[Comparative study of 15 borderline tumors and 31 cancers of the ovaries. Epidemiologic, diagnostic and therapeutic aspects].

46 epithelial tumours of the ovary were seen over a period of 5 years. These tumours could be subdivided into 15 borderline tumours and 31 invasive cancers. An epidemiological study did not make it possible to find special factors in relationship to these two types. Studying the circumstances under which diagnoses were made demonstrated the value of considering: pain, ultrasound pictures and cytology from the Pouch of Douglas. It is essential to carry out laparotomy in every case of an organic tumour of the ovary. It is only by histological examination that it is possible to tell whether a tumour is borderline of invasive. A protocol for carrying out treatment is suggested. The results confirm the excellent prognosis for borderline tumours and the seriousness of other epithelial cancers of the ovary.

[Cervical dilatation using PGF2 alpha gel before voluntary interruption of pregnancy during the first trimester].

To avoid complications of instrumental dilatation of the cervix prior to first trimester abortion, pharmacological dilatation by prostaglandins is proposed. The authors present a prospective randomized study using intra-cervical PGF2 alpha-gel, in 90 patients. Two groups (PGF2 alpha: 5 and 2.5 mg) were compared with a placebo group. The mean cervical dilatation observed in the two groups (8.2 and 8.4 mm) is higher than the placebo group dilatation (6.8 mm), but statistically higher only in group 2.5 mg. Side effects were observed: uterine pain (83 and 57%) and nausea (33 and 13%) in the first three hours after the gel. With the same cervical dilatation, side effects were less important in the low dose group (2.5 mg PGF2 alpha).

[Pregnancy and injuries in traffic. Apropos of a case].

6.9% of women involved in accidents are pregnant. It is difficult to calculate all the fetal and maternal lesions that occur under these conditions. We are reporting a case of a 27-year-old primipara who had internal bleeding with shock without a precise aetiology. Very heavy vaginal bleeding occurred on the third day after admission so that a Caesarean had to be carried out in order to preserve the health of the mother and of the baby. Several complications can follow road traffic accidents in pregnant woman. The worst is usually fetal death and its mechanism is discussed. More frequently occurring, however, are fetal distress that threaten premature labour. Some lesions that are not lethal, such as bone fractures, can affect the long-term outlook for the pregnancy.

Sinusoidal fetal heart rate pattern in severe fetal anemia from feto-maternal transfusion.

A case is reported where a sinusoidal fetal heart rate pattern was found in a fetus at 34 weeks. This pattern, at first isolated and later persistent for several hours, during a normal pregnancy became progressively transformed into a silent pattern. Cesarean delivery then performed resulted in the birth of an infant affected by a severe anemia due to a major feto-maternal transfusion and it died aged 36 h. The authors enquire into the significance of this pattern when found in apparently normal pregnancies, not involving rhesus iso-immunization, and they suggest a possible management when confronted with a sinusoidal fetal heart pattern.

[A case of post partum gonococcal arthritis].

The authors report a case of gonorrhoea occurring immediately after delivery, characterized by acute inflammatory polyarthritis. The diagnosis was made from swabs taken from the lochia that were cultured on enriched media. The outcome of the disease treated with penicillin was good after 48 hours. The new-born's state of health after swabs take from the nostrils were positive was satisfactory all the time. A review of the literature shows how rare these conditions are, how serious they can sometimes be, and the management that should be undertaken in order to prevent complications occurring.

[Therapeutic abortion during the second trimester of pregnancy, using intracervical injection of PgF2 alpha (author's transl)].

Prostaglandins constitute the best means of inducing therapeutic abortion during the second trimester of pregnancy. The authors review the methods in current use and describe their own technique, which consists of dilatation of the cervix with laminaria and intracervical automatic pump infusion of Pg F2 alpha at the rate of 0.6 mg/h. The results obtained in 34 women with a mean total dose of 16.4 mg PgF2 alpha over 20 hours are given. The technique is considered to be as effective as those previously published.

[Prostaglandin and threat of premature labour inhibitors. 1. The possibilities and consequences (as given in the literature) (author's transl)].

The employment of tocolytics is a useful weapon in the treatment of threatened premature labour. The importance of prostaglandins in uterine contraction and the fact that non-steroid anti-inflammatory agents inhibit the synthesis of prostaglandins is well known. Some authors have shown that aspirin has an effect on the outcome of pregnancy (by prolonging it). Others have shown experimentally the tocolytic effect of indomethacin. Several publications have now shown that indomethacin is useful in the treatment of threatened premature labour. Maternal tolerance is good but one fetal complication has been described, namely the persistance of fetal blood circulation after birth. The physiopathology of this syndrome is reviewed and the role played by indomethacin defined. Finally, it would appear that indomethacin is a possible treatment for threatened premature labour.

[Clear cell vaginal adenocarcinoma in young girls. Apropos of a personal case].

A personal case which has been studied has given the authors the opportunity of analysing the characteristics of clear-cell adenocarcinoma of the vagina in a young girl. Adhesions are most often discovered in cases of metrorrhagia or discharge about the time of puberty. Thence as a friable tumour on the anterior wall of the vagina. Usually diethylstilboestrol has been given during intrauterine life. Histologically clear cell carcinomata of the vagina are characterised by a glandular structure in which large cells with large nuclei and clear cytoplasm as well as small cells with "tapestry-maker nail patterns" are found together. Treatment is surgical: total colpohysterectomy with conservation of the ovaries, with lymphadenectomy and with conservation of a vaginal cavity. Prevention consists in screening those who have been exposed to the risk and treating lesions that have been found, but above all in care in using estrogens in pregnant women.

[Fetal hypothyroidism: a complication of amniofetography. Apropos of 3 cases].

The authors present three cases of fetal hypothyroidism following amniofetography, after they have reviewed the technique and complications of this procedure. (For outlining the fetus by injecting radio-opaque iodides into the amniotic sac). These are cases of hypothyroidism in the fetus caused by overdosage with iodides. The fetal thyroid is blocked by the large quantity of iodine which is necessary for amniofetography. The indications then for amniofetography should be reviewed. The only valid indication that remains is for the diagnosis of neurological malformations and especially spina bifida. Whenever amniofetography has been carried out the newborn should be assessed for thyroid function routinely.
