Polyarteritis nodosa and familial Mediterranean fever.

A 22-year-old familial Mediterranean fever (FMF) patient was hospitalized for continuous fever, myalgia, hypertension, vertigo and a petechial rash. Laboratory findings revealed hyperglobulinaemia, thrombocytosis and a leukaemoid reaction. While on steroid therapy the patient sustained a haemorrhage into a renal aneurysm which responded to gel foam embolization. After 12 months of follow-up his condition remained stable under treatment with cyclophosphamide, prednisone and antihypertensive medications. This case provides the fourth example of polyarteritis nodosa associated with FMF.

Percutaneous transluminal angioplasty. A suggested method for analysis of clinical, arteriographic, and hemodynamic factors affecting the results of treatment.

Sixty-nine percutaneous transluminal angioplasties (PTAs) were performed in the iliac and the femoral arteries of 54 patients. The main factors that might affect the results were reviewed: associated diseases, severity of ischemic symptoms, location and type of lesions (stenosis or occlusion and their length), and presence of disease distal to the site dilated. The vascular status of the patients in our series was more severe than in most others. Improvement at follow-up (average 15 months) was maintained in 74% of 66 radiographically successful PTAs. Poor runoff was the only factor found to affect the outcome deleteriously. Intra-arterial pressure measurements performed during the procedure and vascular laboratory studies were most valuable in assessing immediate and long-term results. Wide variations in results of PTA published in the literature are due to differences in methods of selection, evaluation, and reporting. A plea is made for future presentations of PTA series in an analyzable and comparable way, including and correlating all relevant variables as in the model suggested by the present study.

Persistent sciatic artery: a case report.

Persistence of the embryonic sciatic artery is a rare vascular anomaly with only 52 documented cases, of which only 19 had angiographic studies. We describe a patient with angiographic demonstration of persistence of a left sciatic artery associated with arterial anomalies of the opposite limb. The embryology radiological findings and clinical significance are discussed.

The role of spermatic venography after surgical high ligation of the left spermatic veins: diagnosis and percutaneous occlusion.

Bilateral spermatic venography was performed in 40 patients who had previously undergone surgical high ligation of the left spermatic vein. Indications included recurrent or persistent varicocele or oligoteratospermia syndrome. Despite the prior surgery, 21 patients had venographic demonstration of a left-sided varicocele. Right-sided varicocele was demonstrated in 19 patients, 9 of whom also had left varicoceles. Only 9 patients did not have a varicocele demonstrated on either side. The various mechanisms of varicocele filling are discussed. Whenever a varicocele was demonstrated, immediate occlusion using steel coils was performed.

Diagnosis of osteoid osteoma using computerized tomography.

A thirty-seven-year-old male with Osteoid Osteoma of the proximal ulna is described. He had suffered from pain in his elbow for two and a half years prior to diagnosis. Computer Tomography demonstrated and localised the nidus which was surgically removed.

Intestinal neurofibromatosis. Multiple complications in a single case.

The use of radionuclide gastrointestinal scanning with 99mTc sulfur colloid, supplementing angiography, is described in the diagnosis of the source of bleeding in a 60-year-old woman with neurofibromatosis. These two procedures succeeded in locating the origin of bleeding from neurofibroma in the jejunum, which was corrected by surgery. To our knowledge, this case report is the first description of a patient with neurofibromatosis, with all the reported gastrointestinal symptoms: intermittent abdominal pains, palpable abdominal mass, gastrointestinal bleeding and small bowel obstruction.

Percutaneous closure of a Blalock-Taussig shunt.

The technique used in the percutaneous closure of a Blalock-Taussig shunt in a 4-year-old child is described. After selective catheterization of the subclavian artery leading to the shunt, a spring coil was inserted which completely occluded the shunt.

Echinococcus of the urinary tract: radiologic findings.

Echinococcal involvement of the urinary tract is extremely rare. We present 11 such cases, 10 with renal and 1 with bladder involvement. Clinical and laboratory findings were nonspecific. Calcification was present in all but 1 case. The excretory urogram and retrograde pyelogram demonstrated parenchymal soft-tissue masses causing pressure on and dilatation of the collecting system. Angiography in 3 cases showed the masses to be avascular. The ultrasound findings of a multicystic lesion with mixed echogenicity were typical of echinococcal cysts, while a CT scan in 1 patient showed a cyst with densely calcified border. The combined findings of these different modalities aid greatly in establishing the correct diagnosis.

