Evolution of early lesions in leprosy.

We observed 29 patients presenting with vague peripheral neurological symptoms for 6 months or more. During this period, 16 developed clinical leprosy, 3 developed borderline tuberculoid leprosy and the other 13 developed neuritic leprosy. Of these 13 cases 11 subsequently developed skin lesions similar to those seen in indeterminate and in borderline tuberculoid leprosy. Based on the above observations, an attempt has been made to explain the evolution of early lesions of leprosy.

Percutaneous balloon pericardiotomy in the management of recurrent malignant pericardial effusions.

Two patients with recurrent symptomatic pericardial effusions secondary to malignant disease were successfully treated by percutaneous balloon pericardiotomy. Open surgery was avoided and the procedure was completed under local anaesthesia in less than 40 minutes. The first patient was free of recurrence at nine months but pericardial effusion recurred at two months in the second patient. Percutaneous balloon pericardiotomy offers a potentially important new means of relieving recurrent tamponade and substantially reduces trauma to the patient.

Analysis of circulating immune complexes from leprosy patients for Mycobacterium leprae antigens.

Circulating immune complexes (CICs) from 31 leprosy patients (16 tuberculoid, 15 lepromatous) and 12 healthy volunteers, precipitated by 3.5% polyethylene glycol, were individually subjected to SDS-PAGE and immunoblotting using a variety of monoclonal and polyclonal antibodies against Mycobacterium leprae. A common mycobacterial antigen of an apparent molecular size of 65 kDa was identified in CICs from about 40% of the patients. No correlation was observed between the positivity for this antigen and any of the following parameters: bacterial index, M. leprae-specific antibody titers, motor nerve involvement, duration of disease or treatment. Nevertheless, patients with a relatively recent and massive infection were more frequently positive for antigen than the others.

Enhancement of virulence of Entamoeba histolytica by histamine in vitro.

Three isolates of E. histolytica isolated and maintained in modified Boeck and Drbohlav's medium and the axenic nonpathogenic strain NIH-200 maintained in Diamond's TPS-1 medium were used to assess the effect of histamine added to the cultures on their pathogenicity in just weaned rats of Charles Foster strain. Initially when the three polyxenic isolates were examined for their Pathogenicity after growing them with graded concentrations of histamine viz. M-2 through M-6 (log dilutions of the molar concentration of histamine dihydrochloride) the caecal scores were significantly enhanced in cultures grown with M-2 and M-3 concentrations. Subsequently NIH-200 strain was examined similarly after growing it with 20 micrograms/ml of histamine dihydrochloride through three generations. The Neal's caecal score of NIH-200 increased significantly stepwise up to second subculture and became stabilised at third generation with histamine.

Nerve abscess simulated by a lipoma in a leprosy patient.

A case of suspected giant nerve abscess near the radial nerve in the upper arm of a patient with borderline leprosy is reported. On exploration, it turned out to be a deep seated lipoma. Consideration should be given to exploring nerve masses in leprosy.

Retinal hemorrhages in newborn--fetal causative factors.

Two hundred and eight alive newborns examined within 2 hours of birth showed 37.50% incidence of retinal hemorrhage (RH). Out of these, 38.59% were full term and 29.17% were preterm. Head circumference had a significant correlation (p less than 0.05) with retinal hemorrhage, while sex of the newborn, birth weight, gestational age and fetal distress showed no significant correlation. Birth injuries especially ocular and periocular injuries showed a significantly higher incidence of retinal hemorrhage (83.33%).

Leprosy in infants--report of two cases.

Two infants, one 4 months old and the other 2 months old, having histologically confirmed indeterminate leprosy are reported. The route of infection, mode of transmission, and incubation period are discussed with reference to these two cases of infantile leprosy.

Leprosy deformities. II. As parameters to monitor the effectivity of leprosy control programme.

Leprosy deformities have been considered as the main reason for dehabilitation and social ostracism. Prevention of deformities is considered as one of the most important objectives of leprosy control programme. In present work based on deformity status, efforts have been made to evolve new parameters and their possible application in assessment of leprosy control programme.

Leprosy deformities. I. An epidemiological study in a rural area of Rajasthan.

Leprosy deformities have been the cause of dehabilitation, destitution and social ostracism. Present study was planned and conducted in a rural area situated in eastern districts of Rajasthan. Out of 426 cases of leprosy, ninety cases were found suffering with deformities. The influences of various host factors and disease factors, in causation of deformities have been discussed.

Drug compliance among self-motivated leprosy patients.

Regularity of DDS intake among 366 leprosy patients attending our out patient department voluntarily was assessed by urine spot test. It was found that only 54.6% of them had taken their last dose of drug within the previous three days. Those who kept their appointment showed better compliance than those who did not. Urinary DDS positivity was found to be unrelated to sex, occupation or the type of the disease. In the younger age group the compliance was low, as also among the patients coming from nearby places as compared to those who were residing in far off districts.
