Enhancement of pharmacologic responses of pentobarbital by dopram.

The effect of Dopram (doxapram hydrochloride, A. H. Robins Co.) on the pharmacologic responses to pentobarbital was evaluated. In naive and pentobarbital-tolerant mice, Dopram was shown to enhance significantly sodium pentobarbital-induced narcosis in a dose-related manner. The effect of the duration of action of Dopram on pentobarbital narcosis also was assessed. It was observed that Dopram (40 mg/kg, i.p.) significantly increased pentobarbital-induced narcosis even when administered 2 hr prior to challenge with sodium pentobarbital (60 mg/kg, i.p.) A significantly increased hypothermic response to sodium pentobarbital was seen in Dopram-treated animals. The half-life of pentobarbital in brain and serum was shown to be increased significantly in animals receiving Dopram, 40 mg/kg, i.p. The waking brain and serum pentobarbital concentrations were not significantly different in either group. These studies show that Dopram potentiates pentobarbital's effects. Further study is necessary to determine the sites of operation and mechanism of this potentiation.

Butorphanol for the relief of shivering associated with extradural anesthesia in parturients.

STUDY OBJECTIVE: To assess the efficacy of butorphanol for the relief of shivering following the epidural administration of 2% lidocaine. DESIGN: Randomized, double-blind study. SETTING: Labor and delivery department of a university-affiliated hospital inpatient facility. PATIENTS: Sixty-one healthy labor patients. INTERVENTIONS: Patients who had sustained shivering associated with lidocaine epidural anesthesia were given normal saline or butorphanol 1 mg. Patients were observed for 20 minutes following the administration of a study solution. MEASUREMENTS AND MAIN RESULTS: Shivering ceased within a mean time of 12.9 +/- 3.8 minutes in approximately 81% of the patients who received epidural butorphanol (p < 0.01), while 3% of the patients in the placebo group had no shivering following the administration of epidural saline. No sedation or changes in fetal heart rate were associated with epidural butorphanol. CONCLUSIONS: Epidural butorphanol is effective in the treatment of postepidural shivering associated with epidural lidocaine. Epidural agonist opioids have been reported to be efficacious in the management of postepidural shivering. This study demonstrated that a partial agonist opioid also is effective in the treatment of postepidural shivering.

Women and specialty choice: why not anesthesiology?

The number of women in all medical specialties increased during the 1970s and 1980s. Anesthesiology residency programs experienced unprecedented growth from 1980 to 1986. We reviewed statistics published by the American Medical Association to investigate the relationship between the increase in female residents in all specialties compared to that for individual specialties including anesthesiology, pathology, surgery, pediatrics, obstetrics/gynecology, radiology, internal medicine, and psychiatry. We found an above average rate of growth for female residents in gynecology, pediatrics, and internal medicine; pathology, radiology, psychiatry, surgery, and anesthesiology showed a below average rate of growth. Studies have shown a correlation between gender and choice of specialty. Factors influencing the differences in choice and the decrease in the percentage of women choosing anesthesiology are unknown.

Epidural buprenorphine or morphine for the relief of head and neck cancer pain.

We present three cases in which epidural buprenorphine or morphine was used for intractable cancer pain of the head and neck. Excellent pain relief and minimal side effects offered by epidural opioids were of significant benefit. The use of epidural opioids prior to the administration of high doses of oral morphine may be the treatment of choice for pain from malignancy of the head and neck, especially when there is tumor extension or distant metastasis.

The potential for contamination of continuous epidural catheters.

In cultures from 102 patients undergoing continuous epidural catheterization, 22 catheters were found to be contaminated. No statistically significant correlation could be established between the contaminated catheters and the parameters monitored. A worrisome trend of contamination seemed to be developing in association with hospital-prepared epidural trays as well as the procedure of vaginal delivery. Hypotheses for these observations together with current prophylactic measures are discussed. One significant clinical infection occurred; a brief report of this patient is included.
