Management of cancer in pregnancy: a case of Ewing's sarcoma of the pelvis in the third trimester.

Ewing's sarcoma of the pelvic bones was diagnosed in a 21-year childbearing woman, raising major medical and ethical problems. The diagnostic and therapeutic approaches during the sixth month of gestation were tailored in order to cure the patient and avoid unnecessary toxicity to the fetus. Ancillary tests included ultrasound and MRI studies of the pelvis. Ifosfamide and adriamycin, premedicated by granisetron, were administered during gestation, and were found to be safe. Cesarean section was the preferred way of delivery since the tumor involved the pelvic bones. The outcome was a disease-free patient and a small healthy baby who is now two years of age.

[Post-traumatic aneurysmal dissection of the extracranial internal carotid artery; helical computed tomographic and angiographic aspects: a case].

A 20-year-old man presented with mild intracranial bleeding, a Horner's syndrome, and left neck swelling following head injury. Following noncontrast CT of the brain, a contrast-enhanced helical CT was performed through the neck that showed a hematoma in the poststyloid space (carotid sheath) with irregular diameter of the ICA. Selective digital subtraction angiography confirmed the presence of left cervical ICA dissection with pseudoaneurysm formation. The aneurysm was resected and an end-to-end anastomosis was done using an inverted saphenous graft. Histology confirmed a diagnosis of traumatic ICA dissection with pseudoaneurysm formation and there was no evidence of pre-existing pathology. Helical CT is a simple, widely available, and relatively non-invasive imaging technique that correlates well with angiography. It should be considered in the evaluation of patients with suspected cervical ICA dissection.

Late presentation of solitary contralateral adrenal metastasis of renal cell carcinoma.

We report a case of renal cell carcinoma with solitary metachronous contralateral adrenal metastasis occurring 23 years after radical nephrectomy. The patient was treated with adrenalectomy. He is alive with no evidence of disease 1 year postoperatively. Solitary metachronous contralateral adrenal metastases from renal cell carcinoma are rare clinical conditions that may occur very late. Metastasectomy is advocated and is probably beneficial for limited metastatic renal cell cancer.

[Angiographic identification of spinal cord arteries before bronchial artery embolization].

Ischemic spinal cord injury is the major risk of bronchial artery embolization. The spinal artery may be overlooked on initial intercostobronchial trunk arteriography, as a result of reverse flow within the intercostal branch. Its identification, conversely, is easier on postembolization angiography. An illustrative case is presented, with angiographic correlation. The pathophysiology of the reverse flow is discussed. Technical recommendations are proposed for its detection.

[Foreign body granuloma mimicking bronchial tumor. Aspects in x-ray computed tomography with views by virtual endoscopy].

The aim of this study is to report the spiral CT findings of an endobronchial foreign body (chicken's bone) unknown on the postero-anterior chest radiograph and on the first bronchoscopy. The double originality of this case is the association with endobronchial actinomycosis mimicking a bronchial neoplasm and the utilization of virtual endoscopy by endoluminal 3D reconstruction in this context which has never been reported. The endoluminal 3D reconstruction of the bronchial tree could help guide the endoscopic procedure by better localization of the lesions.

[Failures and complications of bronchial artery embolization].

Bronchial artery embolization (BAE) is well accepted and widely used for management of massive and recurrent hemoptysis. Recurrent hemoptysis occurs in 20% of cases. It may be due to partial embolization, recruitment of other systemic collaterals, recanalization of an embolized artery, or progression of primary disease. Severe complications of BAE are limited to spinal cord injury, oesophageal necrosis, and bronchial ischemia. The proper application of bronchial arteriography and embolization techniques depends on a thorough knowledge of the arterial anatomy, a meticulous catheterization technique, the use of nonionic or lowosmolarity contrast materials, and adequate positioning of the catheter. In these optical conditions of safety, BAE is the treatment of choice for severe and recurrent hemoptysis.

[Retrospective analysis of 44 adrenal puncture biopsies under x-ray computed tomographic guidance].

Fourty-four CT-guided adrenal biopsies were performed on 43 patients aged 12-74 years old (mean 57 years). The results (benign adrenal tissue, malignant tissue, and nondiagnostic) were compared with outcomes. Diagnostic samples were obtained in 86% of patients. Among the 37 proved cases with diagnostic results (10 benign, 27 malignant), there were 26 true-positives, 1 false-negatives and 10 true-negatives. CT-guided adrenal biopsy had an accuracy of 82%, a sensitivity of 96%, a negative predictive value of 83%. In the 6 patients with nondiagnostic samples 1 mass proved malignant and two masses proved benign. Seven minor complications occurred in 7 patients (6 pneumothoraces and 1 retroperitoneal hemorrhage). CT-guided adrenal biopsy is a simple safe and well tolerated technique. In oncologic patients, obtaining benign adrenal tissue was highly predictive of benignity.

[Focal nodular hyperplasia and spontaneous intrahepatic porto-portal communication].

One case of typical focal nodular hyperplasia of the liver on CT but not proven at pathology was associated with a variation of the intrahepatic portal venous system. The absence of the horizontal segment of the left portal vein with portal supply between the anterior segmental branch of the right portal vein and the umbilical portion of the left portal vein was observed. This finding observed is significant for planning liver surgical procedure such hepatic lobectomy as the incision would interrupt the portal supply.

Motion artifacts of the aorta simulating aortic dissection on spiral CT.

PURPOSE: Motion artifacts of the ascending aorta may impair image quality and simulate an intimal flap or a false channel. The purpose of this study is to evaluate the prevalence, amplitude, and extent of motion artifacts of the aorta in spiral CT and to specify the effects of acquisition and reconstruction parameters on these artifacts. METHOD: One hundred seventy-one thoracic spiral CT examinations were retrospectively analyzed by two reviewers. The analysis sought to determine the presence, location, amplitude, and extent of artifacts of the ascending aorta. RESULTS: Aortic artifacts were detected on spiral CT in 57% of cases. The mean amplitude and mean extent were 4+/-4 and 6+/-7 mm, respectively. Artifacts in the ascending aorta were significantly higher with the 360 degrees linear interpolation (LI) algorithm than with the 180 degrees LI algorithm. CONCLUSION: The prevalence of motion artifacts on spiral CT is higher in this study than the reported prevalence in incremental CT. However, this higher prevalence is significantly reduced when the 180 degrees LI algorithm is used.
