[Information and education interventions on hygiene levels of a population in which trachoma exists in a hyperendemic and serious form].

The aim of this work is to develop information and education methods in order to obtain better hygienic conditions in Tissint Zaouia (Moroccan province of Ouarzazate). 41% of the population is affected by trachoma with a rate of 18% of active trachoma, 1.6% of the population has a blindness of corneal origin. Topical treatment and surgery of trichiasis must be associated to a better hygiene of the community. Until now mothers and families have not fully perceived the connection between flies and insufficiency of narsing and trachoma transmission. Our specific goals are the following: A--information of target population, B--better understanding of hygiene by mothers and families, C--incitation to face-washing of children, D--perfect identification of trachoma signs by health workers, E--official integration of this program by local authority, F--evaluation of trachoma incidence.

[Cause of blindness in the province of Giang-Vietnam].

An Giang province is one of many provinces of Mekong Delta, South of Vietnam, with 2 million people and 85% population living in the countryside for agriculture. Main causes of blindness are leucoma secondary to trachoma and mycoses (16%) closed angle glaucoma (5%) and cataract (50%). Percentage of eyes requiring ocular surgery is 0.88% of total population. 1200 patients had surgery during year 1995; as 16,000 patients are waiting for surgery, we stress that this backlog must be reduced as soon as possible. On an other hand, we intend to develop corneal transplantation.

[Identification of latent forms of Chlamydia trachomatis in the conjunctiva signifying "new interpretation of ocular chlamydia infections"].

Chlamydia trachomatis (CT) sérovar D through K was demonstrated with electron microscopy and biology in 114 patients with bilateral dry eyes. Long time ago latent CT was recognised in conjunctival epithelium of vernal kerato conjunctivitis patients. CT was found in conjunctival section of patients with an extinct trachoma. Aberrants forms of chlamydia bear the responsibility of this long lasting infection. Reticuled body does not product elementary bodies. It becomes an aberrant body which could remain latent during years. DIF and others classical investigations are negatives because abberrant chlamydiae have lost most part of their MOMP albeit keeping immunopathogenicity.

[New vaccine against trachoma].

The new vaccine against trachoma is established upon a stimulation of MALT system by oral route. The amount of IgA produced using chlamydia trachomatis serovar C is sufficient to allow a protection against infections of the same species of chlamydiae and/or of others varieties. There is a cross immunity between different serovars and different species. The vaccine could bring a protection against trachoma, chlamydial infection of human and animals.

[Floppy eyelid syndrome. Apropos of 4 cases and review of the literature].

Often unrecognized, Floppy eyelid syndrome is an impairment of the eyelid statics with marked conjunctival signs. Over-weight middle aged men are principally concerned. Excessive eyelid laxity in the Floppy eyelid syndrome is responsible for a night ectropion of the upper lid, creating the clinical manifestations. We report four cases of Floppy eyelid syndrome to illustrate our work and discuss the clinical and therapeutic aspects.

[Corneal ulcer and Behçet disease. Apropos of a case].

We report a case of a peripheral corneal ulcer that occurred to a 33 years old female with a Behçet's disease. This patient had a previous history of viral keratoconjunctivitis with subepithelial nodules one year ago. The type, aspect and localization of the ulcer suggested an immunologic origin. The evolution was favourable after systemic and local steroid therapy. The other aetiologies of such ulcer are discussed by the authors.

[Complication of keratoprosthesis of posterior fixation: retroprosthetic membranes. Apropos of 4 cases].

Keratoprosthesis represents the last solution for patients with bilateral corneal blindness or for monophthalm patients who cannot benefit from corneal grafts. Complications related to this technique are recalled together with an in-depth analysis of the problem of retroprosthesis membranes. The frequency of retroprosthesis membranes varies depending on the series reported, although it appears to be a more agreement on treatment and causes. Among 8 patients who underwent a KPF operation using the E. Lacombes technique, 50% of the cases had retroprosthesis membranes with an unusual thickness, vascularization and recurrent character. Emphasis was placed on early and intense intraocular inflammation.

[Orbital dermoid cysts. Apropos of a case].

We report the case of a 53 year-old female patient with an orbital dermoid cyst. A mass effect with proptosis, hypotropia and choroidal folds was noted. Imaging aspects (radiography, ultrasonography and computed tomography) were highly suggestive of a deep orbital dermoid cyst. A fronto-temporal neuro-surgical approach allowed complete removal of the tumour. Histopathology confirmed the diagnosis of dermoid cyst. The incidence of orbital dermoid cyst falls significantly after the second decade of life thus making our clinical case original.

[Lacrimal IgE levels in chlamydia infections].