Percutaneous venography and occlusion in the management of spermatic varicoceles.

Spermatic venography was performed in 140 patients; the main indications were subfertility and abnormal spermatogenesis. Of these 140 patients, 113 had positive examinations, with 33 of them having bilateral varicoceles. Of 146 total varicoceles demonstrated, 42 were found on the right side; the advantage of using the jugular vein approach in both the diagnosis and percutaneous treatment of these cases is stressed. Of the 146 varicoceles, 128 were successfully occluded using spring coils. Because of the frequency of bilateral subclinical varicoceles, spermatic venography and percutaneous embolization is recommended in all men with subfertility and oligoteratoasthenospermia.

Clinical use of captopril in Takayasu's disease.

Three patients with Takayasu's disease (TD), initially observed with hypertension and/or congestive heart failure, were treated with captopril, an orally active inhibitor of converting enzyme. As arteritis in this disease is diffuse and extensive, surgical correction of renovascular hypertension is often delayed or impossible. Our experience enables us to recommend captopril for conservative treatment in TD.

Failure of balloon membranotomy of the inferior vena cava.

A young woman with membranous obstruction and coarctation of the inferior vena cava is described. A balloon membranotomy was performed with a transient improvement. Three months later massive thrombosis of the inferior vena cava developed. It appears that balloon membranotomy alone is contraindicated in the presence of both stenosis and membranous obstruction of the inferior vena cava.

Cardiac complications in vascular procedures: comparison of percutaneous angioplasty and surgery.

Percutaneous transluminal angioplasty (PTA) is currently recommended to high risk cardiac patients as a relatively safer procedure compared to reconstructive peripheral vascular surgery (RPVS). In order to evaluate this approach we studied the incidence of cardiac complications in 86 consecutive patients undergoing peripheral vascular procedures. 45 patients (Group I) underwent RPVS and 41 (Group II) PTA. The two groups were comparable regarding age, sex, the location of vascular lesions, the incidence of previous MI (Group I--40%, Group II--46%), and the incidence of angina pectoris (I--25%, II-29%). No special treatment or monitoring was used for patients with ischemic heart disease (IHD) in the PTA group. Successful revascularization was achieved in 36 (80%) of the surgical patients and 25 (61%) of the patients who underwent PTA. There were two anginal attacks, two MIs, and one cardiac death in group I compared to six anginas, three MIs, and one death in Group II. These differences are not statistically significant. It is concluded that PTA carries a definite risk of cardiac complications in patients with known IHD.

Smooth muscle tumors of the alimentary tract.

Smooth muscle tumors (SMT) of the alimentary tract are uncommon. A series of 32 cases (16 leiomyomas and 16 leiomyosarcomas) are presented. The most common presenting clinical signs are bleeding or obstruction. These are caused mainly by degenerative changes within the lesion or its enlargement. The endoluminal SMT cause symptoms early and are more easily detectable. However, the extraluminal SMT, producing an extrinsic mass, can remain asymptomatic for a long time. Thus, such SMT may be found incidentally during operation. The larger SMT are mostly malignant. Microscopically it is sometimes difficult to determine if the SMT is benign or malignant.

Computerized tomography in ovarian cancer.

Sixteen women suffering from ovarian cancer were staged by clinical and pathological means and concomitantly scanned by computed tomographic means. Computed tomography (CT) was found accurate in nine patients. The staging of the disease was upgraded in three patients following the CT examination. CT examination in four patients was equivocal or failed to detect the true extent of the disease. It was not possible to accurately assess the true nature of the pelvic mass on CT following a partial debulking pelvic procedure, as the remnant pelvic bed tissue could be misinterpreted as recurrent cancer. Small peritoneal cancer seedings were not detected on CT. CT scanning despite certain limitations is a valuable noninvasive adjunct in the assessment of carcinoma of the ovary and its response to treatment.
