Granulomas of the appendix: is it Crohn's disease?

An epidemiologic study was performed 1) to estimate the occurrence of epithelioid granulomas in the appendix wall among patients in whom an appendectomy had been performed, 2) to identify patients with granulomas in the appendix who had symptoms and history compatible with Crohn's disease, and 3) to estimate the long-term prognosis. Within a 5-year period 6051 patients in Copenhagen County underwent appendectomy. Six patients (0.1%) had epithelioid granulomas of the appendix (0.2 per 10(5) inhabitants per year). Follow-up of the six patients for 9-11 years (median, 9.5 years) showed that all had been free of gastrointestinal symptoms since the operation. Among the 373 patients diagnosed as having Crohn's disease in Copenhagen County between 1962 and 1987, 3 patients had their disease initially confined to the appendix. Follow-up in these patients showed no recurrence within a median of 6 years (range, 4-7 years). Patients with epithelioid granulomas of the appendix have an excellent long-term prognosis, which could be explained by the fact that the condition seems to be unrelated to Crohn's disease.

[The endocrine status of clinically conspicuous mares during the peripartum period].

The aim of the present investigations was to characterize the endocrine changes in the peripheral plasma during the periparturient phase of mares with a known history of obstetrical disorders. Blood plasma samples from 9 mares (8 mares during parturition, 1 mare during abortion) were collected and the following steroid hormones were radioimmunologically determined: progesterone (P4), dehydroepiandrosterone (DHEA), dehydroepiandrosterone sulfate (DHEA-S), estrone (E1), estrone sulfate (E1-S), estradiol-17 beta (E2), estriol (E3), cortisol and transcortin. In general, with the exception of cortisol, DHEA and transcortin, all steroid hormone levels declined significantly after parturition. Mean plasma concentrations of DHEA-S, the direct fetal precursor of estrone, were 6 times higher than DHEA. However, DHEA-S levels varied considerably between all mares as well as between daily values of the same mare. Levels of E1-S were found to be about thousand times higher than those of E1, E2, and E3. The rather low concentrations of E3 suggest that this hormone is mainly produced by peripheral conversion and that in the horse fetus the 16 alpha-hydroxylation of DHEA-S is lacking. Despite the fact that most of the examined mares showed signs of severe disease none of the investigated steroid hormones indicated any disturbances of gestation. It is concluded that, from the clinical point of view, serial determinations of E1-S plasma levels are the best way to prove the fetal well-being. However, it should be stressed, that even the E1-S values are no indicators for the onset of parturition or abortion.

[A quantitative ELISA for the measurement of class-specific anti-spermatozoa antibodies].

A class-specific ELISA for the quantitative determination of anti-spermatozoal antibodies is described. The intra-assay variation at the lowest standard concentration for anti-spermatozoal-IgG- and anti-spermatozoal-IgA-specific measurements was 6%. The test possessed an inter-assay variation at the lowest standard concentration in case of the anti-spermatozoal-IgG-determination of 9% and for anti-spermatozoal-IgA-determinations of 8.5%. With this ELISA 5.7% anti-spermatozoal-IgAs and 11.3% anti-spermatozoal-IgGs were identified in sera of a group of infertile females (N = 53). A collective of anti-spermatozoal antibody positive sera, identified by an anti-spermatozoal antibody ELISA measuring total immunoglobulins was subjected to an investigation with the class-specific ELISA. The proportion of anti-spermatozoal-IgAs (54%) was significantly lower than that of anti-spermatozoal-IgAs (78%). Interestingly, no correlation was observed between the parameters IgA and IgG in antispermatozoal positive sera (r = 0.139).

Purification and characterization of the pyruvate-ferredoxin oxidoreductase from Clostridium acetobutylicum.

The pyruvate-ferredoxin oxidoreductase from Clostridium acetobutylicum was purified to homogeneity and partially characterized. A 9.2-fold purification was achieved in a three step purification procedure: ammonium sulfate fractionation, chromatography on Phenyl Sepharose and on Procion Blue H-EGN12. The pure enzyme exhibited a specific activity of 25 U/mg of protein. Homogeneity of the pyruvate-ferredoxin oxidoreductase was confirmed by native polyacrylamide gel electrophoresis and sodium dodecylsulfate (SDS)-polyacrylamide gel electrophoresis. The molecular weight was determined to be 123,000/monomer. The subunit composition of the native enzyme could not be determined because of the instability of the pure enzyme. The pyruvate-ferredoxin oxidoreductase is sensitive to oxygen and dilution during purification. The dilution inactivation could be partially overcome by the addition of 300 microM coenzyme A or 50% ethyleneglycol. A thiamine pyrophosphate content of 0.39 mol per mol of enzyme monomer was found, the iron and sulfur content was 4.23 and 0.91, respectively. The pH-optimum was at pH 7.5 and the temperature optimum was at 60 degrees C. Kinetic constants were measured in the forward reaction. The apparent Km for pyruvate and coenzyme A were 322 microM and 3.7 microM, respectively. With 2-ketobutyrate the pyruvate-ferredoxin oxidoreductase showed 12.5% of the activity compared to pyruvate. No activity was found with 2-ketoglutarate. Ferredoxin from Clostridium pasteurianum could be used as physiological electron acceptor.

Isolated arteritis of the uterine cervix.

Isolated arteritis has the same histological picture as polyarteritis nodosa but affects the arteries of one organ only. Two cases in the uterine cervix are reported and a brief review of the features is given.

[Fertilization results in superovulated ewes and goats after transmural-intrauterine insemination controlled by laparoscopy].

A comparative experiment in oestrous synchronized and superovulated ewes and does is described on which fertilization was studied following natural mating or transmural intrauterine insemination by laparoscopy. Thirty-two ewes were inseminated into each uterine horn with 50-100 microliter of fresh undiluted semen, whereas 26 ewes were mated by fertile rams. Fertilization rates of eggs recovered 5-7 days later were 93.7% (163/174) and 60.5% (78/129) respectively. Eight does were inseminated by laparoscopy while 12 does were mated by fertile bucks. Numbers of eggs fertilized of those recovered 6-7 days later were 42/47 (89.4%) in the intrauterine inseminated group and 51/60 (85%) in the naturally mated group. These results indicate that high levels of fertilization can be obtained in ewes following intrauterine insemination with fresh semen with the aid of a laparoscope and that it is possible to overcome the reduced fertilization rates after natural mating. In contrast to the ewe the intrauterine semen deposition in does did not improve the high fertilization rates obtained after natural mating.

[Clinical aspects of ovary tumors in mares].

The present study comprises 31 mares, that showed a permanent unilateral enlargement of the ovary. In 29 patients the ovary in question was removed by a flanc laparotomy in the standing animal. In the histopathologic examination a granulosa cell tumor was diagnosed in 23 cases, a fibroma twice and a teratoma, hematoma, leiomyoma and a cystic ovary each once. Due to the endocrine activity of the granulosa cell tumor, the clinical picture was characterized by changed behaviour and atrophy of the contralateral ovary. Clinically the ovarial blastomas (teratoma, leiomyoma, fibroma) could not be differentiated from granulosa cell tumors. The postoperative prognosis of fertility is good. It should be noted though, that most of the mares did not foal until two years after the surgery.
