Ultrastructural abnormalities in muscular vessels of hyperthyroid patients.

An electron microscope study of needle biopsies from the quadriceps muscle was carried out in 11 non-selected patients (ten females and one male), with clinically and laboratory-diagnosed hyperthyroid disease. Alterations of the normal structure of muscle fibres were found in all cases. Changes in capillaries were found in ten patients, and ranged from an increase in basement membrane thickness with reduplication, to total destruction of the capillaries. The importance of the vascular involvement in the muscles of patients with Graves-Basedow disease is stressed.

Pharmacological profile of a novel carbacyclin derivative with high metabolic stability and oral activity in the rat.

A novel carbacyclin derivative (16S)-13,14-dehydro-16,20-dimethyl-3-oxa-18,18,19,19-tetradehydro- 6a- carbaprostaglandin-I2 (3-oxa-analogue) has been synthesized in order to find chemically and metabolically stable prostacyclin-mimetics with a potency equal or even superior to PGI2. The 3-oxa-analogue was found to be stabilized against beta-oxidation, a main metabolic degradation step also for chemically stable PGI2-analogues. The compound is orally available and displays a long duration of 4.5-48 h of antiaggregatory and hypotensive action. The 3-oxa-analogue inhibits ADP-induced platelet aggregation with an IC50 of 3.0 nM. Following intravenous application the 3-oxa-analogue lowers diastolic blood pressure in a dose dependent manner, the ED20 being 0.1-0.2 micrograms/kg after injection and less than or equal to 0.05 micrograms/kg/min after infusion respectively. In vivo platelet aggregation is inhibited after i.v. infusion of the 3-oxa-analogue with an IC50 of 0.037 micrograms/kg/min. As compared to Iloprost, the 3-oxa-analogue is 5-12 fold more potent with respect to in vivo hypotensive and anti-aggregatory effects. The results of the present studies indicate that the 3-oxa-analogue has a pharmacological profile comparable to prostacyclin (PGI2) and Iloprost. Due to the fact that the 3-oxa-analogue is chemically and metabolically stable, long term oral treatment can be achieved in clinical conditions in which PGI2 and Iloprost have already been shown to be therapeutically useful principles.

Pharmacological studies on the intrinsic sympathomimetic activity of the beta-adrenoceptor antagonist mepindolol.

1-Isopropylamino-3-(2-methyl-4-indolyloxy)-2-propanol (mepindolol, Corindolan) is a beta-adrenoceptor antagonist with significant intrinsic sympathomimetic activity (ISA): positive chronotropic effects in atria of the rat amount to 24% of those elicited by the full agonist isoprenaline. Relaxant effects in blood vessels approach 50% of those of isoprenaline. This vasorelaxant effect is completely blocked by the specific beta 2-adrenoceptor antagonist ICI 118 551 (erythro-dl-1-(7-methylindan-4-yloxy)-3-isopropylaminobut an-2-ol), suggesting that the vasodilatory effects of mepindolol are elicited by stimulation of vascular beta 2-adrenoceptors. In the anaesthetized cat mepindolol acutely lowers arterial blood pressure by reducing total peripheral resistance without exhibiting significant cardiodepressant action, whereas propranolol, lacking ISA, lowers blood pressure by a marked reduction of cardiac output and left ventricular contractility, however, total peripheral resistance is significantly increased.

[Diagnosis and therapy of iron deficiency anemia in pregnancy].

39 pregnant women, hemoglobin in serum less than or equal to 6,80 mmol/l were treated with Vitaferro, Folicombin and additional vitamins. Test groups were formed by means of randomisation. Hematological and clinicochemical parameters were measured before, as well as 2 and 6 weeks after starting treatment and the results were evaluated.--In the diagnosis of iron deficiency anemia in pregnancy serum iron concentration and iron-binding capacity are sufficient. The diagnosis is proved by an increase of the serum hemoglobin 2 weeks after starting therapy. For the total number of patients an increase of hemoglobin could be proved 2- and 6 weeks after starting therapy. However, the 3 chosen therapeutic methods did not show any differences in statistically proved values in form of therapeutic results. This is, in our view, due to given deviation and the specific character of the test group. During pregnancy iron deficiency anemia should be treated by doses more than 100 mg iron per day. Normalization of the hemoglobin under iron substitution is no indication for immediate interruption of the treatment.

[Results of colonoscopy in surgically relevant diseases of the large intestine].

Colonoscopy has greatly enlarged possible diagnostic, prophylactic, and therapeutic approaches to colonic diseases. The role played by this method for surgically relevant colonic diseases and as an element in precisely defined tumour aftercare programmes as well as for targeted screening of high-risk groups is outlined in this paper with reference to findings obtained from 491 colonoscopic examinations. Endoscopic polypectomy is also described as an important therapeutic approach to precancerous processes and early cancer forms.

Termination of a transcription unit comprising highly expressed genes in the archaebacterium Methanococcus voltae.

The 3'termini of transcripts originating from genes organized in a highly expressed transcription unit were analyzed in the archaebacterium Methanococcus voltae. The putative termination signals were found in an AT-rich intergenic region following the 3'-terminal gene. The two detected signals both contain oligo(T) sequences. A possible stem/loop structure immediately precedes one of the oligo(T) tracts. This secondary structure is considered to have an additional function in stabilizing the transcripts.