Dosage of tear IgE was performed in lacrimal fluid of 110 patients with chlamydioses (12 were trachomatous and 98 had a chlamydiosis serovar D through K). Simultaneously skinpatch testing and RASTS were evaluated. In half of cases, we found a rise of tear IgE. All these patients had documented allergy. We conclude that allergic treatment could be added.

[Results of therapeutic photo-keratectomy using the Excimer laser. Apropos of 12 cases].

We report a series of 12 eyes which underwent phototherapeutic keratectomy with the Excimer laser Aesculap Meditec (model Mel 60). Mean follow up was 11 months (range 3 to 19 months). Six types of corneal pathologic lesions were treated: recurrent corneal erosions, corneal scars after herpetic keratitis, anterior stromal dystrophy, band keratopathy, mucous plaque, and scar after viral infection. Epithelium healed within the first post-operative week in 83.3% of cases. The goal of treatment was achieved in 75% of cases (100% of therapeutic success for recurrent corneal erosions and 66.6% for superficial corneal opacities). The main undesirable effects were postoperative pain, delayed reepithelialization (16.6%). One patient (8.3%) lost more than two lines of visual acuity. The results, the limits and the undesirable effects of phototherapeutic keratectomy are discussed from our experience and data found in the literature.

[Outcome of a corneal graft with antecedents of radial keratotomy].

We performed a penetrating keratoplasty with a donor corneal graft who had been operated by radial keratotomy one year ago. The clinical and endothelial cell density follow-up was 18 months. The endothelial cell loss was comparable to that of a non-operated donor graft. No complications such as reopening or vascular invasion of the incisions occurred. The authors conclude that radial keratotomy is not a contraindication for transplantation and suggest the use of such grafts for eyes with bullous keratopathy with a poor functional prognosis.

[In situ hybridization of cells infected by Chlamydia trachomatis].

Hybridization in situ of chlamydia trachomatis allows identification of the bacteria. Moreover exact chlamydia trachomatis localization at tissue level can be demonstrated. It seems to us very important to know for instance in conjunctiva were chlamydia trachomatis are located. We used DNA DIGOXIGENIN labelled plasmidic probe of 503 b.p. Detection of chlamydia trachomatis on paraffin embedded cells becomes possible. Specificity is 100% as plasmidic sequence is not found in other bacteria. Sensitivity seems to be good quality on experimental model. At present time, it is under clinical evaluation. This method could complete PCR for chlamydia detection in case of controversial diagnosis.

[Permanence of Chlamydia of different serotypes in tissues (methodology)].

Immunofluorescence detection of C. trachomatis requires trained personnel because the threshold between positive and negative reaction is not obvious. Patients antibodies must cover EB and thus prevent binding with Mab. This problems have stimulated research on Chlamydiae probes DNA hybridization technology was applied and the specificity of the detection of CT was improved. Moreover, a polymerase chain reaction amplification of a fragment of about 1,200 bp of the M.O.M.P. gene followed by restriction endonuclease digestion with selected enzymes allows differentiation of serovars. This method seems to be promising for epidemiologic studies.

[Conjunctival autograft in the surgical cure for pterygium. Apropos of 186 cases].

186 cases of pterygium were operated, in the Dr G. Berniac's department of ophthalmology, in the West Indies, at the Hospital of Lamentin, by several surgeons, between January 1, 1988 and December 31, 1990. All of them were operated according to an original technic which included the resection of the pterygium and was followed by a conjunctival autograft. Then the authors compare the results of their series to different series of the literature. The simple resection of the pterygium with a conjunctival autograft seems to be an excellent surgical technic as for its simplicity as for its low rate of recurrences.

[The Schocket tube. A retrospective study of 36 cases].

Thirty-six eyes with refractory glaucoma were operated using a Schocket technique (anterior chamber shunt to an encircling band). The follow-up was more than one year in all cases with a two-year maximum follow up. The intraocular pressure was < or = 25 mmHg in 87.5% to 95% of cases, and < 21 mmHg in 75.8% to 81.5% of cases. The results were analysed for each etiology and compared with those of other series and those of other surgical techniques used in the treatment of this type of glaucoma. (Molteno drainage tube, cyclodiathermy, ultrasonic treatment and yag laser transcleral contact cyclophotocoagulation).

[Diagnosis of ocular chlamydiosis by gene amplification (polymerase chain reaction or P.C.R.)].

Identification of conjunctival chlamydioses could be very difficult since this disease is extremely latent. Moreover previous instillation of topical antibiotic provokes a negativation of D.I.F. and of cultures. P.C.R. is very useful, it was demonstrative in 41 cases out of 47 cases; D.I.F. was positive only in 18 cases. P.C.R. is a very promising method to recognize chlamydiae; identification of different serovars is processing.
