Role of transoral irrigation in mediastinitis due to hypopharyngeal perforation.

Mediastinitis resulting from perforations of the hypopharynx is a life-threatening complication associated with a high morbidity and mortality. In cases of perforation, which are not amenable to primary closure, transoral irrigation has been found to be an effective means of therapy. This technique rapidly controls sepsis, favoring the closure of perforations of the hypopharynx and cervical esophagus. Using this technique we have had no mortality attributed to mediastinitis in patients with mediastinitis due to perforation of the hypopharynx, cervical and thoracic esophagus.

A less invasive approach to monitoring acute respiratory failure.

In a general ICU with a predominant surgical census, patients with acute respiratory failure can usually be treated on a clinical assessment and a minimum of invasive monitoring. An algorithm was used to manage the ventilatory support and weaning. When it failed (15% of the time), the patients' functional diagnoses were questioned and more advanced monitoring was used. The algorithm led to expeditious weaning of patients who did not need mechanical support and to a reduction of invasive monitoring.

[Comparative evaluation of chronic estrone toxicity when administered by different routes].

The paper concerns the results of studying chronic toxicity of estrone injected subcutaneously in the form of oily solution and vaginal suppositoria prepared on the basis of vitepsol. It has been shown that subcutaneous administration of the drug for 2 months results in dystrophic alterations in the liver, these alterations being more pronounced than those induced by intravaginal administration of estrone in the form of suppositoria.
