The precision in measuring left ventricular pressure and its first time-derivative has been evaluated with the study of the physical caracteristics of a system currently used for pressure wave sampling. Besides the different catheters, the set-up tested consists of a nylon connector (110 cm) and of two three way stopcocks. Amplitude and phase response, from 1 to 60 Hz, have been obtained testing the system with sinusoidal pressure waves generated by an hydraulic oscillator. Despite mild differences dependent on the different catheters, the system amplifiers remarkably at frequencies above 5 Hz. Above this limit the harmonic content of the left ventricular pressure wave is negligible and can be ignored in defining the basic morphology and the absolute values of the curve. Similary, the amplification of these high frequency harmonics operated by the system does not affect appreciably pressure measurements. In the case of the pressure time-derivative the significant harmonic content is shifted toward higher frequencies. For this reason a reliable dp/dt cannot be obtained with this hydraulic set-up. A maximal hypothetic error, utilizing a Cook 5.2 F 105 cm catheter, of 8-9% and 28-31% has been estimated for pressure and time-derivative measurments respectively.

The effects of timolol alone and in combination with a fixed dose of hydrochlorothiazide and amiloride have been studied in a double-blind, controlled study in fifty-four patients with mild to moderate essential hypertension. 2. After a 4 weeks placebo period patients were randomly assigned to enter groups receiving timolol alone (group A), hydrochlorothiazide + amiloride (group B) or timolol + hydrochlorothiazide + amiloride (group C). Each treatment was carried out for 6 weeks. 3. The use of timolol (10 mg), hydrochlorothiazide (25 mg) and amiloride (2-5 mg) in a combination tablet given twice daily gave better control of blood pressure in patients with mild to moderate essential hypertension than did equivalent dosages of timolol alone or of hydrochlorothiazide and amiloride. 4. Clinical and laboratory side effects were minimal.
1. The effect of a low-sodium diet and pharmacological therapy has been compared in eighty-one patients with mild hypertension. 2. Both pharmacological therapy and a low-sodium diet reduced lying and standing systolic and diastolic blood pressure significantly.
A case of the Holt-Oram syndrome is described, that is, the association of a skeletal anomaly of the superior limbs, with a constant involvment of the thumbs, and an ostium secundum defect with a prolonged P-R interval. The case presented has the peculiarity of being sporadic, while from the literature a familiarity is almost constant via an autosomic dominant hereditary transmission. The more common syndromes were an alteration of the superior limbs is associated with a congenital cardiopathy are schematically underlined. Finally, the embriological reasons of this not rare association are briefly discussed.
