosy were studied clinically and histopathologically for testicular involvement. Testicular pain or swelling was the commonest complaint (68%) followed by sterility (28%) and impotence (4%). Reduced testicular size associated with soft feel was observed in 76% patients. Gynaecomastia was present in 36% and altered sexual hair pattern in 24%. Eleven out of sixteen (69%) patients had oligo/azoospermia. Out of the twenty testicular biopsies 15 (75%) had definite histological evidence of leprous pathology, irrespective of testicular size, semen picture and clinical signs and symptoms. One out of three epididymal biopsies showed minimal changes. Histopathological changes varied markedly, it was not possible to categorize these into vascular, interstitial and obliterative phases.

Traumatic organized hematoma of the spermatic cord.

We report a case of an organized hematoma of the spermatic cord, resulting from direct blunt injury, and review the literature related to this condition. The probable factors accounting for the infrequent occurrence of this lesion in association with external genital trauma are discussed.

Pattern of renal amyloidosis in Indian patients.

Two hundred and thirty-three patients with renal amyloidosis were studied in an attempt to identify the incidence and pattern of the disease in northern India. The incidence of amyloidosis was 1.01% of 6431 post-mortems and 8.4% of 1980 renal biopsies from patients who presented with clinical evidence of glomerular disease. Two hundred and three patients (87.1%) had secondary amyloidosis, 22 (9.4%) had primary amyloid and 8 patients (3.5%) had amyloidosis associated with multiple myeloma. Tuberculosis of various organs was the commonest predisposing disease accounting for 59.1% of secondary amyloidosis, followed by chronic suppurative lung disease in 24.1%. Rheumatoid arthritis, chronic osteomyelitis and lepromatous leprosy were seen in a small percentage of patients (2 to 8%). Proteinuria of varying degree was present in all the 233 patients and 12.9% of them had a daily protein excretion of more than 10 g. Post-mortem examination of 65 patients with renal amyloidosis showed that 75.3% also had amyloid deposit in the spleen, 63% in the liver, and 50.8% in the adrenals. Clinical evidence of disappearance of proteinuria was observed in 3 patients with secondary amyloidosis; in 2 of them, the regression of amyloidosis was confirmed by serial renal biopsy performed 3 and 5 years after the initial diagnosis.

Renal immune complexes in viral hepatitis.

Twenty-two patients dying of acute viral hepatitis were studied for the presence of renal immune complexes using direct immunofluorescence. A detailed light microscopic study of liver, kidney and other organs was also done and sera scanned for the presence of HBsAg. Results of light microscopy indicate that thirteen of twenty-two patients showed mild to moderate glomerular abnormalities. Immunofluorescence was positive in fifteen of twenty-two patients. IgA being positive in 28%. The incidence of HBsAg was low. It is suggested that both B and non-B type of hepatitis viruses are associated with a high incidence of renal immune complexes along with morphological alterations in the glomerulus.

Diffuse biphasic cutaneous amyloidosis.

2 patients with primary localized cutaneous amyloidosis were seen to have widespread macular and lichenoid types of lesions. Diffuse areas of pigmentation appeared to transform gradually over years into lichen amyloidosus as a result of chronic irritation of the skin from scratching. There was no evidence of systemic amyloidosis.

Observations on the value and limitations of infusion pyelography in anuric patients.

Infusion pyelography was performed in 103 anuric patients with no attendant mortality. It was useful in 83 cases in distinguishing obstructive uropathy from renal parenchymal disease. However, in the remaining 20 cases when either renal parenchymal disease co-existed with obstructive uropathy or obstructive uropathy existed alone under certain circumstances, opacification of the collecting system did not occur. The clinicopathological setting of some of the latter group of cases is illustrated by examples.
